As Social Services Assistant you will be responsible for:
* Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.
* Assist in collecting social history data and in the development of a comprehensive discharge plan, including the MDS and Care Plan for new admits, as directed by the Social Services Director.
* Orient and assist new residents in adjusting to the facility. Assist residents and families with adjustment to changes, including room changes.

SUMMARY: The AHD Social Services Designee at Alameda Hospital (at Park Bridge) assists in planning, developing, organizing, implementing, evaluating, and directing social service programs, in accordance with existing federal, state, and local requirements, as well as hospital policies and procedures, to assure that medically related emotional and social needs of the resident are met/maintained on an individual basis. Performs related duties as required.
DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
