IRF-PAI Coordinator Full Time
Coordination of the IRF-PAI completion with the interdisciplinary team, gathering and input of data, transmission of data to Medicare. Assisting with education of staff regarding PPS and FIMS

Makes independent decisions based on clinical knowledge and training
Coordinate PPD data collection with interdisciplinary team.
- Ensure completion of supportive documentation for FIMs in medical record.
Completes admission and discharge IRF-PAI within CMS designated time frames.
- Collect appropriate information from medical record and summarize on IRF-PAI.
- Ensure completion of admission PAI within four days of admission.
- Ensure completion of discharge PAI within five days of discharge.
Inputs data into CMS accepted program within designated time frame.
Updates IRF-PAI log and maintains its accuracy.
- Check patient location roster and verify accuracy of IRF-PAI log.
- Ensure completion of all columns of IRF-PAI log with each discipline
Assist in team education regarding FIM/PPS and documentation.
- Complete FIM competency.
- Assist in performing FIM competencies with other team members

You will be working with adults over 21 years old to help them live as independently as possible through support services. Support Coordinators (SC) develop each person's Individualized Service Plan (ISP), which details their needs and services. Provide ongoing coordination of services for a caseload of 30 individuals enrolled in DDD, with the ability to add more to the caseload as able
Create and maintain Individual Service Plans.
Assist individuals and families to manage their DDD budget funds.
Collaborate with service providers to ensure individuals are connected to supports.
Conduct annual home visits and quarterly face-to-face visits to assess and address needs.
Maintain consumer contacts, detailed and accurate service records, and case files.
Provide outreach to local resources and services to make appropriate referrals.
Maintain state and federal documentation requirements and deadlines.
Agency on-call rotation after one year.
Other duties as assigned.
Required Education and Experience:
Minimum of Bachelor's degree.
One year of experience with people with intellectual and developmental disabilities.
Fingerprinting, Federal, and State background checks are required.
Must have own transportation, a valid driver's license, and insurance as travel is required.
