About this position
The Licensed Graduate Social Worker (LGSW) provides services to customers and their families to help them adjust to physical or mental illness, injury or hospitalization. Assists the interdisciplinary treatment team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for the biopsychosocial assessments and discharge planning for customers with complex psychosocial and medical and/or mental health problems in collaboration with the responsible family and case management team. Education

Experience

Responsibilities
Crisis Assessment and medical/behavioral emergency
Conducts variety of assessments such as suicidal, Vestibular Assessment, immediate needs, functional, living skills, and resource acquisitions
Complete Interpretative summaries
Develops crisis/treatment/behavioral plans and goals for clients
Writes progress notes, summaries, and discharges
Group and Individual Counseling services
Skills Teaching
Case Management
Household management
Implements the treatment/rehabilitative activities and strategies
Crisis Prevention
Medication Administration
Participates in weekly staff meetings
Reports significant / major incidents or accidents
Establishes high levels of mutual trust and support with others.
Provides coaching/learning environment
Provides and promotes consumer choice and facilitates independence
Able to perform duties independently and as a team
Promoting the operation in a positive manner to the community and outside agencies.
Promotes/encourages community integration and choice making

Qualifications
At least a bachelor's degree in one of the behavioral sciences from an accredited college or university
Minimum of 2000 hours (post Bachelor degree) of verifiable work experience providing services and support to individuals with mental illness
Or, Master's degree in one of the behavioral sciences from an accredited college or university
Valid driver's license and car insurance required
Supervisory experience preferred
Ability to communicate verbally and in writing
Experience in managing systems, processes, and people
Operate general office equipment
Good organizational skills and attention to detail
Capable of working responsibly with highly confidential information
Must be able to work independently as well as part of a team
