PART TWO: Essential Responsibilities and Tasks A. Provides constant observation and monitoring of assigned patient(s) to ensure safety and Prevention of injury (70%) 1. Receives report from exiting patient care sitter, LPN or RN including any patient specific behaviors to monitor. 2. Courteously greets patients, identifies self, and explains purpose. 3. Ensures patient's safety, regardless of the unit assigned to for the shift. 4. Observes and reports to LPN or RN all patient concerns and changes in condition. 5. Promptly calls for assistance as needed. 6. Provides care within scope of practice. 7. Assists patient to restroom. Walks patient in corridor as allowed/ordered. 8. Does not leave patient unless relieved by another staff member. 9. Utilizing the AIDET model, conducts an appropriate introduction, discusses with patient ADL's and plan for the shift and compliments at least one member of the patient care team each shift. Purpose directly relates to patient satisfaction scores and performance evaluation. 10. Notifies LPN/RN if tubes, IV, catheter, etc. becomes disconnected. 11. Reports defective equipment to LPN, RN or charge nurse. 12. Accompanies patient off the department for all tests and procedures or transfer to another department. 13. Transports lab specimens, patient equipment and patient belongings as required with the patient. 14. Document patient care within the Electronic Health Record (EHR) system as needed. B. Maintains a safe and comfortable unit. (5% of time) 1. Keeps patient room neat and orderly. 2. Ensures alarms are audible to alert staff, each shift. 3. Evaluates patient's environment for possible hazards and removes those hazards. 4. Acts rapidly and calmly during emergencies. C. Promotes infection control. (5% of time) 1. Washes hands before and after patient contact. 2. Follows isolation procedures according to the Infection Control Policy. D. Competently utilizes technology. (5% of time) 1. Demonstrates understanding of the unit of department technology, i.e. call light system, bed controls and TV controls. Kansas Neurological Institute The Kansas Neurological Institute (KNI) is an intermediate care facility offering support to approximately 120 residents with intellectual and developmental disabilities. KNI practices a person-centered philosophy to support our residents so they can have the quality of life they deserve by recognizing their individuality, honoring their choices, protecting and maintaining their health, helping to build and maintain their relationships with others, and making sure that they are included in every aspect of their lives; in their homes, in their jobs, and in the community. KNI has 19 homes with direct support staff assisting with daily living activities in each home throughout four buildings and is located on a large campus, east of the VA hospital in Topeka. KNI is funded in part by the Centers for Medicare and Medicaid Services and is subject to requirements bestowed by Federal and State levels of government conditions of participation. About the Position: Provide direct service and support to the people who live in Unit 2 at KNI so they can achieve personal growth and well-being, realize choice, attain full inclusion, and celebrate their individuality. The Pediatric Intensive Care Unit (PICU) is a forty-one bed acute care setting for medical or surgical patients The medical PICU APN will work as part of a team to provide coverage to high acuity pediatric ICU patients following acute respiratory failure, acute kidney injury, bone marrow transplant, kidney transplant, septic shock, burn injury and trauma, among other diagnoses. In this team structure, they will provide direct patient management which includes participating in patient care rounds, ordering/interpreting lab tests, analyzing and prioritizing complex data, developing differential diagnosis and implementing plans of care. These tasks will be done in direct collaboration with a board-certified pediatric intensivist. The APN will be expected to work effectively with the medical team by communicating clearly with families, working well with physicians and physician trainees, being a resource for nursing staff and collaborating with multiple subspecialties that function in the medical PICU. The Advanced Practice Registered Nurse I (APRN-I) provides comprehensive, evidence-based advanced practice nursing to address the physiologic, psychosocial, and developmental needs of patients and families within the framework of the collaborative practice agreement. The APRN-I participates in beginning leadership, evidenced-based practice/ research, and organizational development activities with the support and direction of an experienced APRN mentor.
