ABOUT THE ROLE
The Community-Based Mental Health Rehabilitation Specialist (MHRS) coordinates services and planning for children who are referred through social service, probation or mental health departments and are in the process of stepping down from institutional into family or family-like settings. Services will be provided almost exclusively in the community in families' homes, group homes, foster homes, schools, etc. The primary role is to ensure that all parties are working together toward stability for the child and family. MHRS engage clients in meaningful and authentic relationships while providing a safe, predictable, and positive environment that aligns with our clients' values and cultures.
ABOUT START
START (Short Term Access to Resources and Treatment) provides crisis stabilization and connection to longer term services. We are a small team and we work together closely to support youth and families facing significant challenges. Because this is a hybrid position, proactive communication is an incredibly important part of success in this role. Self-motivation and organization are also extremely important, as we ask staff to manage ever shifting priorities and ongoing documentation expectations independently.

RESPONSIBILITIES
Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalities
Coordinate with multi-disciplinary teams for the purpose of care planning and support
Collaborate with agency staff, caregivers, other professionals and interested parties to formulate and implement individual client care plans, behavioral programs, and interventions
Cultivate and maintain relationships with community members and providers, as needed
Complete assessment and evaluation process of clients in order to develop and implement the care plan
Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate
Chart mental health notes, care plans, outcome measures, incident reports, and other documentation accurately and in a timely manner
Maintain availability for crisis intervention services, such as on-call rotation, if applicable
Comply with all child abuse reporting and Medicaid/Medi-Cal requirements in accordance with standards set by Seneca
Maintain strict confidentiality of client and business operations information
Commit to and engage in self-evaluation as well as organizational goals, strategies, and systemic practices regarding diversity, equity and inclusion
Utilize crisis communication and de-escalation techniques as per Seneca's crisis response training
This may include intervening with the youths' behavioral program through physical management
QUALIFICATIONS
Education/Experience requirements:
Bachelor's degree + 4 years experience working with children/youth and families with complex and enduring needs in a mental health setting
Experience working with children in out-of-home care preferred
Own vehicle, Valid California Driver's License
Ability to travel throughout Contra Costa County
TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements
Bilingual in English and Spanish preferred

The Bilingual Peer Support Specialist provides peer support and advocacy to consumers of Seneca's services. Peer support may include one-on-one peer support as well as in group settings. The Bilingual Peer Support Specialist draws upon their own personal experience navigating systems to work individually with youth and/or family members to engage them in the process, increase their involvement, and assist them in achieving their care plan goals. Responsibilities:
Provide and communicate guidance, expertise, and support for youth, families, and/or any other natural supports one-on-one and in group settings.
Demonstrate high level engagement and alignment skills, to build trust and rapport with youth, families, and collateral supports.
Work as a “collaborative advocate,” helping youth, caregivers, and family members understand the mandates and perspectives of other team members while navigating through county systems.

Be part of a rotating 24-hour emergency on-call system, a minimum of two shifts a month scheduled with supervisor ahead of time.

Provide counseling and case management services to enrolled youth and families
Connect clients and families to community resources and natural supports
Offer crisis intervention and mental health services in various settings (home, school, community)

Our clients are impacted by housing instability, drug use, sever mental health issues, economic stress, and other complex stressors. They are often in acute crisis. The TAY FSP team works with other providers to bring quality services to our clients.
