About the role:
The Women's Health Navigator at SMJH will act as the clinical lead for the LDRP and liaison with clinicians, nursing staff, nurse midwives and administration. Will assist in care coordination for patients requiring additional resources. This position does require some flexibility for evening hours for support groups and trainings within the Charlottesville, VA area.
The Women's Health Navigator is the entry access for women across the life span for Sentara Martha Jefferson Hospital which includes inpatient, outpatient and community services. Daily activities typically account for 60% of the Navigators' time include those duties required to ensure the ongoing operational and managerial elements for the program to run smoothly. These daily activities may include patient and family relations, physician relations, quality patient care through physical assessment, preventative health care, case management, consultation, and referrals.
Program development activities include the activities necessary to ensure the long-term success and growth of the program. Program development activities typically account for 40% of the Navigators' time and include: patient education, program development, patient satisfaction, continuous improvement and data collection, research and marketing. This integral position is designed to decrease fragmentation of care and coordinate services. The navigator creates a seamless flow through the health care system for the patient and family with family centered care as the focus.
LDRP:
Sentara Martha Jeffersons Women's Health includes L&D, and a 24 bed post-partum mother/baby. Our Level 2 newborn specialty care nursery has a 1 to 1 ratio while our intermediate care babies have a 1 to 3 nursing ratio. Our LDRP unit also offers post operative care for complex GYN and Breast surgical needs. This specialty area has a 100-member team cross trained to offer best in class healthcare to Charlottesville, VA, and surrounding areas. The schedule is primarily 12-hour shifts with rotating weekends and holiday requirements. + Acts as a resource person if the unit/division is unable to solve a staffing dilemma. Assists staff in utilizing staff to improve productivity and reduce agency usage.
+ Assigns bed placement for emergency admissions, direct admissions and transfers, considering patient comfort and need, unit staffing and workload. Obtains patient status from attending physician on direct admits
+ Initiates appropriate counseling and disciplinary action of employees in the Nurse Manager's absence in accordance with Human Resources and the Department of Nursing policies; this includes following policy regarding substance abuse.
+ Coordinates and performs patient transport form one area to another to expedite patient movement when needed. Utilizes resources to maintain patient flow and avoid diversion
+ Responds immediately to all codes, Code Atlas and Code Red to assist assess needs and take appropriate actions according to policy. Initiates snow and disaster plans when necessary.
+ Reviews and signs EMTALA forms to assure transfers are documented correctly.
+ Resource for Care Management. Assists with resolving issues of nursing home placement, transportation, adoption and DME on evenings, nights and weekends. Assists staff with accurately reflecting discharge disposition in computer.
+ Communicates with and assists supervisors in Housekeeping with patient flow and housekeeping issues. Priorities bed cleaning in a coordinated effort with the EVS supervisor.
+ Assists as needed to coordinate ancillary services to provide procedures for patients, i.e., Cath Lab, OR, CT, etc.
+ Procures equipment, supplies and patient nourishment in the absence of ancillary personnel. Utilizes Agility tracking for equipment needs. Transports remains to morgue and replaces the code cart.
+ Resolves complaints as expediently as possible, communicating incident and resolution to appropriate management and/or notifying the administrator on-call. Acts as Patient Advocate on off shifts.
+ Utilizes Risk Management as resource in resolving patient care issues.
+ Provides review of protocols for Blood, TB and Flu exposure. Functions in place of Employee Wellness and Infection Control RN on off shifts.
+ Utilizes a problem solving approach when dealing with conflict resolution between staff members, medical staff, visitors or other hospital employees.
+ Serves as administrative resource for staff, physicians, patients, and families in accordance with Hospital policies and procedures.
+ Provides support to families in conjunction with Pastoral Care and Patient Advocates.
+ Notifies appropriate person of unusual hospital occurrence; e.g., patient's concerns, personnel conflict and any other emergency situations.
+ Demonstrates the knowledge and skills necessary to provide care appropriate to the age and needs of the patients served on his or her assigned unit. The Medical Assistant is a multi-skilled clinical professional who works within the scope of practice to assist in patient care under the provider's direction. Essential Job Functions
Under the direction of the provider, contributes to the completion of the patient's chief complaint (including but not limited to respirations, blood pressure, pulse oximetry, height, weight, and temperature) and documentation of patient needs. Float associates will float to various urgent care centers within the market as determined by leadership. May continuously be rotating between centers within the market to assist with periods of high volume, new center openings, and seasonality.
Maintains clinical and administrative skills to meet patient care and daily operational needs. Collect patient specimens and run point of care lab tests. Travel to other facilities for BSMH (Bon Secours Mercy Health) providers or adjusting work hours to meet patient care needs as directed by the manager may also be .
Maintains a safe and supportive environment by keeping the work area, exam rooms, and equipment in clean, orderly, and safe manner, ensuring availability and proper functioning of supplies and equipment.
Within the Medical Assisting scope and state guidelines, administers ordered medications and/or vaccines via oral, injection, topically, or as directed. Collect patient specimens and run point of care lab tests.
Documents in EMR (Electronic Medical Record) accurately and appropriately. Manages My Chart requests and incoming patient calls. Manages in-basket messages under the Provider's instructions.
Other duties include ordering supplies, lab, and diagnostic tests, disposing of contaminated supplies, patient registration, scheduling, phone triage, cash handling, check-in/check-out and performing scribing duties as needed.

Patient Care Technician - Inpatient Rehab Unit

City/State Virginia Beach, VA Work Shift First (Days) Sentara is currently hiring a Patient Care Technician to work at Sentara Virginia Beach General Hospital on the Inpatient Rehab Unit. Hours/Shift: Part-time, dayshift, approximately 7am to 7pm
Inpatient Rehab (IPR) is an 8-bed unit specializing in providing patients with additional rehabilitation needs for 2-3 weeks when they are not independent enough to go home but do not require a skilled nursing facility. We work closely with physical therapy, occupational therapy, & speech therapy teams while providing care to these patients. We become familiar with our patients and their routines over this longer stay. We enjoy watching our patients progress to going home and that is rewarding as a patient care technician. The patient population includes but is not limited to stroke, spinal cord injury, patients that required amputation or other orthopedic patients that require more therapy after being medically cleared from the hospital.
Patient Care Technicians are unlicensed staff members who are accountable to, and work under the direct supervision of a professional nurse to implement delegated aspects of nursing care. Provide patient care, support the admissions and discharge process, and comply with patient safety practices under the direction of the professional nurse. Observe patients' physical, mental, and emotional conditions and report any change to the nurse.

Education
High School Diploma or Equivalent
Certification/Licensure
American Health Associate BLS Certification required within 90 days of hire.
Minimum requirements: Must have one of the following
Sentara Care Partner Certification Program, or
Successful completion of the Certified Nursing Assistant (CNA), or
National Certified Medical Assistant (NCMA), or
National Patient Care Technician (NCPCT), or
Completion of US Navy Hospital Corps School, or
Emergency Services to include EMR, EMS, EMT, AEMT and Paramedic, or
Current student enrolled in a clinical program (beyond prerequisites) having completed at least one semester of RN, LPN, RT or PT, or
Current work experience within last three years as a PCT or Nursing Assistant.

One year of prior health care experience preferred.

Certified Medical Assistant - Bon Secours Urgent Care - Culpeper
The Certified Medical Assistant - Urgent Care is a multi-skilled clinical professional that provides indirect and/or direct patient care within the scope of practice and in alignment with the standards of excellence and quality. Under the direction of the provider, the Medical Assistant contributes to the completion of the patient's chief complaint (including, but not limited, to respirations, blood pressure, pulse oximetry, height, weight, and temperature), documents patient/family needs, participates in planning and implementing care for patients, and administers ordered medications (oral, injection, topically, etc.) within the Medical Assisting scope and state guidelines.
Essential Job Functions
Understands and adheres to the legal responsibilities and requirements within the Medical Assistant role.
Identifies significant changes in patient condition through data collection and reports them to the provider.
Troubleshoots issues and escalate problems to provider, direct supervisor, or appropriate internal resource.
Demonstrates standards of excellence in care in all interactions, for both internal and external customers.
Maintains clinical and administrative skills per the departmental competency guidelines to meet patient care and daily operational needs.
Show patients to examination rooms and prepare them for the physician.
Travels to other facilities for BSMH providers or adjusts hours to meet patient care needs as directed by the practice manager as needed.
Maintains a safe and supportive environment by keeping the work area, exam rooms, and equipment in clean, orderly, and safe manner, ensuring availability and proper functioning of supplies and equipment.
Assists patient and provider with virtual health visits which may include setting up the visit through appropriate platform, pre-charting the virtual visit prior to the scheduled appointment, contacting the patient to ensure preparedness for the virtual visit, troubleshooting virtual visit issues when needed, obtaining chief complaint and health maintenance checks, medication review, and other relevant patient details prior to the actual virtual visit.
Documents in electronic medical records (EMR) accurately and appropriately.
Manages in basket messages in the electronic health record (EHR) under the Provider's verbatim instructions.
May perform front desk workflows (i.e., My Chart requests, patient registration, scheduling, phone triage, cash handling, and check-in/check-out).
Follows up on orders, appointments, referrals, and follow up visits and collects co-pays as needed.
Other duties include, but not limited to, ordering supplies appropriately, tracking referrals, labs, and diagnostic tests, disposing of contaminated supplies, etc.
Actively participate in ongoing training per Bon Secours Mercy Health and market recommendations.
Licensing/Certification
Active Medical Assisting certification from one of the following (required):
Certified Medical Assistant (CMA); American Association of Medical Assisting, Registered Medical Assistant (RMA); American Medical Technologists, Certified Clinical Medical Assistant (CCMA); National Healthcareer Association (NHA); Nationally Registered Certified Medical Assistant (NCRMA); National Association for Health Professionals, National Certified Medical Assistant (NCMA); National Center for Competency Testing (NCCT)
If incumbent is unable to obtain MA certification, an Assessment-Based Recognition in Order Entry (ABR-OE) is acceptable (not accepted in the state of South Carolina)
BLS Basic Life Support, American Heart Association (preferred upon hire, required prior to independent patient care)
Education
High School/GED (required)
Completion of an accredited Medical Assistant post-secondary education program (preferred in all states except South Carolina)
In South Carolina (completion of one of the below is required
Work Experience
Completion of externship or clinical lab training (preferred)
1 year of recent Medical Assisting experience (preferred)
Skills
Record patients' medical history, vital statistics, or information such as test results in medical records.
Interview patients to obtain medical information and measure their vital signs, weight, and height.
Prepare and administer medications as directed by a physician.
Collect blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing.
Provide authorized prescription and drug refill information for pharmacies as directed by provider.
Explain treatment procedures, medications, diets, or physicians' instructions to patients.
Clean and sterilize instruments and dispose of contaminated supplies.
Perform routine laboratory tests and sample analyses.
Perform general office duties, such as answering telephones, taking dictation, or completing insurance forms.
Successfully completes skills or competency checklists to be able to help physicians examine and treat patients, performing such tasks as handing them instruments, giving injections, removing sutures, changing dressings on wounds.
Strong oral and written communication skills
Ability to collaboratively work with patients, families, and teams within a high-volume environment.
