S:US is seeking a LPN for their adult family shelter in Manhattan.
Schedule: Tuesday-Saturday (9am-5pm) with one late shift of choice (11am-7pm)
I. SCOPE OF ROLE: Diagnosis and management of acute, chronic and complex health problems; assessing, diagnosing and
treating illnesses, educating the shelter clients and staff on disease prevention. Provide health
education and counseling to individuals residing in the shelter.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Meets with Program Director, Director of Social Services, Psychiatrist, Program Nurses, and Social
Service staff to coordinate shelter residents to receive ongoing treatment in the community.
Supervise Medication Coordinator and assist Psychiatrist and Psychiatric Nurse Practitioner as
needed.
Administer medications and document administration as required.
Take vital signs, such as blood pressure, temperature and weight
Assist with diabetes medication and treatment
Take medical histories and enter information in computer systems
Coordinate with Case Manager, Senior Case Manager and Director of Social Services to ensure proper
care, nutrition and treatment plans are followed.
Follow up with community health care providers to ensure medical treatment plans are documented,
monitored and followed
Monitor MARs and medication storage.
Work with residents and Case Managers in filling prescriptions and ensuring refills are ordered as
necessary.
Ensure consumer safety, assess consumer reactions to medications, assist with consumer nutritional
needs.
Ensure physician orders are current, properly documented and communicated to staff
Monitor client medical equipment for safety and proper functioning.
Attend and participate in regular staff training and scheduling meetings.
Maintain confidentiality and privacy regarding all clients' information.
Complete daily documentation of all agency required material.
Complete incident reports in accordance with policy and procedures.
Understand and implement the agency's philosophy, policies and mission.
Understand and follow the agency chain of communication.
Ensure the safety and well‐being of the clients on a daily basis.
Work cooperatively with other team members to carry out the program goals and objectives for each
client.
Report any actual or suspected abuse/neglect in accordance with policy and procedure.
Prepare clients for self‐medication status and monitor as needed, health assessment referrals.
Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance
Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
Maintain CPR/First Aid Certification.
Assist in building evacuation during fire drills and building emergencies.
Refer residents to emergency care if needed, work with emergency responders/emergency room staff
to coordinate emergency treatment, coordinate treatment and discharge planning with hospital
staff.
A schedule that includes evenings and weekends will be required.
Qualifications
REQUIRED QUALIFICATIONS & SKILLS
Completion of accredited program and licensed in New York as an LPN.

Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences. Bilingual preferred.

Job Summary
The LicensedPracticalNurse (LPN) - School Based delivers quality health care services to youth in a day treatment school setting under the direction and supervision of the Registered Nurse (RN). This position performs tasks and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care.
This full-time (40 hours) position is on a Monday-Friday school schedule, 7:30am-4:00pm. It will primarily be based at our Halpern Education Center in Webster, but also support our Day Treatment program in the school at our Golisano Campus at Crestwood on the edge of Rochester/Scottsville at least 1 day/week.
Essential Job Functions
Administer medications safely and accurately and per medication policy. Report any unusual patient reactions to a medication to the RN immediately.
Document progress notes in the electronic medical record and any other required system clearly, accurately, and timely meeting professional and agency standards to ensure accurate child and youth records are maintained.
Order and maintain inventory of all stock medications and medical supplies.
Administer basic first aid and CPR in emergency situations.
Provide ongoing medication, wellness and health education to youth.
Facilitate and assist in scheduling youth for appointments as directed.
Independently complete commitments and take responsibility for actions; recognize and fix errors and omissions.
Keep abreast of and adhere to all applicable safety regulations, including universal precautions and hazardous material safety rules.
Support process improvement initiatives in collaboration with leadership.
Understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to all federal, state, and local statutes, regulatory agency standards, and Hillside policies, including but not limited to, Office of Mental Health (OMH), Office of Child and Family Services (OCFS), Council on Accreditation (COA) and New York State Education Department (NYSED).

Education & Experience
Successful completion of an accredited LPN program required.

TITLE: Registered Professional School Nurse
TERM OF EMPLOYMENT: Substitute, Per Diem
REPORTS TO: Building Principal
DISTINGUISHING FEATURES OF THE CLASS: This is a professional nursing position involving responsibility for giving care to students and for performing related nursing services such as pupil health screening and notification of defects and communicable disease control requiring judgment and skills. This position differs from the certified position of School Nurse-Teacher in that there is no responsibility for either classroom instruction or guidance involved.
TYPICAL WORK ACTIVITIES:
The typical work activities listed below, while providing representative examples of the variety of work assignments in the title, do not describe any individual position. Incumbents in this title may perform some or all of the following, as well as other related activities not described.
Assists the School Physician in physical, visual and auditory screening examinations of students;
Administers first aid and emergency treatment to students;
Prepares and maintains health records for school authorities;
Assists at school immunization clinics;
Arranges to transport sick or injured students to hospital, home, doctor's or dentist's office in cases of emergency;
Inspects the school plant, playground and cafeteria and reports on general safety and sanitary conditions;
Orders, inventories and oversees the storage of first aid and related health supplies and equipment;
Consults with attendance teachers, staff members and school nurse-teachers concerning a variety of health factors related to non-attendance and communicable diseases;
Prepares records and reports as required.
FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES, AND PERSONAL CHARAC-TERISTICS: Good knowledge of nursing principles and techniques and their relation to medical practice and skill in their application; working knowledge of materia-medica, dietetics, sanitation and personal hygiene; skill in the application of nursing techniques and practices; ability to understand and follow technical, oral and written instructions; ability to keep records and make reports; ability to get along well with students, teachers, parents and others; ability to carry out successfully the measures prescribed; ability to plan and supervise the work of sub-professional and non-technical personnel; good observation; mental alertness; firmness; initiative; cheerfulness; patience; emotional stability; sympathetic attitude toward the sick; good moral character; good physical condition.
MINIMUM QUALIFICATIONS: Graduation from a school of nursing approved by the State Education Department from a course approved by the State Education Department from a course approved by such departments as qualifying for Registered Professional Nurse.
SPECIAL REQUIREMENT FOR ACCEPTANCE OF APPLICATIONS: Eligibility for a license issued by the State of New York to practice as a Registered Professional Nurse. Possession of the license at the time of appointment.

Position Summary:
Reporting to the Registered Nurse will provide for medication dispensing and nursing assistance in the diagnosis, and continuous primary medical care to CCA participants. Will act and function as the HIV Coordinator of services for the clinic.

Responsibilities:
Assists the Registered Nurse in the performance of the Registered Nurse's duties.
In the absence of the Registered Nurse the LPN will perform all Registered Nurse Responsibilities to assure the continued operation of the clinic.
Verifies the identification and observes the behavior of patients to whom medication or other medical services are provided.
Will act as the HIV Coordinator for the clinic. This includes the responsibility for tracking the HIV patients for medical care.
Must adhere to all procedures referable to accounting for medication dispensed to patients and reporting any deviations immediately to the Registered Nurse.
Maintains the medical charts, the medical unit appointment book and daily visit logbook.
Performs the appropriate independent nursing follow-up care of patients.
Assists in the identification and recall of patients for follow-up medical and nursing care. This includes the monitoring of urine toxicology.
Assists in the performance of admission and physical examinations.
Provides the physician or physician assistant with urine toxicology and other laboratory data as they become available.
Attends on a punctual basis clinic meeting.
Performs any and all tasks necessary to improve the quality of care, to improve the productivity of medical staff, and to improve participants satisfaction with the medical/nursing care they receive.
Performs any and all tasks necessary to enhance reimbursement and reduce denials for services rendered.
Performs phlebotomy, EKGs, vital signs and assists in urine collection.
The schedule is predictable with no overnight shifts
Qualifications:
LicensedPracticalNursing state certification required:
Bachelor's Degree preferred.
Three (3) years of nursing experience is preferred.
Excellent computer skills and working knowledge of electronic medical records required.
Excellent interpersonal skills with patients and co-workers.
Strong teamwork capabilities.
