JOB DESCRIPTION
HOME-BASED FAMILY EDUCATOR
Under the supervision of the home-based services supervisor, the family development educator shall be responsible to:
1. Conduct weekly home visits with families lasting a minimum of one and a half (1.5) hours to a maximum of two (2) hours per session.
Develop and implement weekly lesson plans in partnership with parents.
Provide comprehensive services to families as outlined in the Head Start Program Performance Standards.
Meet all program-required screening and ongoing assessment timelines with regard to health, education, disability and mental health.
Collect, screen, and track all medical examinations and immunizations, working with families to obtain health information and follow-up as needed.
Each month, in collaboration with parents, provide two developmentally appropriate socializations and/or fieldtrips for 0-5 year-olds and families on assigned caseload and complete classroom health and safety assessment inspections.
Provide case management and general counseling to assigned families to ensure provision of quality mental health, health, and social services, including services to high-risk pregnant women, expectant fathers, and their families. Maintain accurate documentation.
Assess, document and maintain strength-based Family Partnership Plan while supporting parents in achieving their family goals.
Participate in regular case conferencing sessions to discuss strengths, needs, and concerns of children and their families.
Refer families to available community services and follow up referral to ensure needed services were obtained. Develop relationships with the community and community resources and monitor to ensure families receive necessary services in a timely manner.
Participate in and attend all required meetings, training, workshops, conferences, and continuing education as required.
Develop and implement activities for workshops and monthly family nights as described in parent involvement procedures. Distribute monthly parent meeting agendas and minutes.
Jointly plan and implement a variety of activities for parent workshops and family nights, including planning and distributing agendas, preparing and translating minutes in English and Spanish.
Monitor and track attendance of children in caseload and follow up on absences, providing support and guidance to families as necessary to ensure successful program enrollment.
Complete program information report (PIR) data and notes into the Child Plus data program on an ongoing basis.
Participate in regularly scheduled recruitment throughout Ventura County, including community events, health and human services agencies, and door-to-door residential contacts.
Other duties as assigned.
EMPLOYMENT STANDARDS
Required Education and Experience:
Minimum of a current state-awarded child development associate teacher permit (
renewed and
maintained active as a condition of employment)
or an associate degree in child or early childhood development, or *related field with equivalent child development coursework. Coursework must include
child/human growth & development; child/family & community or child and family relations; programs/curriculum, and
**six infant/toddler units.
Minimum of two years' experience in a childcare program as a preschool or infant/toddler teacher or in a social service, community-based agency providing case management services.
*Related fields are defined as: human development, liberal arts, psychology, sociology, home economics, nutrition, special education, general education, or health education.
**If hired without the minimum six infant/toddler development units, continued education towards completing the required units within 18 months of hire will be required as a condition of employment.
Preferred Criteria: Fluent bilingual (English/Spanish) skills.
Knowledge and Abilities: Ability to effectively speak, read, and write in English. Must be able to maintain confidentiality. Sensitivity to multicultural beliefs and practices, and ability to relate to children, parents, and pregnant women. Sensitivity to the needs of low-income families. Ability to maintain accurate records. Working knowledge of computers and basic applications such as Word. Ability to work as a team member. Flexibility and ability to plan, supervise, and work independently in the implementation of the home-based program. Provide leadership and guidance to parents. Problem solving and organizational skills. Ability to speak before a group. Must have knowledge of various community resource agencies to support children and families for immediate or long-term assistance. Ability to drive vehicle and travel throughout Ventura County to conduct home visits, recruitment, and other related duties.
Physical Abilities: Ability to effectively use hands and fingers to fill out r forms and use of computer keyboard and other office machines/equipment. Ability to walk and/or stand extensively throughout employment for ongoing outdoor recruitment and field trips. Safely drive company or personal vehicle as part of daily family visit activities. May occasionally walk on slippery or uneven surfaces. Ability to bend, squat, climb stairs, and sit on the floor for family visits and/or group socials. Ability to safely lift up to 25 lbs. Ability to carry, push, and pull rolling carts and/or cases with supplies.
License or Other Requirements: Must have access to a dependable auto with minimum appropriate insurance coverage and possess a current valid California driver's license. Must hold Infant First Aid and CPR certificate and renew upon every expiration. Must submit to a job-pertinent, agency-paid, pre-employment physical examination, negative tuberculosis test, and criminal record background check. Employment is conditional pending satisfactory results of all required tests and background checks. Periodic physical examinations and TB clearances will be required if hired. Mandated reporter training required as part of employment.

PURPOSE OF POSITION:
A Home-Based, Floater Teacher at Catholic Charities:
Supports the Home-Based team when planned or unplanned extended absences occur such as a Sabbatical or Leave of Absence.
Plans and develops with parents an individualized program for their family, through establishing a caring professional relationship and a climate of mutual trust and respect through weekly home visits.

Works with parents to strengthen the family's knowledge of health and nutrition, by integrating health and nutrition education into the program, coordinating with other staff and parents health screenings for family members and providing information and referrals, if necessary.
Assists parents in strengthening their knowledge of community resources and supports parents in developing problem solving skills.
Provides developmental screenings and information to parents and families and provides referrals to other agencies when appropriate.
Plans and implements the appropriate number of socializations according to Head Start Performance Standards.
Maintains accurate and timely program data to meet reporting requirements. Enters data and generates reports.
Ensures accurate and complete records are maintained.
Completes necessary administrative duties (paperwork, etc.) timely and accurately.
Works with parents to develop weekly home visit activity plans based on each child's developmental assessment and the Family Partnership Agreement.
Manages and informs parents of scheduled field trips, extracurricular activities and other scheduled activities.
Prepares and submits internal and external reports and documentation in accordance with federal, state and local regulations, guidelines and operating standards.
Requirements
QUALIFICATIONS:
Demonstrated knowledge and experience in applying the practices and principles of child development and early childhood education; child health, safety, and nutrition; adult learning; and family dynamics.
Skilled in communicating with and motivating people, including people whose first language is not English
Must be knowledgeable of all Head Start Program Performance Standards
Able to communicate effectively orally and in writing in English and Spanish
Ability to effectively manage time and meet deadlines within established timeframes

Salary: $22.39 - $26.08/hour- Full-Time, 40 hours per week.

Job Summary

POSITION SUMMARY:
The Autism Treatment Specialist is part of an interdisciplinary team of professionals who are expected to provide high quality service provision, data collection, therapeutic intervention, and instruction in individual and/or small group formats. The Autism Treatment Specialist provides direct service and support to the clients that reside in the Monarch Boarding Academy programs and assist the client in acquiring the skills that will assist them in their transition to a step-down level of care. RESPONSIBILITIES INCLUDE:
Deliver and facilitate behavior intervention and response plans and instructional program designs that are based on Individualized Treatment Plan (ITP) goals and objectives, in coordination with the Monarch School.
Observe, describe, and document client data regarding client behavior presentation and skills targeted for acquisition by utilizing a variety of different measurements for data collection to support and add to clinical objectives and outcomes including but not limited to descriptive analysis, rate/duration of target behaviors, level of support/help, and trials-to-criterion.
Provide care and support for all clients by providing individualized supervision and support to complete personal care activities, advocating for their needs, teach them and assist them as necessary in activities of daily living, and help in the management of their clothes and personal belongings
Utilize structured activity schedules as developed by the treatment team in order to support the clients in organizing and navigating through their daily routines with predictability afforded through these types of visual supports.
Participate as a team member through effective communication with co-workers and leadership team members and following all guidelines and procedures that have been developed to be used with our clients based on assessments that have been conducted.
Lead planned daily activities and routines to optimize progress towards treatment programs and contribute to the clients' continued development.
Protect the clients from physical and emotional harm emanating from themselves, others, or the environment including the use of Safety-Care crisis management strategies and interventions, using physical management procedures as necessary for safety due to an imminent risk of harm present to the person or others.
Respond appropriately to emergency situations including but not limited to client and/or staff injury, missing clients, and crises situations in which a client is engaging in dangerous, unsafe behavior(s).
Strictly follow treatment guidelines in order to develop and foster skill acquisition of universal communication responses that replace previously engaged in responses to communicate needs including but not limited to disruptive, destructive, and/or dangerous behavior.
Teach the clients and model independence in activities of daily living, the use of effective functional communication, emotion regulation, self-management, and cooperation and acceptance skills.
Develop a keen knowledge and understanding of each of the clients and their typical presentation in order to ascertain any medical or physiological changes that may occur.
Communicate effectively with the interdisciplinary team.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and standards.

Deep cleans the cottage each evening according to specific assigned by shift plan, including but not limited to mopping, vacuuming, cleaning bathrooms, cleaning kitchens, taking inventory, organizing office space, cleaning couches, washing and folding laundry, and disinfecting.
Wakes residents in the morning. Ensures that each child's morning routine goes smoothly, including, but not limited to showers, chores, cooking, and serving breakfast, and walking students to class. Administers any medications, as necessary, upon approval from Third Shift Supervisor or Leader. Communication with first shift Treatment Specialist(s) any special circumstances or events that occurred during the night. When arriving on shift, check with second shift Treatment Specialist(s) to be aware of any special needs of the residents.
Follows daily routine/schedule. Collaborate with clinical team to incorporate individual interventions and treatment plans. Use appropriate techniques including, but not limited to providing close supervision, and following Safe Crisis Management (SCM) guidelines for de-escalation and Emergency Safety Physical Interventions (ESPIs). Responsibilities
Essential Duties and Responsibilities:
Staff will provide structured, goal-oriented, and time-limited Family Centered Treatment (FCT) in the natural environment to families who need assistance recovering from a variety of issues, including but not limited to mental health needs, substance abuse, physical, sexual, and emotional abuse, and neglect. FCT will be provided to children and families as an alternative to other services, including residential placement, Intensive In Home, and Multi-systemic Therapy. FCT Specialists must complete an intensive training program and work with some of the most challenging and complex client systems. FCT Specialists are required to work with the entire family system and address problematic Areas of Family Functioning as outlined in the FCT Model.
FCT Staff will also provide the following therapeutic services:
Crisis intake
Assessment of needs through the comprehensive Family Centered Evaluation Process
FCT Interventions
Monitoring of family/parental progress
After-care linkage and follow-up
Development of short and long-term family goals with measurable outcomes.
Attend and participate in Child and Family Team Meetings
Attend weekly (more or less, depending on the Specialist's developmental needs, case complexity, etc.) case review supervision.
Provide thorough documentation, including treatment plans, FCT fidelity documentation, progress notes, monthly reports, and assessments.
Testify in court as subpoenaed or requested.
The FCT Specialist will provide Family Center Treatment to families and guide them through the four phases of treatment in a timely manner.
The FCT Specialist will complete and maintain all program fidelity documents in a timely manner.
Family Centered Treatment is a comprehensive treatment program that requires FCT Specialists to make at a minimum two, multi-hour face-to-face sessions per week with each family system except for titration at discharge.
Each FCT Specialist will maintain a case load of an average of five FCT-referred families.
A FCT Specialist works a schedule that is flexible and able to meet the needs of each family they serve. This will often require meeting in the evenings and on the weekend.
The FCT Specialist is a member of a team that meets weekly. Participation in team meetings is mandatory and requires the FCT Specialist to serve as facilitator and recorder on a rotating basis.
The FCT Specialist will present cases to the team using the MIGS format on a rotating basis.
May assist with training as requested by management.
May assist/complete other projects/duties as requested by management.
Required Training for the Provision of FCT Services-
The FCT Specialist will complete the Wheels of Change online training course within 8 weeks of hire
In addition to the online-based training and required readings, the certification process requires Field Certification: the videotaped or observed demonstration of competencies in the field with real clients.
FCT staff should be fully FCT Certified (completed the online training program and field-based competencies) within 6-9 months of hire. Any FCT staff not certified within 12 months of hire risks their employment being terminated.
Once certified, the FCT Specialist will complete all activities necessary to maintain certification from FCTF, including recertification every two years.
Compliance
Maintains regular contact with stakeholders involved in client caseload to ensure communication about program participation and progress to goals to support client's success.
Maintain 50% of work hours in direct contact with parents and caregivers participating in the program and assigned to caseload.
Monitor client engagement in services and communication gaps and methods for re-engagement.
Conducts assessments with outcome tools to understand needs and support identifying and achieving goals.
Complete documentation promptly, accurately, consistently, and according to agency contractual agreements and COA standards.
This role is office, and community-based and requires frequent travel throughout the community to support program recruitment, events, and outreach. Office responsibilities may be a mix of hybrid (in-person, online). Working outside of standard office hours, including evenings and weekends, is also required.
Qualifications
Education and/or Work Experience Requirements:
Bachelor's degree in Social Work or a comparable human service field and at least two years' experience working with children and families or an advanced degree in Social Work or comparable human service field.

Salary Information

A twelve-month probationary period is required.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000 ($1500 on first paycheck, $1500 after earning permanent status (usually 1 year)). (UW System employment isn't disqualifying.)
