AYS (Adjudicated Youth Services) is a specialized residential program that supports female juvenile offenders, from all over the state, ages 12-18, by teaching them new skills and ways to cope with the growing pains of life and individual circumstances. This program combines therapy with 24-hour supervised group living, to enable youth to learn new skills and behaviors appropriate to their recovery process and individual treatment plans.

People who excel in this program have a passion for helping others. You must be able to build rapport with teen clients while upholding strong professional boundaries and maintaining program rules. A bachelor's degree in a related field, prior experience in a residential facility, or experience working with at-risk youth is preferred.

• Assist with discharge planning, including arrangements for home health services, DME, transportation, circulation of post d/c plan of care, mail survey to discharged residents, post discharge follow up calls in the log, document steps taken in the medical record under discharge planning etc. with appropriate documentation in the medical record.
• Assist with PASRR Level II paperwork and ensure it is completed timely and
accurately.
• All duties pertaining to social services office functions such as: filing, faxing,
phone calls, mailing etc.
• Assist with all room changes and document in the medical record. Follow up
within 48 hours of the room change and document.
• Coordinate with Activities staff to obtain personal items, clothing items and
adaptive equipment as needed.
• Assist with the ICP Medicaid process, including applications for Medicaid.
• Social Services Assistant is to complete all 3-day Medicare letters in the absence of the Social Services Director, after appropriate training has been provided.
• Complete assigned social services audits on a PRN basis to insure compliance
with state and federal regulations pertaining to advance directives and social
services documentation.
• Assist with any complaints or formal grievances filed by residents and/or
responsible parties. Provide investigative results to the Administrator and Social
Services Director as applicable. Ensure that the Grievance log is up to date and
grievances are complete and filed appropriately in the Grievance book.
• Coordinate and assist with all departmental referrals, including dental, eye care, hearing, podiatry, psychiatry and psychological support.
• Assist the resident and/or responsible party with the application process for Social Security representative payee paperwork as needed.
• Social Services Assistant is encouraged to attend extraneous meetings for
marketing purposes etc. under the direction of the Administrator and Social
Services Director.
Qualifications:
- Must be able to read, write and communicate in English
- Must be able computer proficient with applicable programs i.e. Micro-soft word, Excel, Outlook and Giftwrap etc.
Education and Experience:
- High School Diploma required.
- Must have at least 3 years experience in related field.
- Long Term Care preferred.
