Precepts new staff to support professional relationships with newly hired team members as required. Community Outreach Liaison Qualifications: Must have a bachelor's degree or equivalent experience in healthcare or business development Preferred two years in a sales/marketing role within the healthcare industry with a proven record of accomplishments in direct sales and marketing. Computer skills sufficient to properly document services and communicate effectively, CRM experience preferred. Ability to drive during daytime, nighttime, or inclement weather. Working at NYAP * Generous Time off: 22 Days of Paid Time Off + 11 Paid Holidays, Half Day Friday's during the summer! * Health and Wellness: Comprehensive healthcare packages for you and your family; Paid Parental leave * Professional Growth: CEU's, ongoing training/education, student loan repayment program, and supervision hours * And So Much More: 401K and 401K Matching flexible hours, mileage reimbursement, phone allowance Responsibilities * Provides in-home supportive services, transportation, parenting education, counseling, community resource referral, advocacy, and other social service interventions to the youth and family, meeting or exceeding established outcome thresholds on behalf of persons served. * Travels daily, to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements. * Identify resources and services in the community that may not be available through the internal service continuum. * Participate in the development of the youth treatment/service plans with the client, caregiver(s), referring agency, caseworker, significant family member(s), and natural support. * Regularly review the goals, progress, and activities of the assigned youth. * Actively and directly engage with the youth to meet service needs and goals/objectives of the treatment/service plan. * Interact with the youth to develop a trusting, supportive relationship while assisting the youth in achieving identified goals. * Provides safety assessment, crisis response, and behavioral stabilization services as required to assure safety and stability for youth served. * Complete all relevant training on a timely basis as required by state, contract, and/or licensing agency standards. * Complete all required documentation in a timely manner. * Perform other duties as requested. Minimum Qualifications * 21 years of age or older. * GED or High School Diploma required. Associate's Degree in social services with experience in child welfare is preferred. * 2 years' experience working directly with youth and families is preferred. * A willingness to work flexible and non-traditional hours with afternoon and evening availability. * Works well independently and as a team member. * Multi-task efficiently and be flexible in all situations. * Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.
