    HR Coordinator Scope: The HR coordinator administers a variety of human resources activities and programs, including, but not limited to, those related to recruiting, compensation, benefits, training, and workplace safety. Must keep facility vehicle clean. Conduct assigned Residential groups and document based on developed written curriculums for each group. Working as a peer to physicians and as a consultant to Care Management and administration, the PA intervenes when practice patterns or behaviors or documentation issues create disparity between pathway standards, intensity of service, severity of illness, patient and family rights, teamwork, or other issues regarding the stewardship of resources for individual patients, diagnostic populations, and the organization as a whole. In the area of Readmissions, the Physician Advisor will help to lead the effort to reduce avoidable readmissions. The Physician Advisor will engage with Physicians, Advanced Practice Professionals, nursing and other ancillary personnel as well as administrative leaders as part of his/her role in Performance Improvement, Resource Utilization and ongoing education on imp8ortant trends in healthcare management. Quality Improvement: In addition to the above duties, the PA will work with the VPMA and the Quality Director or Manager at the hospital on quality improvement initiatives to assist the hospital to achieve its annual goals. Core Responsibilities and Essential Functions: The Physician Advisor (PA) will work closely with the Care Coordination/Care Management team to provide timely consultation and clinical expertise to ensure fiscally responsible and efficient utilization of resources.     The Primary Therapist will be responsible for providing clinical services to a wide range of individual and families ensuring each client within the assigned caseload is sufficiently supported by the Skyland Trail Program Services from the client's entry into the program until discharge. Responsibilities: Work with individual client caseload. Conduct individual therapy sessions at least twice per week with assigned caseload while completing documentation within Skyland Trail's guidelines. Develop individualized treatment plans for caseload and revise per documentation guidelines and as necessary. Conduct meetings with parents/guardians every two weeks to review progress, discuss treatment goals, and plan discharge. Review clients' treatment plans, documenting progression towards treatment plan goals and all changes in status. Conduct clinical groups for adolescents based on the following principles: Cognitive Behavioral Therapy (CBT), Behavioral Activation, and Interpersonal Therapy for Adolescents (IPT-A), Acceptance and Commitment Therapy (ACT), DBT Skills (i.e. distress tolerance) and problem solving groups. Create/implement additional resiliency/safety plans as necessary based on client needs. Assist clients when crisis situations arise through the use of verbal de-escalation as well as crisis assessments and evaluation for potential involuntary hospitalization. Participate in on-call rotation, covering on-call responsibilities. Participate in meetings (between staff, clients and/or families) when Additional leadership or situation de-escalation is needed within the meeting. Participate in weekly interdisciplinary treatment planning meetings including presenting and discussing clinical cases assigned speaking to goals, treatment plans, expectations and needs of the client. Responsible for completion of assessments that will include entering data and tracking outcomes on a weekly basis in order to ensure continuous quality improvements and evaluate treatment response Plan discharge meetings with families and patients outside of group sessions and be responsible for documentation associated with discharge including discharge summaries and associated paperwork. Attend weekly supervision with Director of Clinical Services for administrative supervision, case consultation and trainings Attend supervision with Chief Medical Officer to review client case and clinical concerns. Complete all documentation in accordance with guidelines Comply with facility policies and accreditation standards (i.e., Joint Commission, ORS, etc.) Other duties as assigned. The Dietitian will develop and facilitate nutrition assessments, comprehensive nutrition assessments, as needed, eating disorder assessments, as needed and recommended, evidence based nutritional lesson plans, group work, and will help integrate nutritional and holistic care into the organization's overall metabolic wellness program. Duties: Performs nutritional assessments and follow-up as appropriate with patients. Identifies patients in need of additional nutrition education/support and meets with these patients at an appropriate interval throughout treatment to provide direct care and monitoring. Determines necessity and appropriateness of modified diets for patients with specific nutritional needs. Provides nutrition counseling for patients on modified diets. Communicates with appropriate patients, psychiatrists, primary care clinic providers and Sage Catering to coordinate and implement nutritional recommendations and/or dietary needs.
