The physicaltherapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives. They will develop individualized treatment plans with patient-centered goals to reduce pain, restore function, and prevent disability, working collaboratively to prevent the loss of mobility.
Responsibilities
Perform patient evaluations through the process of obtaining history, relevant systems review, and administering specific tests and measures.
Establish an individualized plan of care and select treatment techniques addressing specific impairments identified in the evaluation and physician orders.
Conduct ongoing reassessment of the patient and their response to treatment.
Initiate and manage the discharge plan from the first visit through treatment.
Supervise and delegate to ancillary staff and students consistent with state laws and department policy.
Complete clear and accurate documentation within established timeframes in accordance with accreditation requirements and insurance regulations.
Communicate effectively with patients, families, caregivers, and other healthcare team members to promote maximum benefit of care.
Assess educational needs of patients and provide appropriate education to meet those needs.
Participate in professional growth and development through various educational programs and current literature.
Actively participate in quality improvement activities and provide a safe environment for patients, visitors, and staff.
Function within scope of practice as defined by state practice acts.
