The Patient Care Coordinator is responsible for overseeing the management and coordination of care for the acute inpatient population. The PCC collaborates with rounding MAPMG Hospital Based Service Physicians, patient/family, nursing, utlization review and other members of the healthcare team to assure continuum of patient care progression for clinical and cost-effective outcomes. The PCC facilitates and coordinates with community providers and ambulatory case managers to assist with the appropriate level and transition of care for a safe discharge and preventing a re-admission.
Essential Responsibilities:
Completes an initial face-to-face assessment for every admitted member to identify discharge needs within 24hrs of admission.
Document in KPHC and communicate the assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family.
Active participant in daily Care Without Delay (CWD) rounds reporting on patient progression towards the established discharge plan.
Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements.
Document any updates, care progression and barriers to discharge daily, and as indicated on assigned patients.
Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays and or days.
Collaborate with Social Worker to coordinate, long-term care, assisted living, financial assistance, and other services, as required.
Send referrals/communicate with in-network vendors for coordination of post-acute levels of care such as Home Health, DME, IV infusion, SNF, Sub-Acute and Acute Rehab.
Timely identification, recording, and escalation of delays in care and barriers to discharge. Provide solutions to correct delays and recognize systemic patterns that require corrective action.
Assure follow up appointments and referrals to ambulatory case manager for high-risk patient population are scheduled and communicated to patient/family prior to discharge.
Observe all facility safety policies and procedures (infection control, Members Rights policies, and any regulatory requirements)
Participate in Quality Assurance duties and implementation of programs to improve care Quality Indicators.
Maintain professionalism with all duties in an effective and timely manner as directed or assigned by designated supervisor.
Consistently work cooperatively with patients, patients representatives, facility staff, physicians, consultants, and ancillary service providers.

Minimum of one (1) year in an acute medical/surgical/ED or critical care nursing area.

Associate Nursing degree required.

Some of 9 Kaufman's vascular surgeries include aortic aneurysm repair, carotid endarterectomy, femoral bypass surgeries and amputations. 9 Kaufman is an extension of the renal/pancreas transplant team to include the outpatient transplant center and all the nurses on 9 Kaufman are specially trained and can sit for the PCCN certification which we will help you obtain. Department/Position Overview:
Sentara Leigh Hospital Intensive Care Unit, AACN Gold Beacon Award recipient, is comprised of 20 licensed critical care beds and has an average daily census of 18 patients. Staffing ratios vary and can be 1:1 to 1:3 with 1:2 being the usual assignment.
The ICU RN's are required to have a wide array of knowledge, skill and ability to competently and safely provide critical care to our patients. Frequent ongoing education, new skill acquisition and skill validation are provided and supported by an experienced ICU CNS for new and experienced ICU nurses. Newly hired RN's will have between 2-14 weeks of orientation depending on prior experience. Our preceptors are purposely selected for their teaching ability and formally trained to meet the training needs of new staff. Specific attention is provided for new graduate ICU RN's as they participate in our Vizient AACN Nurse Residency Program and AACN's essentials of Critical Care Orientation online education. Experienced RN's are encouraged and supported with educational and financial assistance to achieve critical care certification.
ICU staff are supported by an interdisciplinary care team which in part includes unit based respiratory, physical, speech and occupational therapists. Interpreter, nutrition, palliative, chaplaincy and ethical services are readily available as well. Our ICU has onsite and eICU pulmonary critical care intensivists and their team of physician extenders 24/7. Physician lead, nurse driven interdisciplinary rounds are held daily for continuity of care and to address nursing's concerns or questions. These rounds are made educational and even fun at times by our very engaged intensivist MD's.

General Description of the Job Class
Responsible for managing patient supply needs, including collection of patient data, inventory monitoring, processing, and coordinating services with nursing, pharmacy, operations, and finance. Updates progress notes, enters medication profile changes in HomeCare360 computer system, confirm confirmations and delivery tickets. Supports the clinical staff. Performs any other duties as directed by the Pharmacy Technician Supervisor, Assistant Director of Pharmacy, or Director of Pharmacy.
This individual performs, under supervision, a variety of duties in accordance to department procedures, which may be related to the procurement, preparation, distribution and storage of drugs, as well as, provision of clinical pharmacy services according to the Pharmacy's role in ensuring safe medication use.
Individuals eligible for pharmacy technician level II are competent in all tasks associated with pharmacy technician level I and have achieved the area specific advanced competency requirements. Pharmacy technician level II staff must achieve and maintain certification by the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
Duties and Responsibilities of this Level
Refills drug therapy orders for patients as directed by Team pharmacists.
Perform pre-delivery telephone calls to patients, nursing staff, or other agencies to monitor patient supply inventories and determine specific patient supply needs.
Contact patients to discuss supplies prior to each delivery and documents in a progress note in the clinical record. Updates patient profile in computer when there are any medication changes or additions.
Evaluate product usage and updates/revises patient inventory usage numbers in the computer based on actual usage, change in orders, and communication with nursing. Document intervention and script or status changes.
Calculate, process, and verify adequate supply inventory for each patient to ensure computer entry and delivery ticket generation.
Document appropriately in software system and notifies pharmacist of any significant problems.
Coordinate/schedule deliveries of all patient orders with warehouse and Patient Delivery Representatives.
Assist patients with supply requests and delivery issues.
Answer incoming telephone calls with triage to clinical staff when appropriate.
Print and utilizes delivery event report. Maintains and updates data files to assure timely and accurate deliveries.
Update/revise patient inventory usage numbers in the computer based on actual usage, change in orders, and communication with other team members.
Enter progress notes with every call and reviews/updates patient profiles in HC360 computer system when there are changes in patient home meds.
Notify the pharmacist of any significant problems.
Prepare “Admission Packets” for new patients as needed, including forms requiring patient signatures, patient training manuals, pertinent pump and/or formula information, patient drug information monographs, and other information as appropriate.
Communicate verbally with patients, caregivers, nursing staff, Patient Delivery Representatives, warehouse personnel and/or other agencies to monitor patient supply inventories and determine specific patient supply needs/availability.
Manage patient discharge from service: completes patient status change forms and distributes them to other team members to assure proper tracking. Updates computer and pulls chart for Medical Records per policy. Monitors status of patients readmitted to the hospital.
Coordinate pick-up of supplies and equipment at the completion of patient therapy.
Understand basics of reimbursement plans to maximize profits.
Perform verification of completed (compound confirmations) and delivered ship orders (delivery ticket confirmations) for daily revenue report and billing procedure.
Cooperate and works well with other staff members in the planning and organizing of pharmacy services.

Through effective communication via text, email, and phone, you will facilitate seamless care coordination, ensuring clients are appropriately staffed for their care needs. This position is on-site at our Tendercare office in Indianapolis.
Essential Duties:
Communicate clearly, kindly, and effectively as a primary representative of Tendercare Home Health.
Acts as the main point of contact for patients and employees regarding schedules which can include hospitalizations, call-offs, etc.
Build patient schedules that align with the patient's health insurance benefits (will be provided).
Clear alerts in Tendercare's electronic medical records system, CellTrak.
Collaborate with other departments to provide top quality, kind, and compassionate support to Tendercare patients, families, and employees.
Must strictly adhere to the Health Insurance Portability and Accountability Act (HIPAA) requirements regarding privacy and security of health information of clients of Tendercare.
Participate in a rotating Sunday on call schedule (8 a.m. Sunday to 8 a.m. Monday). Schedulers will also take turns covering on-call shifts on holidays. One scheduler should not do more than 2 holidays per year.
Performs other duties as assigned.
Required Qualifications:
Excellent verbal and written communication skills.
Must be a strong multitasker with exceptional follow-up skills.
Exceptional interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Associate degree or equivalent experience preferred.
Strong attention to detail within multiple platforms.
Proficient with Microsoft Office Suite or related software.
Experience with medical records systems or similar software is preferred.
Ability to sit at a desk and work on a computer for extended periods (up to 8 hours per day).
Ability to communicate clearly in person and over the phone.
