Rochester Regional Health senior case manager job description

The Financial Case Manager - Senior is responsible for reviewing uninsured and under-insured patient accounts by following the insurance verification process.

Community/Acute - The FCM - Senior will assist under-insured patients by screening for secondary coverage options, applying for financial assistance and other government based programs. This person assists patients with setting up interest free payment plans and work closely with Patient Financial Services to help resolve coordination of benefits (COB) issues.

Long Term Care - The FCM - Senior will assist residents in applying for and recertifying in Medicaid coverage. This entails interviewing and screening resident and family for financial history, investigating and obtaining financial documentation needed from but not limited to banking institutions, financial markets including life insurance agencies, pension programs, and investment firms. This person will work closely with system and outside attorney agencies as well as the Department of Social Services.

All communication must be conducted in a manner that will result in positive patient relations and prompt reimbursement for services rendered. The FCM may also help in internal continuous improvement initiatives around financial performance. This individual works collaboratively with multiple departments of our organization.
MINIMUM REQUIREMENTS:
Associates Degree in related field with four years of experience preferred or an equivalent combination of work experience in financial counseling, long term care setting, healthcare billing or customer/patient service is required.Three or more years of Medicaid enrollment experience required.

RESPONSIBILITIES:
Assesses the healthcare coverage needs of uninsured and under-insured patients.Verifies insurance coverage to identify uninsured patients who may be eligible for insurance enrollment, and provides the FCM II accurate information for patient interview when needed.Assists under-insured patients with applying for secondary coverage, in acute setting financial assistance applications and other programs in which they be eligible.Complete all financial and secondary insurance applications with high quality work, gathers all necessary documentation and submit them to the proper agency / staff for processing.

The Case Manager Supervisor/Senior Case Manager will provide day to day supervision to the case management team within a 200+ bed women’s homeless shelter. He/ She will need to work collaboratively with other on-site departments to facilitate effective methods of problem solving and the alignment of strategies. Proof of COVID-19 vaccine required.

DUTIES & RESPONSIBILITIES:

  • Manages intake and develops a comprehensive assessment for all new participants.
  • Develops systems to guarantee the quality and efficiency of casework services.
  • Ensures that staff appropriately identifies the needs of the clients, provide appropriate referrals and ensures case manager follow up with resources to promote the program participant’s successful achievement of the service plan goals.
  • Ensures assessments, service plans and interactions with the clients are appropriately documented.
  • Reviews all required case management documentation and any data collection tool in use.
  • Responsible for the supervision of service staff.
  • Ensures clear, concise and professional interventions consistent with the Agency’s policies and procedures and evidence based practices.

Must have LMSW with 3 years of relevant experience and supervisory experience. Fluency in Spanish is highly desirable. Ability to work some evenings and occasional weekends as needed. Excellent and demonstrated writing ability. Strong assessment, direct practice, counseling, supervision, community outreach, group leadership and program development experience preferred. Advanced knowledge of MS Word, Excel and database management skills. Must have own car and valid NYS driver’s license required

FamiliesFirst Network of Lakeview (FFN) represents a partnership with the Department of Children and Families (DCF) to provide an array of foster care and related services in coordination with network partners. The network includes DCF, Lakeview Center (as the lead agency), judiciary, sub-contracted service providers, foster parents, the Circuit 1 Community Alliance, agency stakeholders, and the community working together to implement the legislative mandate for community-based care. Services include: case management for out-of-home and in-home placements; foster home recruitment, training, recommendation for licensure, and support; adoption support; an independent living program; dependency court resource facilitation; sub-contract management; and other services related to abused and neglected children and their families.

The senior CWCM's primary duty is the performance of work requiring advanced knowledge in the field of social work or human behavioral studies, and to use such advanced knowledge and prior work experience to consistently exercise discretion and independent judgement in providing the most appropriate services and support to families (1) in the Dependency Court system who have been identified by the court as needing professional services to prevent future abuse and/or neglect of children, and (2) who voluntarily request similar, but non-judicial in-home services. Requirements
