ESSENTIAL DUTIES & RESPONSIBILITIES: Engagement: • Create a sense of community through the mutual sharing of lived experiences. • Create professional boundaries when working with participants. • Maintain a caseload • Supervise peer specialist • Facilitate engagement with teams by forging strong connections with participants and families. • Undertake outreach activities designed to promote community awareness of S:US services. • Encourage help-seeking, and at times serving as a bridge between team members and participants when they experience ambivalence about treatment or other services. • Develop authentic, meaningful relationships with individuals and families through empathy, listening and collaborating with genuine curiosity and interest. • Understand, share and discuss multiple frameworks for understanding life experiences with participants and the rest of the S:US team. Peer Specialists intentionally use language in the service of listening to understand and making space for complex personal stories of recovery and resilience. Advocacy and Support: • Advocates for and facilitate access to needed health care services, proactively addressing potential barriers such as missed appointments, transportation, fear and stigma, and communication with professional staff. • Advocate with and for program participants both in the larger community and on the team. • Facilitate individual and group-based educational, recovery, wellness and skill building opportunities including: wellness self-management; WRAP; peer support; harm reduction; and other social or leisure activities. • Collaborate with participants to clarify their personal visions and develop their wellness toolkit. Support participants in strengthening their self-awareness, building life skills, and connecting to resources and community outside of the S:US team. • Work actively within the team itself to help build an environment that operates from a belief in the human potential to grow and an understanding of human diversity. • Accompany individuals to appointments and community/leisure activities. • Carry a small caseload, as needed. Pre-Crisis and Crisis Support: • Providing reassurance and support to an individual in distress or crisis. • Employing non-clinical crisis support, especially after periods of hospitalization or incarceration. • Assisting the individual with putting their wellness, crisis plans or recovery plan into action. • Accompanying, supporting, and advocating on behalf of an individual in an emergency room or crisis unit or preparing to be admitted to detox, residential or other service to deal with crisis. • Providing short-term post-crisis support to an individual who has been discharged from an emergency room, crisis unit, or crisis respite residence, or incarceration. Communication: • Peer Specialists maintain open and frequent communication with the team. This includes reporting safety and other concerns to team members. • Responsible for timely and accurate documentation including documentation of interactions with guests, services provided, critical information and events, contacts with collaterals and referrals, as required by S:US and program policies. • Actively participate in agency and staff team meetings, individualized/team supervision, coaching and professional development activities. • Attend all required trainings. Maintain the highest levels of confidentiality. • Participate in required training and professional development activities. • Perform other duties as required. Community Participation: • Utilizing shared personal experience to support skill development that guides the individual towards a more independent life in the community. • Exploring community spaces that promote integration, inclusion, and participation (bridging from the individual's home to the community). • Supporting the individual in attending community activities and appointments when requested. • Assisting the individual to help connect to long-term natural supports that enhance the quality and overall security of life. III. Use a systematic approach manage change in a way that clarify priorities and reduce concern experienced by others. • Engagement: Use strong interpersonal skills to help individuals feel comfortable and motivated by using language and an approach that helps overcome barriers or stigma. Involve care managers or other team members when appropriate to help individuals and families engage fully in their care. Schedule: Tues-Friday 1:00-9:00 pm & Sat 10:00 am-2:00pm Job Responsibilities: -Persistent and assertive outreach and engagement using strength-based approaches beginning either at known “hang-outs” or “Hot spots” within the transit system or during an inpatient hospital admission or emergency department visit; -Continuously assess the health and social needs of participants through SOS's conversational and observational assessments and formalized risk assessments tools for those identified as being at high risk; -Work in collaborations with the centralized SOR Hub to identify available housing and to support participants through the process. Tasks may include completing HRA 2010e, applying for housing, prepping for interviews, follow up with housing providers, and assistance with moving in (day of move) with obtaining housing supplies and learning the neighborhood; -Participate in hospital discharge planning meetings to identify the best community resources for returning patients; -Collect and report data, as required and work with team leader, data analyst and other SOS teams to use data to inform future care delivery; -Once housed work with participants and their housing providers to resolve clinical issues that are impacting on the participant's ability manage, and retain supportive housing; -Foster relationship with community provides to ensure that recipients are connected with appropriate services as they transition back into the community; -Appointment navigation including accompaniment to appointments, travel training, reengagement in community care, and addressing barriers to care; -Review documentation and conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing and other social needs in the community; -Obtain historical and collateral information from multiple sources to support participants behavioral and physical health needs; -Monitor, evaluate and record participants progress with respect to care plan goals; -Attend and participate in team meetings and supervisory sessions. -Perform other related duties as assigned. Qualifications MinMinimum Education and Experience Requirements: Minimum High School Diploma or equivalent or higher.     The Crisis Respite Center (CRC), part of Parachute NYC, provides an innovative and unique “hospital diversion” model whereby persons seeking temporary residential/respite care can stay in a warm, friendly, safe, and supportive home-like environment where they are taught to use new recovery and relapse preventions kills. This is achieved through 24-hour peer support, self-advocacy education, self-help training, and mutual understanding. The CRC offers people experiencing a psychosis-related crisis with a short-term (one day to two weeks)residential alternative that will allow individuals to maintain daily activities and connect with community-based services and supports, including primary care. The program will provide recovery oriented services incorporating Need Adapted Treatment and Intentional Peer Support. Parachute NYC is an innovative CMS funded pilot project designed to support a continuum of community based services for individuals experiencing a psychosis related crisis, including specially trained mobile crisis teams and Crisis Respite Centers. Qualifications IV.     Experienced Peer Specialist to work on a newly funded innovative Care Transition team that serves participants transitioning to the community from a higher level of care. As a member of the Pathway Home multi-disciplinary team the Peer Specialist position will work under the supervision of the Team Leader. The role will involve facilitating transition from one level of care to the community by addressing the preparatory needs of participants in the early stages of recovery, discharge planning, development of daily living skills, and coping mechanisms through team and individual work in the community. The team will follow individuals into the community using the Pathway Home model of care to ensure that they become both linked and engaged with identified community providers and that their support network is sufficient to meet their needs. The Peer Specialist engages participants in peer-based services to identify and overcome barriers to community integration, utilizing a peer relationship to foster connection, trust, understanding and validation with community providers and supports. The role will require on call coverage and a willingness to work flexible hours. Job Responsibilities: Engagement beginning from referring location with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant; Individual sessions with participants emphasizing prevention and preparing for independent community living and the promotion of optimum mental and emotional health.
