However, these hours could be extended due to patient care. ENVIRONMENTAL / WORKING CONDITIONS: Work is performed in a hospital / clinic environment. Requires contact with patients and other providers. PHYSICAL DEMANDS: Work may require hand dexterity. Work may require standing or sitting for extended periods of time. May require lifting of objects more than 50 pounds. Ability to stand, walk, reach above head, squat, bend at the waist, and twist. Position Summary This position requires a mission-driven and detail-oriented Advanced Practice Provider (APP) to serve as our lead clinician for Medicare Annual Wellness Visits (AWVs), preventive care services, and related quality initiatives. This is a high-impact position designed to help our organization improve preventive care rates and quality outcomes across all 28 Cahaba Medical Care clinics. The ideal candidate will have a passion for health promotion, quality improvement, and rural health equity. This provider will perform in-person wellness visits across our locations, follow up on necessary documentation (including insurer-specific requirements), and support training for other staff and providers in performing high-quality, compliant AWVs. This position reports directly to the Chief Executive Officer (CEO), Chief Medical Officer (CMO), and works in close coordination with the Quality Improvement Officer. Responsibilities & Duties Perform in-person Medicare Annual Wellness Visits and other preventive care visits across Cahaba Medical Care sites. Ensure all required elements of AWVs are accurately completed, documented, and submitted per payer and CMS guidelines. Submit insurer-specific forms and documentation in payer portals (e.g., closing care gaps, diagnosis attestation, HCC coding updates, etc.) to ensure loop closure. Collaborate with the Quality Team to ensure visits meet internal quality standards without additional remediation. Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process. Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director. The provision of these services must be performed to ensure the health, welfare, safety and independence of our residents as the primary concern. PREVIOUS EXPERIENCE IS PREFERRED. Responsibilities: * Assist residents with; activities of daily living, including but not limited to bathing, dressing and grooming, transfers, ambulation, which may include wheelchair mobility * Encourage the residents to take an active part in the planned activities. * Participate in set up of dining room as directed, serve meals to residents, and assist residents during meal time as needed. * Provide laundry services as directed or as needed by a resident. * Clean a resident's room on a weekly basis by vacuuming carpets, sweeping and washing tile floors, sanitizing as necessary. * Practice safety procedures and comply with the established reporting of incidents, accidents and injuries to associates or residents * Complete all training and orientation requirements and attend all in-service training as outlined by State regulations and policies and procedures. * Report all changes in resident's status immediately to the charge nurse * Maintain local and state certification, if applicable * Comply with company policies and procedures * Clock in and out on their time card, no sooner than six (6) minutes before or after their scheduled shift. * Follow the instructions and supervision of their shift supervisor and will follow the chain of command when solving problems. * Call in appropriately to allow their shift to be filled if the need arises. Must call them self unless physically unable to do so and must call in two (2) hours before the shift is to start. * Follow resident's rights * REPORT ANY OTHER EMPLOYEE THAT IS NOT DOING THEIR JOB OR THAT IS BEING ABUSIVE TOWARD A RESDIENT OR STEALING FROM A RESIDENT OR ANYTHING THAT IS AGAINST THE WORK ETHIC OF REGENCY RETIREMENT VILLAGE. * Understands this is an AWAKE facility and that they are required on the night shift to be awake and alert at all times for the safety of the resident and the facility. * Understands that to refuse to do an assigned task will result in termination. * May be asked to stay extra time or to stay overnight in the facility in winter months if the need arises to help care the for residents in the event of disasters. * Will be expected to respond to an emergency call in the event of a disaster, fire or evacuation of the facility. Physical: * Requires frequent sitting, standing, bending, reaching, stooping, twisting, pushing, and walking. * Must be able to lift at least fifty (50) pounds. * Must be able to operate standard housekeeping equipment such as vacuums, mops, etc. Mental: * Must be alert and attentive to detail. * Able to read, write and follow written instructions. * Able to make positive decisions and show good judgment * Must present a neat, clean, and professional appearance and demonstrate a positive and cheerful attitude towards other associates and residents and their families Environment: * Must understand that they are subject to exposure to hazardous chemicals and infectious substances. * During winter weather may be exposed to inclement conditions.
