PRIMARY JOB RESPONSIBILITIES
Conduct initial consults, as well as follow-up visits for patients.
With keen attention to the specific needs of patients, deliver excellent care, with compassion, motivation, and sound medical knowledge.
Coordinate and monitor patients' status in conjunction with attending physicians, social workers, and other providers.
Respond to patients' healthcare needs, providing triage, urgent care and interventions as necessary/appropriate.
Follow patients, including longitudinal care and follow-up, in conjunction with other providers.
REQUIREMENTS
Nationally certified ARNP/PA-C with DEA.
Current and valid Washington State Registered Nurse Licensure and ARNP Licensure or Physician Assistant License.
Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited PA program.
Proven excellence working in an interdisciplinary collaborative team environment.
Effective interpersonal skills, leadership skills, and clinical expertise in patient care.
Experience in the provision of transplant services including initial assessment, diagnostic evaluation, and management of patients.

24-Hour Live In Shifts at With a Little Help:
Caregiver will typically maintain a work schedule of sixteen (16) hours of work and eight (8) hours of non-working, unpaid sleep time.
However, if the Caregiver is called to duty during any of the excluded sleep time, the work performed during the interruption must be recorded as compensable hours worked for which the Caregiver will be paid in a manner compliant with federal law (and Seattle law, if Caregiver works in Seattle).
Up to eight (8) hours of sleep time is excluded as non-compensable time when the Caregiver works a shift of at least 24 hours, has been provided with adequate sleeping facilities, and can generally enjoy an uninterrupted night's sleep. Caregiver agrees to notify their supervisor if the sleeping facilities are inadequate.]
Caregivers must take meal and rest breaks as required by Seattle/Washington-state law. Caregiver agrees to follow the Agency's Meal Period and Rest Break Policy for Caregivers. If Caregiver sleeps or resides at their place of employment, Caregiver must take an unpaid 24-hour period of consecutive rest for every six consecutive days worked.

Skills and Responsibilities:
Resourcefulness in assisting the lead care provider with babies, toddlers, and children in a richly interactive, learning environment.
Establish and enforce rules for behavior and procedures for maintaining order through indirect guidance principles.
Help observe and evaluate, behavior, social/spiritual development, and physical health.
Perform positive behavior support and management.
Provide personal care or assist with the necessary self-help requirements in areas such as toileting.
Know and follow the health, safety, and emergency procedures of the school.
Work cooperatively and communicate successfully with staff and parents.
Demonstrate a commitment to the education of all children.
Observe strict confidentiality regarding children and personnel information.
Adapt to a variety of working situations and learn new skills.
Promote and practice professional growth and development.
Demonstrate professionalism through conduct, punctuality, and minimal absence.
Attend staff meetings.

Requirements
At least 18 years old & completion of HS Diploma or Equivalent
Must be physically able to sit or stand for periods of time.
Availability and flexibility to work between the hours of 8:15 am-6:00 pm, schedules available two weeks in advance.
Must complete MinistrySafe Child Abuse training.
Be able to pass both local and federal background checks and all policies required for volunteering at PURSUIT.
Be committed to the local church and loyal to its senior leadership.

What You'll Do
As a Home Health Aide, you will provide essential care and support, ensuring the well-being of our patients. Your responsibilities will include:

Job Requirements
* High School Diploma or GED
* CNA- Completion of 75, 120, or 150 hour Certified Nursing Assistant Pgm/must be in-State C.N.A. Registry.
* HHA- successful completion of a competency evaluation program.
* CNA- One year of Certified Nursing Aide-Home Health Hospice.
* HHA- One year home health aide, patient tech or other related
* Experience and/or passes CHI Health at Home's competency

The Home Health Aide is a paraprofessional member of the Elders In-Home Services support team who works under the supervision of a Registered Nurse/LPN and Manager. This position performs various personal care services as necessary to meet the client's needs. The home health aide is responsible for observing clients, reporting these observations and documenting observations and care performed. This position will be supported through continuous training and assigned in a manner which promotes quality, continuity and safety of a client's care.
MAJOR TASKS AND RESPONSIBILITIES

Provides personal care which may include:
Baths
Oral hygiene
Grooming
Changing bed linens
Assisting with dressing and undressing

Planning and preparing nutritious meals, following dietary requirements as instructed from care plan; assists in feeding the client, if necessary.

Performs range of motion or other simple procedures as an extensional therapy service when required (with appropriate completed/demonstrated skills competency).
Gives clients medication under nurses' or doctors' orders; assists clients in the self-administration of medication.
Accompanies clients to doctors' office and the hospital to provide assistance.
Performs light house cleaning and does laundry, as appropriate.
Assists with applicable errands, such as shopping for food or household supplies.
Meets safety needs of client's and uses equipment safely and properly (i.e., foot stools, side rails, etc.).
Reports on client's condition and any significant changes to the assigned nurse.
Provides instruction to clients, emotional support, and companionship.
Adheres to the programs documentation and care procedures; maintains confidentiality and HIPAA compliance.
Attends all required training, meetings, and/or conferences. Maintains paraprofessional development, through continued education and on-the-job training; maintains required certifications.

EDUCATION - EXPERIENCE AND TRAINING FOR POSITION
Required: Culturally relevant experience in a care giving role. Experience working with Elders, within a Tribal community preferred. Must maintain a current and valid Washington State Driver's License and provide own transportation with valid insurance coverage. Must have flexibility to work varying hours/shifts. Graduation from high school or equivalent.
Special Requirements: Must pass a criminal background check, a drug screen and meet all health and immunization requirements.
Must be able to obtain required certifications within timelines indicated upon hire:
Food Handler's permit within 30 days of hire.
First Aid, CPR, Blood Borne Pathogens, and Oxygen certifications within 3 months of hire.
Home Health Aide certification through MEIHSS within 6 months of hire.
Successfully passing home care aid exam within a year of hire (license verification upon completion of exam).
