LifeLong Medical Care social work associate job description

Taking part in community health is rewarding and meaningful. LifeLong Medical Care is seeking a part-time or full-time Licensed Clinical Social Worker (LCSW) or Associate Social Worker (ASW) to work with a multi-disciplinary collaborative team. Under the direction of the Program Manager, the LCSW/ASW provides comprehensive social services to residents in the respite program. LCSW licensure hours are provided for ASW as well as clinical training opportunities.

The Medical Respite Program consists of 4 apartments with 15 bedrooms surrounding a courtyard and has a daily census of 27 individuals. The program provides integrated medical and cares management services to homeless adults, most of whom are recently discharged from hospital care. This is a short-term (up to 45 days) respite facility providing 24/7 staff support.

Benefits:
We offer a competitive annual salary of $68,000-$72,000 for ASW and $80,000-$100,00 for LCSW, depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs, and commuter benefits. LifeLong is qualified for federal and state loan repayment programs (HPSA score is 16). Acceptance rates may vary year to year based on funding levels and the number of applicants.

Responsibilities
Provides counseling, psychosocial support, and crisis intervention to residents. Coordinates and conducts training and clinical supervision for MSW interns and consults and guides other program staff. Provides timely and accurate documentation using appropriate EHR systems Performs comprehensive age and culturally appropriate psychosocial assessments on residents
in conjunction with the resident, formulates a social service care plan;
Refers and links residents to appropriate substance abuse treatment, mental health treatment, case management programs, home care services, transportation programs, domestic violence resources, and appropriate community social services resources, including those required by mandatory laws such as child and elder abuse
Maintains ongoing communication and coordination on resident's care plan and psychosocial status with clinic staff, other community programs, and in-patient providers to ensure continuity of care.
Participates as a member of a multi-disciplinary care team.
Assists and educates residents on financial and medical entitlement programs, provides appropriate referral, and ensure linkage to these programs with assistance from the care management team.
Provides information on housing options, assistance with a referral, completion of applications, and gathering of needed documents to assist residents in obtaining temporary and permanent housing.
Performs community outreach and acts as liaison with other health care and social services providers
Participates in Respite staff meetings.
Participates in gathering and reporting outcome measures and quality improvement efforts.
Develops and facilitates groups and 1:1 therapy session to assist patients in coping with health-related and social services issues;

Qualifications

Social Emotional Learning Associate
Reports to: Senior Director of Social Emotional Learning


Organization Overview:
The Steppingstone Foundation (TSF) prepares students from historically marginalized communities to access, navigate, and graduate from college through two initiatives, the Steppingstone Scholars Program and the National Partnership for Educational Access (NPEA). The Steppingstone Scholars Program is a direct-service program in Boston that provides academic, social-emotional, and college-readiness services to students, called Scholars, beginning as early as fifth grade and continuing all the way through college graduation. The Steppingstone Scholars Program consists of five components: Admissions, the Academy, Middle School Success, High School Success, and College Success. NPEA connects the people, practices, and innovations for eliminating barriers to educational access, and college and career success for historically marginalized students.


Department and Position Overview:
At Steppingstone, we believe that social emotional learning is essential to Scholars' success in middle school, high school, and beyond, and that this learning needs to be infused into all parts of the program for it to be most effective. To this end, the SEL programming at Steppingstone is largely preventative, consisting of community building programming, psychoeducational curriculum, an Advising system based on strong relationships, and staff professional development.


The SEL Associate will work with the Senior Director of Social Emotional Learning to ensure the social and emotional needs of Scholars are incorporated into all aspects of Steppingstone's programming. The majority of the SEL Associate's time will be spent helping to design and implement Steppingstone's psychoeducational curriculum, working to identify the SEL needs of Scholars and families and recommending and developing relevant programming. The SEL Associate will also be a member of several collaborative teams, helping to plan professional development for staff and initiatives to create a positive program climate and culture. There will also be opportunity to work with Scholars and families, providing short term 1:1 interventions, small groups, and referrals.


Academic Year Schedule:
The SEL Associate will work a Tuesday-Saturday schedule during the school year. Wednesday afternoons during the school year end around 6:30 p.m., and occasional additional evening work is mandatory. In person programming occurs at Boston Latin Academy on Saturdays and the SEL Associate will be expected to be at that location from 8:00 a.m. - 3:00 p.m.
During the school year, this position will be a hybrid role with two days in office, one day at the program site on Saturdays, and two days remotely, with some flexibility around work hours.


Summer Schedule:
During the summer, employees will work a Monday-Friday schedule. Steppingstone hosts a 6-week in-person program for Scholars, which will be held at Milton Academy in July and August. The SEL Associate must be able to work in person at Milton Academy, Monday-Friday from approximately 7:30 a.m. - 3:45 p.m., and until 5:00 p.m. on Wednesdays for faculty meetings. Due to the nature of the summer session, PTOs are granted on a very limited basis from the end of June through August.
A $500 home office set-up stipend will be provided upon hire, followed by quarterly stipends of $50. Laptops are provided to each employee to enhance the hybrid experience.


Salary: $49,000
Start Date: October 2022


Responsibilities:
Support the development and implementation of Steppingstone's social emotional curriculum Help to ensure effective social emotional support of all Scholars through consultation with Advisors, participation in weekly Advising Team meetings, and co-planning trainings on topics such as working with diverse populations, group facilitation, and stress management Assist in the creation of developmentally appropriate community building activities and events to support a positive program culture Assess Scholar needs, provide short term intervention as appropriate, provide coaching to Advisors, and/or make referrals to outside resources Utilize data collection, tracking, and documentation to inform program improvements and monitor Scholar progress Help to deliver the core program by supporting the admission process and providing logistical support for and participating fully in the summer session
