MHS is hiring married couples to become flex houseparents. Flex houseparents are couples who reside in on-campus student homes, guiding and nurturing a group of approximately 8-12 students. Flex houseparents provide a consistent family-like structure for students and handle responsibilities such as driving, administering medication, budgeting, reporting, etc. When initially hired, flex houseparents cover different student homes before being assigned to one specific home.

The Role:
We are looking for a compassionate and dedicated Care Companion to join our service center team at Carrot in Des Moines Iowa. At Carrot, we firmly believe in providing accessible, inclusive, and high-quality fertility and family-forming care to all individuals. As a Care Companion, you will have the unique opportunity to guide our members through their family-forming journeys, starting from their initial interaction with Carrot. The Team:
The Care Companion team is composed of compassionate individuals with experience in the healthcare industry who have completed foundational training in reproductive health in addition to ongoing learning in the topics of adoption, fertility treatment, fertility preservation, donor assisted reproduction, gestational carrier arrangements, LGBTQ+ care and more. The Care Companion team supports all legal paths to parenthood and works with members living in the US and 50+ countries (and growing) around the world.

Minimum Qualifications:
1-3 years of experience working in a direct customer service, healthcare, or client-facing role.

The **Provider Contract Cost of Care Consult Senior / VBC** is responsible for providing the highest level of analytical support to the Cost of Care and/or Provider Contracting organizations.
**How you will make an impact:**
+ Focuses efforts on lowering claims costs, improving the quality of care, and increasing member and provider network satisfaction.
+ Provides expert advice, analytic and consultative support to Medical Directors and management on cost of care issues. Leads large scale initiatives with high dollar cost savings opportunities.

+ Acts as a strategic partner to management.
+ Uses analytic tools to track both health risks and compliance, as well as supporting the contract negotiation process.

**Minimum Requirements:**
Requires BA/BS degree in Mathematics, Statistics or related field and a minimum of 7 years experience in broad-based analytical, managed care payor or provider environment as well as in depth experience in statistical analysis and modeling; or any combination of education and experience which would provide an equivalent background.
**Preferred Skills, Capabilities, and Experiences:**
+ Experience providing leadership in evaluating and analyzing complex initiatives strongly preferred.
+ Master's degree preferred.

Advanced Care Provider (ACP) is a Certified Physician's Assistant (P.A. -C) or an Advanced Registered Nurse Practitioner (ARNP) or a Doctoral Nurse Practitioner (DNP) who delivers primary and specialty patient care with a high degree of expertise. Provider must be fluent in speaking Spanish. Professional services include formulating an evidence-based care plan based on assessment, examination, and selected diagnostic procedures within their scope of practice.
ACP
Under the general guidance of the Chief Medical Officer and in accordance with the Board of Medical Examiners directives, the ACP will follow Standards of Care - Best Practice Guidelines in the clinic and/or hospital setting.
Evaluates, diagnoses, and treats diseases and adverse health conditions.
Order and evaluate diagnostic testing. Identify proper diagnosis and create appropriate plan of care.
Patient education regarding treatments, procedures, tests and their subsequent results.
ACP/Convenient Care & Primary Care Clinic
In addition to the role and function of ACP above, the ACP/Primary Care Clinic will:
Provide timely and efficient medical services to all patients who present to the Clarke County Family Medicine Clinic.
Patient Care
Utilizes a holistic approach (considering physical, psycho/social, spiritual, educational, safety, and related criteria) appropriate to the age of the patients served in the assigned service areas.
Establishes a database of information on the client by:
Obtains health history.
Conducts basic periodic physical assessment using appropriate tools.
Orders appropriate screening and/or routine diagnostic tests.
Assesses patient and provides medical care according to the policies and procedures as set forward by evidence based medicine and practice guidelines.
Manages client care by:
Assists other members of the health care team with recommendations, adjustments and therapies in care plans as needed.
Participates in and when appropriate continuing health care plans involving the patients, the patient's family and other resource personal.
Assists patients with activities of daily living.
Plans and provides medical care to patients in hospital setting who are suffering from chronic or acute physical health.
Responds to patients needs in a timely manner.
Obtains accurate and adequate specimens; labels all specimens in accordance with standards.
Incorporate quality improvement data and/or patient satisfaction data in clinic and Inpatient Service goals.
Utilizes Electronic Health Record.
Relationship Building & Patient Education
Coordinates care with outside sources including Lab, Diagnostic Imaging, Private Health Care Providers, Social Workers, and Public Health.

Develops and maintains curriculum for inpatient and outpatient diabetes education programs.
Utilizes effective education techniques for age related care of pediatric, adolescent, and geriatric patients.

Practices in accordance with current ADA Clinical Practice Guidelines
Collects data, monitors and reports patient outcomes in relation to national benchmarks.
Incorporate quality improvement data and/or patient satisfaction data in clinic goals.
Assists in unit specific duties (i.e. management responsibilities, as appropriate to the assigned area.
Provides education to clinical staff when needed.
Attends continuing medical education programs to maintain licensure and Board Certifications in area of primary specialty
Attends 80% of medical staff meetings.
Reports problems or concerns to the Chief Medical Officer on a timely basis.
Education
Certified Physician's Assistant (P.A.-C) or
Advanced Registered Nurse Practitioner (ARNP) or
Doctoral Nurse Practitioner (DNP).
Graduate of an accredited Nurse Practitioner Program.
Certification in specialty areas or demonstrate active progress toward attainment of certification.
Current licensure as P.A.-C in state of Iowa
Current licensure as RN and ARNP in state of Iowa
Education in management and care of chronic and acute wounds and ostomy care.
Experience
Experience in management and care of chronic and acute wounds and ostomy care.
EPIC charting.
License(s)/Certification(s)
Valid driver's license when driving any vehicle for work-related reasons.
Current, active, and unrestricted Physician's Assistant Board Certification.
Current, active, and unrestricted RN and ARNP license in the State of Iowa with appropriate certifications as necessary by licensing boards.
Current prescriptive authority in the State of Iowa including DEA Registration.
Current BCLS and ACLS certifications.
