Summary: Monitors patient condition by observing physical and mental condition, intake and output, and exercise. Supports patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands
Essential Functions:
⦁ Performs HomeHealthAide duties in accordance with proper procedures and methods at specified time periods.
⦁ Provides home care services to individuals at home or groups at board and care homes or other institutions.
⦁ Provides non-medical personal care services, such as bathing, dressing and grooming, to individuals at home.
⦁ Focuses on the promotion of health, prevention of disease and care of the ill at home.
⦁ Escorts the ill/elderly to medical appointments as needed.
⦁ Occasionally provides housekeeping and domestic services to the ill/elderly.
⦁ Performs other duties as assigned.
Qualifications:
⦁ Must be a graduate of Self-Help for the Elderly's HomeHealthAide Training Class.
⦁ Must have a HomeHealthAide Certificate issued by the California Department of Public Health.
⦁ Must have current CPR & First Aid certifications.
⦁ Ability to communicate in English preferred.
⦁ Must pass a strength test for physical fitness for the job: must be able to lift items up to 30 lbs; able to move heavy and bulky items up to 50 lbs during the course of cleaning. Must be able to walk, sit and stand for 8 hours when required.
⦁ Must pass health screening for TB clearance and background check
⦁ Must be able to travel via public transportation or personal/department vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.

Summary: Provides hands-on personal care to patients and assists with activities of daily living in order to maintain the patient's health or to facilitate treatment of the patient's comfort, illness or injury.
Essential Functions:
1. Assists patients with personal care such as skin, mouth, and hair care and bathing. Assists patients with using bathroom, commode, bedpan or urinal.
2. Assists patients with getting in and out of bed/chair and assist with ambulation. Assists patients with prescribed exercises that have been taught by appropriate licensed health care personnel.
3. Performs other activities taught by a health care professional for a specific patient. These may include changing colostomy bags, changing of non-sterile dressings, taking of vital signs, and non-sterile bowel and bladder hygiene care.
4. The Aide shall not administer medications of any kind but shall report to the RN Case Manager any issues or concerns regarding medication management.
5. Prepares meals, including therapeutic diets, and assist patients with eating. Performs specific household tasks that will facilitate the patient's care at home.
6. Reports changes in the patient's condition and needs to the supervising nurse or therapist. Informs the supervisor of unusual or potentially problematic patient and family issues.
7. For Hospice patients, the Aide may also provide temporary respite and relief from total care responsibility for the primary caregiver.
8. Maintains accurate and timely documentation of all care delivered and ensures compliance with local, state and federal regulations.
9. Serves as translator for SHHCH clinical staff during patient visits when needed.
10. Attends departmental meetings, case conferences and/or patient interdisciplinary group meetings and participate in in-service training.
11. Ensures SHE and SHHCH policies are followed in the performance of duties.
12. Performs other duties as assigned.
Qualifications:
1. California Certified HomeHealthAide.
2. Current CPR Certification.
3. Ability to recognize the needs of patients, family members and patients with both limited and terminal illness and to maintain good relationship in dealing with them.
4. Bilingual in English and Cantonese.
5. Familiar with different patient transferring techniques. Must pass a strength test for physical fitness for the job: able to lift and transfer patient over 110 lbs.
6. Must pass health screening for TB clearance, physical examination and background check.
7. Must be able to travel via public transportation or personal/department vehicle for performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.

Specific Responsibilities:

Conduct comprehensive initial and follow-up assessments in clients' homes using motivational interviewing and evidence-informed approaches.
Identify service needs, goals, and barriers for both the individual with dementia and their caregiver.
Develop and implement individualized care and lifestyle plans that promote cognitive engagement, safety, and overall well-being.
Provide emotional, behavioral, and educational support to caregivers and families navigating the challenges of dementia care.
Offer companionship and structured engagement activities-such as attending community events, facilitating hobbies, or accompanying clients to appointments-to foster joy and purpose.
Lifestyle Coordination & Daily Life Management:
Manage daily logistics including transportation, grocery shopping, meal planning, and light household organization.
Coordinate schedules for paid caregivers, home services, and family routines to ensure seamless support.

Support transitions such as downsizing, moving to memory care, or adapting the home environment for safety and comfort.

Qualifications Required:

* Demonstrated experience in caregiver coaching, family support, or dementia care coordination.
* Exceptional interpersonal, organizational, and communication skills with a compassionate, flexible approach.

As the Community Liaison for Institute on Aging's home care division, Corio, your primary objective is to generate qualified referrals through strategic business-to-business (B2B) sales initiatives. Working collaboratively with IOA teams (Corio, Companioa, Connect, and Marketing), you will ensure that every client referral aligns with IOA's exemplary service standards. Through fostering community partnerships, engaging in public relations activities, leveraging events, social media, and executing marketing initiatives, you will champion the positive image of IOA. Additionally, you will play a pivotal role in facilitating the intake to enrollment processes for prospective clients, their fiduciaries, and referral sources.
Essential Job Functions:
Develop and execute a Strategic Account Plan for B2B community partnerships, nurturing relationships, tracking outreach activities, and following IOA's consultative sales process.
Cultivate and sustain relationships with referral sources to enhance brand awareness and increase enrollments for IOA Home Care (Corio) and Services for those living with Dementia (Companioa) programs. Ensure ongoing business satisfaction through routine follow-ups and relay feedback to program leaders.
Maintain comprehensive knowledge of Corio and Companioa service offerings, consistently conveying messaging through social media and community presentations.
Handle incoming inquiries, assess prospects' needs, provide information, and facilitate necessary follow-ups with team members.

Develop and maintain an effective network of community resources for client referrals, keeping Corio and Companioa team members informed.
Provide after-hours support to referral partners, prospective clients, and active clients as necessary.
Participate in weekly pipeline meetings to review prospects, devise enrollment strategies, and determine follow-up actions for B2B relationship management.
Participate in weekly or biweekly sales coaching meetings
Lead weekly business development meetings with Corio and Companioa teams, providing oversight to support outreach efforts.
Attend weekly Marketing and Sales Key Performance Indicator (KPI) meetings with the CMO and VP of home care and ensure departmental goals are met.
Actively engage in additional team meetings as required.

Qualifications and Skills:
Bachelor's degree preferred or equivalent experience in communications, business, gerontology, or a related field.
Previous experience in healthcare management, inside sales and/or outside sales or marketing, preferably in Homecare, Assisted Living, or a related field desired.
Demonstrated knowledge of the senior care industry.
Ability to work independently with strong organizational skills, assertiveness, and the capacity to prioritize tasks, manage schedules, and meet deadlines.
Effective relationship-building skills with internal and external clients and constituents.
Proficiency in applying consultative (needs-based selling) sales approaches.
Strong written and oral communication skills, including presentation abilities.

Summary: Provides personal care services, supervision and domestic housekeeping services, and implements activities for residents in accordance with state regulations.
Essential Functions:
All essential functions must be performed in accordance with State regulations and the manual of Operating Policies & Procedures of Self-Help for the Elderly Residential Care Facilities and other locations.
1. Personal Care and Supervision
⦁ Performs personal care services such as dressing, grooming, bathing, toileting, and transferring residents. Transferring may involve lifting residents from bed to chair or chair to toilet/commode.
⦁ Performs laundry services - washes, dries, and folds residents' clothes, sheets, towels, and other items as required.
⦁ Assists residents with medication.
⦁ Assists with routine and prescribed exercises or therapy.
⦁ Engages residents in daily activities and group exercises in accordance with the activity schedule; assists residents in performing exercises as needed.
⦁ Responds promptly to the buzzer system and provides appropriate assistance to visitors.
⦁ Escorts residents to medical appointments and field trips as required.
⦁ Observes and monitors residents' condition, including vital signs, and reports any changes to the supervisor.
⦁ Prepares reports on the condition of residents and notifies the Director promptly in case of emergency, unusual incident, or alleged abuse.
2. Food Services
⦁ Serves meals and snacks; assists residents who need help with feeding.

Qualifications:
⦁ High school graduate and adult care experience preferred. HomeHealthAide Certificate is highly preferred.
⦁ Ability to work with people of all socio-economic backgrounds, particularly the elderly population.
⦁ Familiar with different resident transferring techniques. Must pass a strength test for physical fitness for the job prior to employment and every 2 years thereafter: able to lift up to 30 lbs from floor to waist level. Ability to transfer residents of up to 120 lbs.
⦁ Must be able to withstand physical activities such as pushing, pulling, stooping, standing, walking, etc.
⦁ Good interpersonal skills with the ability to work well in a multidisciplinary team.
⦁ Ability to communicate with simple English. The Chinese language is preferred but not required.
⦁ Must pass health screening for TB clearance, and have or obtain criminal record clearance and First Aid Certificate.
