**What You Will Do**
Under the supervision of a pharmacist, provides technical knowledge, skill and expertise required for the provision of comprehensive pharmaceutical care that supports the mission, values and philosophy of Pharmacy Services and Sharp HealthCare. Position requirement must include the ability to perform a majority of defined tasks below.
**Required Qualifications**

**Essential Functions**
+ IV preparation management Prepares all IV admixtures in strict accordance with established guidelines.Properly handles and prepares chemotherapeutic agents using the appropriate precautions in the Biological Safety Cabinet (BSC).Runs fill lists (IV labels) and organizes workload to accomplish all admixture tasks in time to meet established batch delivery times.Performs primary check of labeling to ascertain completeness of label according to established procedures.Always obtains pharmacist check prior to dispensing IV admixtures.Responds to special admixture requests to meet criteria established for STAT, ASAP and NOW IV admixtures.Recycles IVs where possible and removes expired products from circulation.Maintains IV area in a clean/orderly manner and restocks prior to the end of shift.
+ Inventory control Communicates special or urgent inventory needs to buyer, or follows-through with ordering or procuring if required.Cleans and documents cleaning of horizontal laminar flow hood, Biological Safety Cabinet contiguous admixture areas, and shelving areas.Performs all tubing and IV solution changes, equipment setup, testing and calibrations according to department procedures 100% of the time.Ensures proper med storage to prevent premature degradation or expiration.Monitors and documents refrigerator/freezer temperatures daily and follows up on variances.Completes assigned pharmacy and nursing inspections.Removes outdated and unusable medications to segregated storage site.Maintains neat and organized work areas.Maintains integrity of Pharmacy Waste streams.
+ Medication delivery Delivers medications to and accurately places medications in Pyxis MedStationsExchanges medication cassettes within designated time frame, appropriately transfers bulk medications/ removes discontinued medications from non-exchange medication cassettes.Routinely checks nursing station refrigerators for discontinued medications.Responds to special delivery requests to meet criteria established for STAT, ASAP and NOW medication orders.
+ Medication safety and use Participates in surveillance initiatives such as: Pyxis override report monitoring, anesthesia controlled substance audits, medication safety event reporting etc.Proactively identifies potential medication safety improvement opportunities such as look alike, sound alike medication storage and packaging issues, delivery / storage / security related issues, inventory management issues etc.Proactively participates in the development of medication safety initiatives to prevent occurrence or reoccurrence of medication safety events.Demonstrates awareness of medication safety communications delivered by pharmacy management and safe medication practice pharmacist.Prepares laboratory reports, target renal lists and other targeted report data as required for pharmacist consult or advisory service review.
+ Processes medication orders Adheres to all legal and pharmacy procedures, including pharmacist check of all work prior to dispensing.Assures that incoming phone calls are answered promptly and with courtesy.Assures that all tasks are accomplished during shift.Prioritizes new orders/requests and enters them into the computer (where applicable) for verification by the pharmacist.Accurately prepares and labels first and subsequent doses for pharmacist check prior to dispensing.Prepares pharmacist checked medications for delivery.Helps to resolve discrepancies in a timely manner.
+ Supply maintenance Demonstrates consistent commitment to efficient pharmacy operations by printing, accurately filling and verifying completion of all fill lists associated with Pyxis MedStation operations, unit dose patient cassettes, floor stock, and stock out notices.Consistently prioritizes and accomplishes fill list related workload in a manner that promotes smooth pharmacy operations and the medication preparation delivery process.Sustains a high level of accuracy by consistently selecting, ordering and restocking the correct medication, dosage, and strength.Obtains pharmacist check prior to dispensing 100% of the time.Accurately documents doses dispensed/credited on fill lists.Prepares, labels and documents the pre-packing / re-packaging of unit dose, bulk and compounded medications according to policy, procedure and regulatory standards.

Essential Functions
Medication Dispensing
Demonstrates consistent commitment to efficient pharmacy operations by printing, accurately filling and preparing first and subsequent doses, unit dose for patient cassettes, floor stocks, and medication requests.
Consistently prioritizes and completes workload in a manner that promotes smooth pharmacy operations.
Sustains a high level of accuracy by consistently selecting, ordering and restocking the correct medication, dosage, and strength.
Always obtains pharmacist check prior to dispensing.
Prepares, labels and documents the pre-packing / re-packaging of unit dose, bulk and compounded medications according to policy, procedure and regulatory standards.
Medication Delivery
Delivers medications to the accurate locations
Exchanges medication cassettes within designated time frame, appropriately transfers bulk medications/ removes discontinued medications from non-exchange medication cassettes.
Routinely checks nursing station refrigerators for discontinued medications.
Responds to special delivery requests to meet criteria established for STAT, ASAP and NOW medication orders.
IV Preparation
Demonstrates competency to perform work in the IV Room per USP 797 and 800 guidelines, policies and procedures.
Prepares all IV admixtures in strict accordance with established guidelines while maintaining a high-level of accuracy.
Runs fill lists (IV labels) and organizes workload to accomplish all admixture tasks in time to meet established batch delivery times.
Performs primary check of labeling to confirm completeness of label according to established policies and procedures.
Utilizes the pharmacy workflow management system and barcode technology for IV compounding per established policies and procedures.
Always obtains pharmacist check prior to dispensing IV admixtures.
Responds to requests to meet criteria established for STAT, ASAP and NOW IV admixtures.
Recycles IVs where possible and removes expired products from circulation.
Maintains IV preparation and dispensing areas in a clean/orderly manner and restocks prior to the end of shift.
Communicates workload or operational issues to supervisors in a consistent and timely manner.
Hazardous/Chemotherapy Preparation
Demonstrates competency of Hazardous Medication / Chemotherapy Compounding and USP 800 guidelines.
Properly handles and accurately prepares chemotherapeutic agents using the appropriate precautions per established policies and procedures.
Is competent in the use of Closed System Transfer Device (CSTD).
Inventory Control
Monitors drug inventory, including PAR level management, to prevent stock-outs and communicates special or urgent inventory needs to Pharmacy Buyer.
Ensures proper medication storage per established policies, procedures and manufacturer guidelines to prevent premature degradation or expirations.
Removes outdated and unusable medications to segregated storage site.
Maintains neat and organized work areas.
Maintains integrity of Pharmacy Waste streams.
Pyxis Technology Management
Must demonstrate proficiency in the use of Automated Dispensing Cabinets (i.e., Pyxis) by performing the following, not limited to: refilling, loading and unloading medications, utilizing barcode technology, completing RX checks, pulling override reports, fulfilling the stock out reports, assisting with inventory discrepancies.
Medication Safety
Proactively identifies potential medication safety improvement opportunities such as look alike, sound alike medication storage and packaging issues, delivery / storage / security related issues, inventory management issues, etc.
Participates in the development of medication safety initiatives to prevent occurrence or reoccurrence of medication safety events.
Demonstrates awareness and adherence to medication safety communications and initiatives delivered by pharmacy management and Safe Medication Practice team.
Participate in surveillance initiatives such as: Pyxis override report monitoring, controlled substance audits, medication safety event reporting, etc.
Regulatory Compliance
Maintains competency and abides by all medication management standards in accordance to Sharp policies and procedures, along with Federal and State regulations.
Completes assigned monthly procedural area/ drug storage area inspections in a timely manner utilizing appropriate tools for documentation.
Assures all documentation and records are complete and maintained appropriately as per policies and procedures for compounding, hazardous/chemotherapy, pre-packing, controlled substance audits, Pyxis, etc.
Quality Assurance
Consistently prioritizes and accomplishes assigned workload in a timely manner that promotes smooth pharmacy operations.
Must actively demonstrate the ability to identify process improvement opportunities.

Controlled Substance Management
Must demonstrate the ability to work as a narcotic pharmacy technician which entails the following:
Meets service level expectations for the preparation and distribution of controlled substances.
Performs the daily shift duties as assigned with respect to controlled substances, including but not limited to open discrepancies, inventory counts, PAR level management, and C2 safe compare report oversight per set guidelines.
Medication Reconciliation
Must demonstrate the ability to perform medication reconciliation duties as needed to comply with all related state and federally mandated requirements that include conducting patient interviews to capture and record patient information electronically (i.e. home medication name, dose, route, frequency, last dose date and time, allergies, vaccination history, patient weight, pregnancy and lactation status) in an accurate and efficient manner in accordance with
recognized standards of practice and hospital policy and procedures.
Knowledge, Skills, and Abilities
Must be able to demonstrate competency to perform work per USP 795, 797 and 800 standards.
Must possess knowledge of Automated Dispensing Cabinet technology and operations.
Must possess ability to communicate clearly and effectively with other members of the healthcare team and patients.
Must be able to manage deadlines, prioritize and manage time appropriately to accomplish assigned tasks.
Must be proficient in all technology utilized for drug distribution.
Must possess excellent verbal and written communication skills required.
Skilled use of Microsoft Office and Excel preferred.
