The Inpatient Master of SocialWork (MSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management MSW is to ensure the continuity of care for vulnerable patients by identifying needed resources to address social, financial, cognitive/behavioral or legal barriers to care access.
The Inpatient Licensed Clinical SocialWorker (LCSW) Care Manager provides professional, comprehensive patient centric care management services for at risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. The goal of the Inpatient Care Management LCSW is to ensure continuity of care for vulnerable patients by identifying and addressing social, financial, cognitive/behavioral or legal barriers to accessing necessary resources and care.

The Social Service Specialist:
Exemplifies excellent customer relations with clients, visitors, and other employees; shows courtesy, friendliness, and respect.
Introduces clients to integrated MSS services (either in home or at clinic) and performs systematic social services screening to determine current status and needs in a variety of areas.
Based on identified client needs, makes referrals to appropriate agencies/services or provides information and assists clients for self-referral.
Provides advocacy as needed to encourage participation in implementing the care plan or to resolve problems which are interfering with active participation in the care plan (such as transportation or language barrier).
Charts notes of the services provided as required by the state and agency in Sea Mar's Electronic Health Records.

Maintains regular face-to-face contact with client in order to monitor implementation of the care plan.

Reports any unusual incidents to their manager or the Preventive Health Services Director. The Social Service Specialist is a state mandated reporter, as such; he/she will report to the state any suspected case of child/elder abuse or neglect as mandated by WAC.
Complete a thorough, culturally sensitive, psychosocial assessment which includes but is not limited to: Personal history (cultural background, health status, coping strategies, mental health status, substance use, abuse and violence); current family functioning; review of support systems (formal and informal); and basic physical needs. This assessment is shared with the client and team members to assure that all relevant psychosocial issues are addressed in the care planning.
Provide brief, culturally relevant, counseling to client/families which would include: crisis intervention; mental health concerns; pregnancy and birth options; grief; stress; parenting; coping skills; problem-solving; abuse/neglect; family violence. Educate the client to make informed decision regarding personal, child and family health/mental health care and treatment. Brief counseling is defined as counseling to assist the client in developing some skills to cope with issues presented and developing a plan to address ongoing longer term treatment with appropriate mental health providers.
Communicate clearly, oral and written, with clients, community professionals and interdisciplinary team members. This includes maintaining records that are clear and concise regarding psychosocial and/or needs assessment, service plans, treatment and follow-up care. The documentation must reflect legal mandates for reporting abuse/neglect.
Participate fully in all interdisciplinary team meetings to provide input on client's assessed psychosocial needs and on interdisciplinary team plans for client. The goal is to build family strengths and resiliency.
Develop working relationships within the service system network in particular services that have significant impact on pregnant and parenting clients such as: Mental Health and Chemical Dependency systems; network within service providing community serving this population to develop strategies to improve services delivery.
Demonstrate knowledge and skills necessary to provide care appropriate to the age of the client served.
Demonstrate knowledge of the principles of growth and development over the life span.
Able to identify and categorize each patient's age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients.

Your Job as a Substance Use Counselor:
The Substance Use Counselor is instrumental in our patient's treatment and recovery from opioid use disorder. Substance Use Counselors provide high quality, compassionate care through various mediums which include both individual and group counseling sessions.
Job Responsibilities:
Provide high quality, compassionate guidance in both individual and group counseling sessions.
Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.
Ensure all documentation regarding patient care, treatment, and incidents is completed timely and in a clear, concise manner.
Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.
Evaluate patient needs and determine if referrals to other programs or facilities are needed.

Compensation for roles at Bellingham CTC varies depending on a wide array of factors including but not limited to the specific location, role, skill set, and level of experience. Qualifications
Required Education, Skills, and Qualifications:
HS graduate or equivalent required;
Prefer Bachelor's or Master's degree in social or health services field;
Degree must be from an accredited college or university.
Previous experience in addiction recovery or behavioral health settings, such as outpatient, residential, or correctional facilities.
Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.
Experience conducting individual and group counseling sessions focused on substance use recovery.
Knowledge of evidence-based practices, such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and relapse prevention techniques.
Documentation experience, including maintaining clinical notes, treatment plans, and progress updates in accordance with state and federal guidelines.
Familiarity with state regulatory standards (e.g., 42 CFR Part 2, HIPAA, Joint Commission standards).
Experience collaborating in a multidisciplinary team with medical staff, case managers, and peer support specialists.

* Perform community outreach to individuals with serious mental illness, including visiting their homes and other locations within the local community where they spend time, such as clubhouses, drop-in centers, homeless shelters, hospitals, and emergency departments.
* Empower individuals to identify and pursue their personal recovery goals by modeling and teaching safe and healthy coping strategies.
* Engage in care coordination to identify health related social needs and connect individuals with local resources to address needs.
* Assisting in successfully attending and completing clinical visits and assisting with telehealth access
* Assist with gathering information required for obtaining benefits and services and facilitate warm hand-offs to other team members for assistance in navigating SSI/SSDI benefits and self-management support.
* Address immediate follow-up needs identified by care team providers such as hospital post-discharge follow-up and support.
* Promote and monitor individuals' success in adhering to their personally approved care plan and provide motivational interviewing support for their adherence to their informed medication and treatment choices

The experience you bring to this role includes:
Required:
* Lived experience with SMI and/or SUD
* Qualified as a Community Health Worker OR eligible and willing to complete coursework within first 3 months and pursue certification as soon as possible but no later than 1 year of employment
* Ability to operate a vehicle including a driver's license that is active and in good standing; must be eligible to drive under Company insurance policy
* Intermediate working knowledge using Google Workspace Programs specifically Gmail, Sheets, and Docs.
* Exceptional communication & interpersonal skills with the ability to build rapport with internal and external customers and stakeholders.
* Decision making skills regarding own work approach/priorities, and work assignments, standards and resources.
