We're hiring a Clinical Liaison (Hospice) - a compassionate and collaborative professional who serves as an advocate for patients and families during the pre-admission process. This role bridges clinical, sales, and marketing teams to ensure a smooth transition into hospice care and supports the growth of our service line through strategic outreach. What You'll Be Doing Serve as an advocate for patients from pre-admission through discharge from medical facilities Collaborate with sales, marketing, and clinical teams to source hospice referrals from healthcare providers Review Explanation of Benefits (EOBs) and consent forms with patients and families Perform comprehensive nursing assessments to evaluate hospice eligibility Support the Hospice service line in developing and executing sales and marketing strategies Promote Silverado's mission and values through community engagement and public relations Qualifications LVN license required Knowledge and experience working with hospice patients and families, professionally or as a volunteer, highly preferred Effective communication and interpersonal skills Must pass a criminal background check, physical exam, and drug screening Valid driver's license and clean driving record required Must comply with all state and federal driving regulations Reliable transportation required Why Choose Silverado?
