Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The Family Intervention Specialist:
Carries a caseload of 4 to 6 families
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)
Collaborates with other providers, case workers, and courts to formulate a collaborative treatment plan
Provides on-call crisis support to the youth and family (schedules vary by location)
Completes accurate and timely documentation in an electronic medical record system (EMR)
Performs other duties as assigned
Additional Information:
Schedule is flexible and non-traditional as it is based around the availability of youth and families served.
Applicants must possess a current, valid driver's license, an automobile for work purposes, and proof of auto insurance.
Community-based staff will be reimbursed for applicable mileage.

International Health and Medical Services is looking for an experienced Behavioral Health Specialists to assess, diagnose, and treat mental health disorders among patients, ensuring they receive appropriate psychiatric care and support. This role includes collaborating with the healthcare team to implement treatment plans, monitor progress, and provide crisis intervention as needed, fostering mental health and well-being within the facility.
Key Responsibilities:
Provides direct psychiatric care to patients via onsite and telepsychiatry services for scheduled appointments, emergency, or urgent care visit.

It is expected that student intern will maintain ethical standards and follow all BestCare's policies as well as State and Federal regulations throughout the program to guide their development into professional clinicians. The following represents the primary opportunities and responsibilities that a Student Intern may expect during the internship.
ESSENTIAL OPPORTUNITIES & RESPONSIBILITIES:
The Student Intern can expect to work in a diverse, clinical setting with clients that are experiencing a variety of hardships.
At the beginning of the internship, the Student Intern will receive and complete required/assigned training as well as a site-specific orientation.
A qualified, designated Clinical Supervisor will serve as a resource and will be responsible to timely provide all necessary documentation and coordination with the affiliated university. The Clinical Supervisor will meet all requirements under OAR 309-019-0125.
The Student Intern, with the Clinical Supervisor, develops goals for the internship (Student Scope of Practice and Education Agreement), identifying areas of particular interest to the intern, as well as goals related to the competencies of the profession. These goals will be reviewed and updated throughout the internship at least every 3 months to evaluate progress towards goals, identification of deficiencies, and development of a plan to address any deficiencies.
The Student Intern will participate in a variety of shadowing opportunities with the entire outpatient team, including other program offerings to learn the entire array of services provided based on the program site, which may include case management, psychiatric management, providing therapy, medication-supported recovery, MOMs program, and substance use/mental health counseling and therapy.
After completing the shadowing opportunities and being approved by the Clinical Supervisor, the Student Intern will begin practicing therapy/case management on their own for a small caseload, under continuing supervision.
The Student Intern will be provided the designated weekly supervision, as well as individually and in group settings with the clinical team. Weekly, 1-hour, individual supervision will include but is not limited to the review of cases relating to the:
Diagnosing and making appropriate treatment recommendations
Developing service plans and setting goals for treatment
Termination of therapy services, review of documentation to ensure appropriate, thorough, and accurate documentation, and coding of services
Opportunities will be provided for observation through provision of direct clinical services under supervision;
Opportunities will be provided to learn about other aspects of their profession including, but not limited to, burnout and compassion fatigue, self-care, theoretical orientations/interventions, proper documentation in EHR (Electronic Health Records), transference and countertransference, boundaries, legal considerations, and testifying in court, etc.

Demonstrated cultural responsiveness, effective communication and competence in care coordination, development of working alliances with individuals, inter- and intra-agency collaboration, and the rendering of services and supports within their scope and in accordance with the service plan, including transition planning; and
Demonstrated basic clinical competency in the following areas: diagnostics, cognitive behavioral therapy, acceptance and commitment therapy, motivational interviewing, trauma informed care, foundational knowledge in substance use disorders, and suicide risk assessment and safety planning. If topic areas have not been trained up in, willingness to participate in designated training to scale up skills is recommended. SUCCESS PROFILE

POSITION SUMMARY
Educators assigned to the position will perform as a teacher leader and content area specialist who facilitates academic and positive behavioral professional development related to the school and district's instruction, curriculum and behavioral goals. The coach will work with individuals and teams of teachers on goals aligned with demonstrated instructional/professional needs.
As a leader and specialist, the employee works with administrators, coaches, teachers and support staff in identifying, developing, and implementing curriculum and assessment, preventative and proactive behavioral practices, and research based instructional strategies designed to improve student learning. As an instructional coach, the employee may work in a variety of elementary and/or secondary educational, individual or team teaching settings. Instructional coaches share in the responsibility for student safety and appropriate collaboration and attention to each student's readiness to learn including needed guidance, preventative and proactive behavioral strategies, and welfare.

Identifies victims of child abuse, domestic violence, and elder abuse. Makes appropriate referrals and notifies the Department of Human Services (DHS) and/or law enforcement when applicable.
Provides information, education and resource referrals related to advance directives, power of attorneys, guardianship, conservatorship, and long-term care services. Assists in navigating legal avenues for guardianship and navigating pathways through local and state resources.
Gathers and documents data promptly, ensuring compliance with department standards. Maintains accurate records of patient interactions, assessments, and interventions. Collaborates with electronic health record systems to document socialwork interventions.
Conducts all activities with the highest standards of professionalism and confidentiality. The Internship Program is designed to serve as a bridge between the knowledge they gain in completing required coursework and applying that knowledge to clinical client services. It is expected that student intern will maintain ethical standards and follow all BestCare's policies as well as State and Federal regulations throughout the program to guide their development into professional clinicians. The following represents the primary opportunities and responsibilities that a Student Intern may expect during the internship.
ESSENTIAL OPPORTUNITIES & RESPONSIBILITIES:
The Student Intern can expect to work in a diverse, clinical setting with clients that are experiencing a variety of hardships.
At the beginning of the internship, the Student Intern will receive and complete required/assigned training as well as a site-specific orientation.
A qualified, designated Clinical Supervisor will serve as a resource and will be responsible to timely provide all necessary documentation and coordination with the affiliated university. The Clinical Supervisor will meet all requirements under OAR 309-019-0125.
The Student Intern, with the Clinical Supervisor, develops goals for the internship (Student Scope of Practice and Education Agreement), identifying areas of particular interest to the intern, as well as goals related to the competencies of the profession. These goals will be reviewed and updated throughout the internship at least every 3 months to evaluate progress towards goals, identification of deficiencies, and development of a plan to address any deficiencies.
The Student Intern will participate in a variety of shadowing opportunities with the entire outpatient team, including other program offerings to learn the entire array of services provided based on the program site, which may include case management, psychiatric management, providing therapy, medication-supported recovery, MOMs program, and substance use/mental health counseling and therapy.
After completing the shadowing opportunities and being approved by the Clinical Supervisor, the Student Intern will begin practicing therapy/case management on their own for a small caseload, under continuing supervision.
The Student Intern will be provided the designated weekly supervision, as well as individually and in group settings with the clinical team. Weekly, 1-hour, individual supervision will include but is not limited to the review of cases relating to the:
Diagnosing and making appropriate treatment recommendations
Developing service plans and setting goals for treatment
Termination of therapy services, review of documentation to ensure appropriate, thorough, and accurate documentation, and coding of services
Opportunities will be provided for observation through provision of direct clinical services under supervision;
Opportunities will be provided to learn about other aspects of their profession including, but not limited to, burnout and compassion fatigue, self-care, theoretical orientations/interventions, proper documentation in EHR (Electronic Health Records), transference and countertransference, boundaries, legal considerations, and testifying in court, etc.

Minimum number of hours per week as determined by the Collaborative Educational Agreement, with a stable and reliable schedule to be coordinated and designated with the onsite clinical supervisor.
Demonstrated ability to learn the skillset to conduct basic competence in the following essential job functions: facilitation of mental health assessment, completion of treatment plans, facilitation of mental health psychotherapy treatment for individuals and within a group therapy setting;
Demonstrated cultural responsiveness, effective communication and competence in care coordination, development of working alliances with individuals, inter- and intra-agency collaboration, and the rendering of services and supports within their scope and in accordance with the service plan, including transition planning; and
Demonstrated basic clinical competency in the following areas: diagnostics, cognitive behavioral therapy, acceptance and commitment therapy, motivational interviewing, trauma informed care, foundational knowledge in substance use disorders, and suicide risk assessment and safety planning. If topic areas have not been trained up in, willingness to participate in designated training to scale up skills is recommended. This should be discussed with onsite supervisor at start of practicum for learning plan clarity;
Ability to develop strong collaborative relationships with the clinical team, willingness to participate in a variety of shadowing opportunities with the outpatient teams;
Ability to get along and work collaboratively and effectively with the clinical team;
Ability to work within the scope of practice and competencies identified by the collaborative educational agreement and the policies and procedures for the credentialing of clinical staff as established by the provider and the graduate program;

Adherence to program procedures for client documentation within designated electronic medical record. SUCCESS PROFILE

DEPARTMENT SUMMARY: St. Charles Health Systems Acute Social Services and Psychiatric Assessment Team (PAT) positions may support anyone, or a combination of the following departments: Social Services, Home Health, Behavioral Health, or Hospice. Services provided to the community and region include the following: Acute mental health crisis and triage, inpatient psychiatric hospitalization, treatment for SPMI population, substance use triage, local, regional and state resources for long term care access needs, and collaboration with the outpatient community at discharge. These departments provide system-wide support including, but not limited to working with multi-disciplinary team to assess needs of patients and families, provide appropriate resource referrals, provide supportive counseling as needed, coordinate discharge planning and provide other identified services as needed to support those affected by chronic or acute illness.
POSITION OVERVIEW: The Mental Health Practitioner/Social Services Specialist provides counseling services including assessment, diagnosis, and treatment for patients experiencing medical/social/behavioral/mental health concerns. Discharge planning; supportive counseling and brief interventions. Assist in meeting the psychosocial needs of patients and their families to help patients utilize services to achieve optimum level of function and to coordinate care throughout the continuum of care.
This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Participates with team members in facilitating patient's and family's learning through the continuum of care. Reinforce patient's continued health care through teaching and/or referral to community agencies for follow-up needs.
Provides diagnostic, preventative and treatment services of a psychosocial nature pertaining to personality adjustment, behavior problems and interpersonal dysfunction.
Respects privacy and protect the confidentiality of patients and family members while maintaining boundaries of the professional discipline as outlined in the affiliated licensing board's Code of Ethics.
Functions as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost-effective manner.
Provides individualized and family-based therapies using a variety of evidenced-based treatment modalities, interventions, and strategies primarily from a DBT, ACT, CBT, Solution-Focused, Motivational Interviewing, or another approved modality.
Works in a fast-paced environment at times multi-tasking and triaging numerous needs of patients, their families and community agencies.
Completes psychosocial assessments (including substance use), economic assessments, risk assessments, and treatment plans that are individualized and measurable in compliance with the department and state regulatory standards.
Assesses the potential threat of harm to self-and/or others and/or inability to care for oneself. Determine in coordination with the treatment team and county, the legal status of patients in accordance with the treatment team and County, the legal status of patients in accordance with State or Tribal Civil Commitment laws.
Provides crisis intervention to a patient in immediate crisis in the emergency department or hospital inpatient unit(s) and to recently discharged patients on an as-needed basis.
Provides psychoeducation and case management support to patients and their families. Including psychosocial counseling, support, information and referral to patients and families regarding illness and related issues.
Responsible for maintaining knowledge of all state and federal regulations as they apply to the admission, care, and discharge of patients including all pertinent civil commitment procedures and protocols.
Coordinates after-care with outpatient providers including therapeutic appointments, warm handoffs, medication management, and follow up medical appointments that are in compliance with regulatory standards and/or other metrics.
Attend unit meetings and trainings as scheduled.
Identifies victims of abuse (child, domestic and elder). Make appropriate referrals and notification to appropriate services, including the Department of Human Services and/or law enforcement when applicable.
Works in concert with the psychiatrist to admit patients to acute psychiatric inpatient treatment by developing a working diagnosis and treatment goals.
Additional Duties Specific to PAT role:
Conducts crisis, risk, and psychosocial assessments, treatment planning, and safety planning within the guidelines of OHA and Joint Commission standards.
Performs daily rounding of patients in the emergency room or admitted to medical units of the hospital for purpose of assessment and/or ongoing treatment.
Follows up risk assessment and safety planning for patients scoring a moderate to high-risk through the hospital system.
Review referrals for psychiatric inpatient beds requests and provide coordination of admission for patients accepted for admission.
Attends bi-weekly meetings with both PAT and Acute Social Services Manager.

Required: Code Grey Training is required with 3 months of hire. Ability to travel to business functions/training/meetings and all SCHS worksites.
Preferred: Current unrestricted Oregon LCSW, LMFT, or LPC licensure. Member of the National Association of SocialWorker's or another national association membership.
Bilingual in Spanish and English communication.
EXPERIENCE:
Required: One year experience in a healthcare/hospital or behavioral health inpatient/residential setting. Knowledge of community resources. Internship experience in an emergency department/inpatient behavioral health setting may be considered.
Preferred: Two years of experience in a hospital or behavioral health inpatient/residential setting. Postgraduate, clinical experience in inpatient/outpatient settings, crisis assessment and/or community organizations that work to support planning and administering of community issues and needs. Essential Responsibilities:

Based on the needs and values of the client, and in collaboration with all service providers, the clinician links clients with appropriate providers and resources throughout the continuum of health and human services and care settings, while ensuring that the care provided is safe, effective, client-centered, timely, efficient, and equitable.
Clinicians have direct communication among, the client, the payer, the primary care provider, and other service delivery professionals. The case manager is able to enhance these services by maintaining the client's privacy, confidentiality, health, and safety through advocacy and adherence to ethical, legal, accreditation, certification, and regulatory standards or guidelines.
Interact with other PacificSource personnel to assure quality customer service is provided. Act as an internal resource by answering questions requiring medical or contract interpretation that are referred from other departments, as well as physicians and providers of medical services and supplies. SUCCESS PROFILE

OR Registered Nurse with current appropriate unrestricted state license based on line of business: Commercial and Medicare: Oregon, Idaho or Montana, and Washington; Medicaid and DSNP: Oregon. Certified Case Manager Certification (CCM) as accredited by CCMC (The Commission for Case Management) strongly desired at time of hire. CCM certification required within two years of hire.
