The Service Coordinator plays a key role in supporting service operations, serving as the central administrative link between residents, service teams, and vendors. The Service Coordinator works under the direction of the Service Manager and collaborates with Lead Technicians, ensuring that work orders are triaged correctly, residents receive timely updates, and vendors complete services efficiently. ESSENTIAL DUTIES
Coordinate and manage service requests by accurately entering, scheduling, updating, and closing work orders, while assigning technicians and vendors for efficient routing and timely service.
Serve as the primary point of contact for residents, managing service requests, scheduling, and follow-ups via phone, email, and resident tickets.
Respond to and manage resident service tickets, addressing resident inquiries, escalating urgent issues, and ensuring timely follow-ups.
Collaborate cross-functionally with service teams and property operations to ensure a seamless resident experience and timely issue resolution.
Communicate with vendors and service teams, confirming work order assignments, verifying work completion, and resolving scheduling conflicts.
Monitor work order progress and follow up on outstanding service requests, escalating as needed to the Service Manager or Lead Technician.
Use Power BI and other reporting tools to track service trends, assess response times, and provide insights on operational needs.
Identify scheduling gaps, delays, or inefficiencies, bringing them to the attention of the Service Manager and/or Lead Technician.
Maintain accurate records in work order systems, ensuring compliance with company policies and reporting requirements.
Support the Service Manager and Lead Technicians with data entry, performance tracking, and service-related administrative tasks as needed.
Additional duties may be assigned as needed.
WORKING CONDITIONS
Office-based role with a focus on high-volume resident communication and work order coordination.
Frequent phone, email, and system interactions, requiring strong communication and multitasking skills.

REQUIRED EDUCATION AND EXPERIENCE

The Aftercare Coordinator will act as a liaison between the SSVF grantee and the VA or community medical provider by enhancing community-based suicide prevention efforts to meet the needs of Veterans and their families through outreach and suicide prevention services. The Aftercare Coordinator will provide outreach in accordance with an approved plan to engage Veterans in the community identified as experiencing certain health, environmental, and historical risk factors that may contribute to suicidal ideation and/or self-injurious or lethal behaviors. The Aftercare Coordinator works collaboratively with SSVF Program staff, VA staff, the Veterans medical team and other community resources to meet the needs of eligible Veterans and their families. SSVF Aftercare Coordinators provide services that include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care with the focus of preventing suicide. SSVF Aftercare Coordinators provide a case management function and care coordination, health education, interdisciplinary collaboration, coordination, and consultation. The SSVF Aftercare Coordinator works closely with the Veteran's assigned multidisciplinary team, including medical, nursing, and administrative specialists, and case management personnel. The SSVF Aftercare Coordinator works within this team to provide timely, appropriate, Veteran centered care equitably. The SSVF Aftercare Coordinator works collaboratively with the team and the Veteran to identify and address system challenges for enhanced care coordination as needed. + Identify adverse events required by the manufacturers and notify the clinician ensuring all documentation requirements are met.
+ Review discharge work instructions to ensure all attempts have been made to service the patient prior to discharge; attempts include but are not limited to contacting the patient and the prescriber.
+ Follow up with the patients, prescribers, internal associates, and others to provide or obtain information needed.
+ Support additional therapy programs as they arise which may require additional documentation and tasks.
+ Participate in any variety of meetings and work groups to integrate activities, communicate issues, obtain approvals, resolve problems, and maintain specified level of knowledge pertaining to new developments, requirements, and policies.
**Minimum Requirements:**
+ High School Diploma or GED and minimum 2 years of experience in pharmacy environment.
+ Current, valid, active, and unrestricted State Pharmacy Tech. Certification or national certification based on applicable state(s) required.
**Preferred Skills, Capabilities, and Experiences:**
+ Previous experience working in specialty pharmacies is highly preferred.
+ Experience in a call center environment is preferred.
+ National Pharmacy Technician Certification (CPhT) is preferred.
