Essential Functions
Conducts the admission of the patient to the Agency by: obtaining a medical history, to include Oasis when needed, particularly as it relates to the present condition, from the patient and / or family member (s); performing a physical assessment of the patient; assessing the patient's support systems to determine what assistance will be available from family members and their ability to meet the patient's immediate needs upon admission and throughout care; assessing the patient, family member (s) and home situation to determine what health teaching will be required; assessing the patient's condition and home situation to determine if other home health services are needed; interpreting nursing and other services of the Agency to patient's and families as a part of planning for care.
Develops and implements the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient in MatrixCare.
Manages assigned cases and provides skilled nursing care as outlined on the physician's plan of treatment and in accordance with the Agency's policies and procedures.
Observes signs and symptoms and reports to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patient's physical or emotional condition and outcomes of intervention. Ensures on-going evaluation of patient / family response to care.
Adequately educates, teaches, supervises, consults, and counsels the patient / family members regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern. Also provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care.
Initiates appropriate preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety.
Ensure that care is provided by the Home Health Nursing Assistant by providing the Nursing Assistant with a written plan of care and specific duties to be performed, by supervising and evaluating the care given at least once every two weeks, and by participating in periodic conferences with the Director of Clinical Practice or Assistant Director of Clinical Practice concerning the Nursing Assistant's performance.
Appropriately consults with the attending or on-call physician concerning alteration of the plan of treatment and documents, in writing, appropriate change of orders where necessary.
Communicates with supervisor concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization.
Turns in all paperwork to the designated person in the office by 5pm on Monday, and syncs ipad nightly when visits are done. Makes a verbal report of new admissions and ER visits to the primary nurse Sunday night or Monday morning between 8:00-9:00 a.m.
Collaborates with supervisor to meet assigned productivity standards
Keeps up-to-date on reimbursement criteria and charting needs for all patients under his/her care and supervision.
Sees scheduled visits after 5:00 p.m. on Fridays as warranted. Works at least one weekend day when Christmas falls on a weekend.
Attends and participates in office staff meetings, patient care conferences, record audits, utilization review, quality improvement/assurance activities, in-services, workshops, and seminars when appropriate. Adheres to the minutes of the ITC and staff meetings.
Continually strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings. Evaluates own needs for support and uses identified system(s) to meet those needs.
Takes weekend call as assigned.
Attends the appropriate hours of in-service training per year as required.
Adheres to the Agency's personnel policies and Compliance Plan.
Willingly performs other duties as assigned by supervisor.
Ensures referrals are evaluated timely with a NPU goal of 6% or less.
Assists with covering visits in other offices as assigned.
Displays a high degree of courtesy, tact, and knowledge of services provided by the agency in all contact with staff, patients, and visitors.

JOB SUMMARY
We are currently seeking a full-time Microbiology Analyst II, working in an accredited commercial laboratory, responsible for performing microbiological testing, data analysis, and documentation to ensure product quality and regulatory compliance. This role involves operating specialized lab equipment, supporting method development, and supervising junior staff. The analyst also collaborates with Quality Assurance, contributes to investigations and audits, and helps maintain a high standard of laboratory accuracy, safety, and efficiency.
ESSENTIAL FUNCTIONS:
* Operate, calibrate, and maintain specialized laboratory instrumentation for microbiological testing, such as incubators, autoclaves, microscopes, and particle counters.
* Prepare media, reagents, and samples for analysis following SOPs (Standard Operating Procedures).
* Ensure proper aseptic techniques to prevent contamination.
* Identify microorganisms using traditional and/or rapid methods (e.g., Gram staining, biochemical tests, MALDI-ToF).
* Maintain microbial culture collections and growth promotion.
* Monitor sample workflows and proactively address backlogs to ensure timely completion of testing.
* Manage laboratory supplies and support procurement activities for relevant departments.
* Perform advanced microbiological testing on raw materials, finished products, and environmental samples.
* Accurately record detailed observations, analyze data, interpret test results, and maintain documentation in lab notebooks and LIMS.
* Assist in the development, validation, and implementation of new test methods and procedures.
* Investigate anomalies or inconsistencies in analytical data and implement corrective actions.
* Explore and evaluate new technologies or methodologies to enhance laboratory capabilities.
* Prepare and review Certificates of Analysis (CoAs) and other technical documentation.
* Ensure all documentation complies with internal SOPs and external regulatory standards.
* Adhere to and enforce Good Laboratory Practices (GLP), Good Manufacturing Practices (GMP), and other relevant quality standards as well as relevant guidelines, such as FDA, USP.
* Participate in quality investigations (e.g., OOS, deviations) and support root cause analysis and CCP development.
* Collaborate with the Quality Assurance team to review, revise, and maintain SOPs.
* Support internal and external audits by ensuring laboratory readiness and documentation accuracy.
* Provide day-to-day supervision, mentorship, and training to laboratory technicians and junior analysts on equipment usage, testing protocols, and laboratory practices.
* Assign and oversee daily testing activities to ensure efficient and compliant lab operations.
* Review and approve analytical results and reports, including peer review of data generated by junior staff to ensure accuracy and compliance.
* Support ongoing staff training and contribute to performance evaluations and development planning.

MINIMUM REQUIREMENTS:
* Bachelor's degree from a four-year accredited institution in a related field of applied science; plus, five or more years of related experience; or six plus years of related laboratory experience with no degree
* Strong understanding of principles, terminology, practices, techniques and instrumentation commonly used in a laboratory setting
* Ability to interpret data, record observations, prepare reports, and perform peer review
* Ability to effectively prioritize work and manage time to meet deadlines and rush orders
* Computer literacy
* Ability to work with delicate laboratory equipment
* Ability to communicate effectively in both written and verbal formats
* Ability to understand and adhere to established SOPs
PREFERRED QUALIFICATIONS:
