Job Summary:
The RN Care Manager is responsible for providing coordination of care for patients to support safe, seamless, timely transitions across the continuum. This role utilizes a collaborative process, the RN Care Manager identifies (using quantitative and qualitative methods), assesses, plans, implements and evaluates the options and services required to meet an individual's health and health related needs, including social- determinants that affect ones' overall wellbeing. Essential Functions:
* Identifies and prioritizes patients in need of care management services, using a holistic approach inclusive of biopsychosocial, functional, cultural, spiritual, and financial factors. Plans with the patient, caregivers and members of the healthcare team to maximize health care responses, quality and cost-effective outcomes. Monitors and revises the plan as indicated when patient condition changes.
* Completes all necessary documentation. Maintains, clear, concise, and timely documentation in the patient record to reflect the needs of the patients. Handovers are expected to be utilized at points of level of care change, staff change, as well as care transitions.
* Documentation will reflect plan of care to address post hospital care needs and resources and evidence of patient, family or caregiver involvement in planning. Ensuring patient's and caregiver's treatment goals and preferences are incorporated into the transition of care planning and communicated to the multidisciplinary team.
* Follow standardized practices and process related to Advance Care Planning, Length of Stay management and readmission prevention.
* Supports denial prevention related to medical necessity through addressing / removing barriers to progression of care and participating in Interdisciplinary Discharge Rounds.
* Supports and promotes assertive, proactive care for patients, assisting in removing barriers related to achieving timely testing and treatment. Ensures resources are utilized appropriately and offering alternatives to acute care to the care team.
Education:

* BLS Basic Life Support - American Heart Association (required)
* RN License in the state in which they are working or covered by compact (required)

The Associate Health RN provides services for associates according to BSMH policies and procedures, professional standards, federal regulations, and CDC recommendations. **Collaborates with: Associate Health Medical Director, Infection Prevention to prevent the spread of communicable disease between patients and staff, Wellness team for referrals and consultations.**
**Participates in direct service delivery, including but not limited to immunizations, TB screening, color discrimination testing, respirator fit testing, communicable and blood borne exposure management. Organizes testing and immunization programs to include Hepatitis B, influenza and respirator fit testing.**
**Collaborates with managers and Absence Management Specialists for needs related to leave of absence to include FMLA, MLOA, PLOA and ADA leave as accommodation.**

In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, and safely demonstrates basic knowledge of the contemporary nursing practice
Provides episodic (outpatient) and inpatient (at designated units) pediatric nursing care where applicable
Applies knowledge of contemporary Pediatric nursing practice and future innovations
Utilizes nursing practice to the care of pediatric patients that require nursing assessment and interdisciplinary team interventions
Evaluates, updates and revises plan of care to facilitate achievement of planned and expected outcomes
Administer medications in a safe manner consistent with the State of Practice and the organization's policies and procedures

Education:
* Bachelor of Science Nursing (preferred, not required)
Certifications:
* Current state licensure as a RegisteredNurse (RN)
* BLS Basic Life Support or PALS Pediatric Advanced Life Support (preferred at hire, must obtain from approved source prior to direct patient care, PALS required within 6 months of hire based on departmental requirements)
Experience:
* One (1) year of professional experience practicing as a RegisteredNurse (RN)
* Recent Medical/Surgical, NICU, Pediatric, OB, ED or critical care experience (preferred, not required)
Training:
* EPIC Electronic Health Record training (preferred, not required)

The RegisteredNursing (RN) is responsible for performing a competent level of nursing care as demonstrated by the critical thinking model known as the nursing process. The standards of practice (assessment, diagnosis, outcomes identification, planning, implementation, coordination of care- teaching and health promotion and evaluation) along with the standards of professional performance (ethics, culturally congruent practice, communication, collaboration, leadership, education, evidence- based practice /nursing research, quality, evaluation, resource utilization, and environmental health) encompass the actions and foundation of professional nursing. The RN possesses clinical knowledge and skills to meet standards as required by specific clinical areas.
Transition to Practice/Nurse Residency Program
This 10-week TTP program focuses on the orientation transition experience with intentional knowledge support and skill acquisition for practice in Medical-Surgical areas. For new graduates, this TTP program is the start of a comprehensive 6-month residency program to support your transition to practice.
Program includes:
Formal delivery of fundamental nursing concepts and standards with in-person sessions utilizing case studies, gaming, simulation, and skills labs
Exposure to Sentara policies and evidence-based practice guidelines
Hands-on experience alongside a trained nurse preceptor
Opportunity to share your experiences/ networking with peers in a safe, protected environment
