Develops and monitors objectives/strategies to address AOD risk factors, increase protective factors and ensure treatment delivery according to the risk-need-responsivity principle. Monitors youth progress in AOD specialized treatment; updates treatment objectives for assigned youth in accordance with CMS; and completes transition planning and discharge summaries to ensure continuity of care and management of risk factors for relapse.Conducts program orientation sessions. Completes all required electronic medical record training, MTF-specific orientation and SNC training programs. Consults with medical personnel, legal authorities, military commanders and school districts, as required. Attends and participates in meetings during normal duty hours and professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, commander's staff meetings, primary care staff meetings, and others required by applicable regulations, MTF guidance, or as directed by department leadership. Qualifications Minimum Qualifications: * Education: Master's degree in Social Work (MSW). Graduate from a School of Social Work fully accredited by the Council on Social Work Education (CSWE). * Experience: Documentation of at least three years and 4,500 hours of paid, supervised post-professional experience in an organization of agency that provides case management services. * Licensure; Current, full, active, and unrestricted LCSW license from any state. * Life Support Certification: Possess and maintain a current AHA or ARC BLS Healthcare Provider certification.
