SCH is looking for a Patient Access Specialist for our Phone Room in the Martinsburg, WV location. Under the direct supervision of the Patient Access Manager, the Patient Access Specialist - Phone Room is primarily responsible to ensure proper answering and direction of all incoming calls for the department in a friendly and professional manner and manage urgent or emergency telephone situations efficiently. Employee will be involved with several aspects of the practice including patient contact, scheduling, clerical duties, and business functions. Role & Responsibilities: Answering incoming calls in a professional and courteous manner. Accurately scheduling patient appointments and managing provider schedules. Updating the EHR (Electronic Health Record) as needed and advised. Follow HIPAA Guidelines Provide information on referrals. Enter refill requests. Patient Portal Requests Work with Well Child and Annual GYN reports to fill SCH schedules and keep patient current on preventive care, enter data information into chart for reminder calls. Data entry of call service messages Return calls to patients with information from providers Other duties as assigned. Essential Skills and Education Requirements: Possess good listening and communication skills High school diploma or the equivalent Office skills including word processing, data entry, and recordkeeping principles Excellent time management and organizational skills Possess the ability to work in a courteous and professional demeanor with patients, co-workers, and community contacts. Previous knowledge of healthcare systems is preferred. Possess the ability to handle a multi-line phone. Demonstrate a friendly, patient, and compassionate demeanor when dealing with people, even in difficult situations. SPECIFIC DUTIES AND RESPONSIBILITIES: Effectively performs initial diagnostic interview and accurately completes the initial diagnostic forms within the established time frame. Collaborates on and shares with patient an appropriate plan of treatment based upon the initial interview, PCP input, and psychiatric evaluation and completes the documentation required in the medical record. Provides crisis intervention. Provides recommendations for hospitalization, residential or outpatient treatment, possibly including a more appropriate placement with another SCH therapist within the co-located Behavioral Health Services (BHS) Department. Communicates and collaborates with patients and staff in an age appropriate, culturally sensitive, non-threatening, productive manner. Makes referrals to appropriate staff or case management services to include Social Services, SUD treatment, Domestic Violence Programs, and further evaluation for psychosocial, spiritual and cultural needs. Provides individual, and family therapy based on the inter-disciplinary treatment plan, within privileges and scope of practice at SCH Primary Care & BHS, and following minimum standards of practice for diagnosis or problem identified. Substance abuse evaluation and transfer to the appropriate team member. Effectively coordinates and communicates with other providers involved in the treatment of the patient; attends multi-disciplinary BHS treatment meetings weekly and Primary Care team meetings, huddles and other meetings deemed appropriate by Primary Care leadership. Presents new treatments and effectively coordinates with other team members. Additional duties as assigned. QUALIFICATIONS: Ability to function professionally within a multidisciplinary team. Ability to demonstrate courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff. Essential Functions: Effectively performs initial diagnostic interview or a substance abuse evaluation; accurately completes the initial diagnostic forms within the established time-frame. Develops an appropriate plan of treatment based upon the initial interview and psychiatric evaluation and completes the documentation required in the medical record. Coordinates with Providers in the Primary Behavioral Health Program regarding diagnostic impressions and needs for psychotherapy. Crisis intervention or critical incident debriefing. Recommendations for hospitalization, and residential or outpatient treatment Communicates with patients and staff in an age-appropriate, culturally-sensitive, non-threatening, productive manner. Referrals to appropriate staff or case management services to include Social Services, CPS, and Domestic Violence Programs. Evaluations and triage regarding physical health and nutritional screening make referrals for further evaluation including assessment of pain and pain management, psycho/social, spiritual, and cultural needs. Individual, group or family therapy based on the inter-disciplinary treatment plan, within privileges and scope of practice at SVMS/BHS, and following minimum standards of practice for the diagnosis or problem identified. Substance abuse evaluation; and, treatment or transfer to the appropriate team member. Effectively coordinates and communicates with other providers involved in the treatment of the patient, attends multi-disciplinary treatment meetings weekly, and presents new treatments and effectively coordinates with other team member. This position is primarily responsible for assisting and providing safe, consistent, and appropriate patient care within his or her scope of practice. Promotes and restores patient's health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. RN's role will include but not limited to: the assessment, planning, implementation, and evaluation of patients' statues, plan of care, and goal achievement. Role & Responsibilities: Promotes patient independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions. Maintains a thorough knowledge of special programs offered by SVMS including family planning, EPSDT and cancer control, and of eligibility and guidelines of these programs which are available to patients Demonstrates flexibility, open mindedness, and versatility in adjusting to changing environments and requirements as necessary. Demonstrates courteous and cooperative behavior with patients, families, co-workers, subordinates, supervisors, and other department staff.
