PART ONE: Dimension and Description
Primary Purpose: To provide support to the patient care team as well as directly attending to certain specified needs of the patient.
Formal Policy-Setting Responsibilities: No formal responsibility. The policies associated with the job's purpose and essential responsibilities are set by others.
Routine Decision Making: Determine when to report patient concerns and abnormal vital signs to the RN, determine appropriate supplies necessity, to maintain patient care.
Formal Supervisory Responsibility: No formal supervisory responsibility.
Required Knowledge:
Required Skills and Ability: Organization, problem-solving, computer skills, customer service, written and verbal communication skills.
Unusual Working Conditions: Risk of blood, body fluid contamination. Risk for injury from aggressive, combative or disoriented patients. Risk of injury related to poor body mechanics. Potential exposure to hazardous drugs and chemicals.
Education and Certification/ Registration Required for the Position: High school education. NVCI or equivalent class as determined to be required by the facility.
Age Specific Competencies: may sit with pediatric/child patients between (2 yrs-12 yrs); adolescent patients between 12 to 18 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.
PART TWO: Essential Responsibilities and Tasks
A. Provides constant observation and monitoring of assigned patient(s) to ensure safety and
Prevention of injury (70%)
1. Receives report from exiting patient care sitter, LPN or RN including any patient specific
behaviors to monitor.
2. Courteously greets patients, identifies self, and explains purpose.
3. Ensures patient's safety, regardless of the unit assigned to for the shift.
4. Observes and reports to LPN or RN all patient concerns and changes in condition.
5. Promptly calls for assistance as needed.
6. Provides care within scope of practice.
7. Assists patient to restroom. Walks patient in corridor as allowed/ordered.
8. Does not leave patient unless relieved by another staff member.
9. Utilizing the AIDET model, conducts an appropriate introduction, discusses with patient ADL's and plan for the shift and compliments at least one member of the patient care team each shift. Purpose directly relates to patient satisfaction scores and performance evaluation.
10. Notifies LPN/RN if tubes, IV, catheter, etc. becomes disconnected.
11. Reports defective equipment to LPN, RN or charge nurse.
12. Accompanies patient off the department for all tests and procedures or transfer to another
department.
13. Transports lab specimens, patient equipment and patient belongings as required with the
patient.
14. Document patient care within the Electronic Health Record (EHR) system as needed.
B. Maintains a safe and comfortable unit. (5% of time)
1. Keeps patient room neat and orderly.
2. Ensures alarms are audible to alert staff, each shift.
3. Evaluates patient's environment for possible hazards and removes those hazards.
4. Acts rapidly and calmly during emergencies.
C. Promotes infection control. (5% of time)
1. Washes hands before and after patient contact.
2. Follows isolation procedures according to the Infection Control Policy.
D. Competently utilizes technology. (5% of time)
1. Demonstrates understanding of the unit of department technology, i.e. call light system, bed
controls and TV controls.

KNI practices a person-centered philosophy to support our residents so they can have the quality of life they deserve by recognizing their individuality, honoring their choices, protecting and maintaining their health, helping to build and maintain their relationships with others, and making sure that they are included in every aspect of their lives; in their homes, in their jobs, and in the community.
KNI has 19 homes with direct support staff assisting with daily living activities in each home throughout four buildings and is located on a large campus, east of the VA hospital in Topeka. KNI is funded in part by the Centers for Medicare and Medicaid Services and is subject to requirements bestowed by Federal and State levels of government conditions of participation.
About the Position:
Provide direct service and support to the people who live in Unit 2 at KNI so they can achieve personal growth and well-being, realize choice, attain full inclusion, and celebrate their individuality. Provide an environment that promotes a meaningful life.
Job Responsibilities may include but are not limited to the following:
* Maintain frequent positive interactions with the individuals living at KNI.
* Support and assist in carrying out individuals' personal lifestyle plans
* Plan community outings and activities
* Serve as a positive role model to the individuals living at KNI
* Provide appropriate person centered supports and services utilizing task partnering and incidental teaching when assisting with activities of daily living, such as:
* Basic housekeeping

MHDDTs are offered overtime on occasion and rotate working on weekends. Physically, the MHDDT must be able to lift up to 30 pounds repeatedly throughout a shift and be able to stand, sit, walk, bend, stoop and twist for long periods of time throughout a shift. MHDDTs who obtain their CPR, CNA and CMA certifications are required to attend continuing education classes to maintain their certification (s). All staff at KNI must stay current with policies, procedures, and protocols, proper lifting techniques, and all other training needed to keep KNI in regulatory compliance. Work locations on campus between homes may be changed as staffing needs change. KNI is funded by the Center of Medicare and Medicaid Services and is subject to requirements bestowed by Federal and State Levels of Government conditions for participation.

The Pediatric Intensive Care Unit (PICU) is a forty-one bed acute care setting for medical or surgical patients
The medical PICU APN will work as part of a team to provide coverage to high acuity pediatric ICU patients following acute respiratory failure, acute kidney injury, bone marrow transplant, kidney transplant, septic shock, burn injury and trauma, among other diagnoses. In this team structure, they will provide direct patient management which includes participating in patient care rounds, ordering/interpreting lab tests, analyzing and prioritizing complex data, developing differential diagnosis and implementing plans of care. These tasks will be done in direct collaboration with a board-certified pediatric intensivist. The APN will be expected to work effectively with the medical team by communicating clearly with families, working well with physicians and physician trainees, being a resource for nursing staff and collaborating with multiple subspecialties that function in the medical PICU.
The Advanced Practice Registered Nurse I (APRN-I) provides comprehensive, evidence-based advanced practice nursing to address the physiologic, psychosocial, and developmental needs of patients and families within the framework of the collaborative practice agreement. The APRN-I participates in beginning leadership, evidenced-based practice/ research, and organizational development activities with the support and direction of an experienced APRN mentor.

REQUIREMENTS, SKILLS, AND QUALIFICATIONS:

* CPR and First Aid Certification are strongly preferred. If you are not yet certified, a willingness to obtain this certification is required (we will provide training).
Good judgment and decision-making skills are important for this position. Patience is a key attribute.
Childcare workers should have stamina and a good level of energy to complete all job duties and interact with games and physical activities. You must be able to stand for long periods of time and walk frequently. Must be able to use hands and fingers to operate and handle general office equipment. Ability to lift 20 pounds.

Key Responsibilities
Provide comprehensive urgent and episodic care for patients of all ages
Evaluate and manage urgent, emergent, and non-urgent conditions in a high-volume setting
Order, perform, and interpret diagnostic tests and imaging
Diagnose and treat illnesses and injuries, including prescribing medications, vaccines, and therapies
Perform clinical procedures such as laceration repair and orthopedic evaluations
Educate patients on diagnoses, treatment plans, and follow-up care
Complete accurate and timely documentation; charts closed within 24-48 hours
Collaborate with clinical staff and supervising physicians (available via direct telecommunication)
Provide guidance and supervision to clinical staff as needed
Participate in training, recruiting support, and special projects as assigned

Qualifications
Education
Nurse Practitioner or Physician Assistant degree from an accredited program
Licensure & Certification
Active professional license (or ability to obtain)
Current board certification
