PURPOSE OF POSITION
This position is accountable for identifying the social, psychological, spiritual and emotional strengths and needs of residents and the provision of services to meet such needs for all residents on assigned units, thus alleviating the trauma associated with nursing home placement. This position has the responsibility for assuring compliance with the Resident's Bill of Rights, the Patient's Self-Determination Act as well as meeting the requirements of the State and Federal regulations and facility policies and procedures on a continuing basis. This position cooperates with other facility disciplines in the carrying out of responsibilities and assuring that the needs of the residents are being met in a pleasant working atmosphere.
NATURE AND SCOPE
This position reports to the Healthcare Administrator. In performance of duties this position is entrusted with, but not limited to, the following responsibilities:
· Assess the psycho-social strengths and needs of residents on the unit to which the socialworker is assigned. Develop and maintain a social history, which becomes a part of the resident's permanent medical record.
· Provide intensive and/or continuing counseling services to residents and families on assigned nursing unit during the adjustment period of placement and thereafter as needed. Assist in orientation of families to the facility.
· Attend and facilitate family conferences, discharge conferences and patient care conferences.
· Maintain an awareness of community resources. Act as a liaison with social, health and community agencies. Coordinate discharge planning, transfers and room changes of residents for whom it is appropriate.
· Serve as an advocate for residents, including those instances in which appointment of a legal conservator, guardian, durable power of attorney or trustee is appropriate.
· Complete required documentation, including MDS and quarterly summaries.
· Serve as a member of the admissions committee. Participate in other committees and attend meetings, which have a direct impact on quality care for residents.
· Assist as needed in marketing the facility to prospective residents, their families and the community at large. Maintain communication and positive working relationships with referral sources. Demonstrate an attitude of customer service in all situations.
· Develop, and update as needed but a least annually, department policies and procedures.
· Participate in the Keynotes Speakers Group as requested.
· Develop and facilitate education/support groups as requested.
· Complete necessary social service paperwork on admission and/or as needed, i.e. PSDA. Residents Bill of Rights, facility policies and electronic submission of MDS data, etc.
· Assume duties of a weekend manager on a rotating basis with other facility staff.
· Provide in-service education on PSDA requirements advance directives, resident's rights, etc.
· Other duties may be requested or assigned by the Healthcare Administrator.
CUSTOMER SERVICE AND PERFORMANCE EXPECTIONS
In this facility each resident, family member, visitor and co-worker is considered your customer and is vital to the success of our organization.
· Responds to resident or family concerns within 24 hours and follows through on resolution of issue.
· Assures smooth transition for new admission or resident transferred from other level of care. Source of referrals to other facilities in community. Familiar with outside professional services in order to accommodate residents' needs. Reports resident' change of health, physical or mental, to the Director of Nursing. Maintains complete confidentiality.
· Actively listens to resident and family complaints in order to resolve or to determine if requests require outside involvement.
Qualifications
QUALIFICATIONS
Graduate with a BA or BSW degree in the fields of SocialWork, Psychology or Sociology. Licensed by the Minnesota State Board of SocialWork is required. This position requires a person with a genuine interest and concern fore senior adults and who supports the overall mission of Covenant Retirement Communities. Must be able to communicate (written and verbally) in English with residents, families and staff. Minimum of two years experience in geriatric socialwork. JOB SUMMARY:
Catholic Charities is seeking a compassionate, detail-oriented, and self-motivated individual to join our BluePlus team as a SocialWorker I.
This position provides trauma-informed, culturally responsive support to individuals and families served through Catholic Charities programs. The SocialWorker I works directly with assigned clients to develop service plans, complete intake and annual assessments, and connect clients with appropriate community partners, resources, and Catholic Charities services.
The ideal candidate is dedicated to client success and social justice, skilled in documentation and advocacy, and able to navigate complex systems with care and professionalism. This role is responsible for maintaining accurate case records, adhering to funder and regulatory requirements, and providing education to clients and the community about available services and challenges.
ESSENTIAL FUNCTIONS:
Provides initial and annual assessments, education, and case management services to a complex, often long-term group of assigned clients, which includes in-home visits.
Prepares, evaluates, and maintains a service plan to meet the needs of the client including both Catholic Charities programs and other available community resources.
Supports and advocates on behalf of client in the procurement of services and benefits entitlement, assisting with required paperwork as necessary.
Consults cooperates, and educates community systems to facilitate linkage, referral, crisis management and follow up, focusing on attaining goals.
Maintains complete case documentation and records, as required by Catholic Charities, the funders, or licensure/regulatory bodies.
Coordinates with the supervisor on all aspects of client care and seeks appropriate guidance and feedback from other team members.
Utilizes behavior management, trauma-informed, person-centered, and solution-focused techniques to facilitate personal growth and minimize crisis.
May facilitate group or individual meetings to provide education, teach critical skills, and/or provide support for program participants.
Mentors other individuals within program.
May transport clients dependent on program guidelines and needs.
Other duties as assigned.
MINIMUM QUALIFICATIONS/EXPERTISE:
Current SocialWork licensure approved by the State of Minnesota required (e.g. LSW, LGSW).
Bachelor's or master's degree in socialwork, Psychology, Sociology, Human Services, or related field preferred.
Or a bachelor's degree with a major in any field and at least one (1) year of experience as a socialworker/case manager/care coordinator in a public or private social services agency.
Pass DHS background check and meet their requirements for the provision of case management by meeting the socialwork standards under the MN Merit System.
Additional training, education, or licensure may be required per program need.
Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
Must have a valid driver's license and proof of current insurance coverage

Social work internship job in Minneapolis, MN
This is a 0.6FTE (6 shifts per two week pay period) Acute Care Float SocialWork position at Fairview- The University of Minnesota Medical Center! This position floats to cover various units throughout the hospital including the ICU, Cardiology, Neurology, Ortho, Med/Surg, Observation and Emergency Departments. The hours are 8a-4:30pm. There is an every 6th weekend rotation and 1 holiday per calendar year requirement.
The acute socialworkerworks on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute socialworker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The socialworker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
Responsibilities
* Attends daily in person interdisciplinary care team rounds
* Conducts thorough psychosocial assessments with patients and/ or their surrogate decision makers
* Helps the care team navigate and mediate complex psychosocial dynamics
* Is well versed in the surrogate decision maker policy and assists care team members with identifying surrogate decision makers according to the policy
* Communicates frequently with the interdisciplinary team to ensure continuity and effective coordination of care
* Provides emotional support and counseling surrounding adjustment to sudden illness, grief/bereavement and end of life planning
* Addresses social determinants of health and assists with connecting patients to needed resources in the community such as chemical dependency treatment, domestic violence resources, housing stabilization services, mental health services, food shelves, transportation, financial assistance etc.
* Arranges and facilitates in person care conferences as needed to support patients and their families surrounding navigating complex medical decision-making processes/ goals of care
* Documents all interactions thoroughly and timely following established department standard work
* Serves as the expert in system adult and child protection reporting policies
* Collaborates with patients, families and interdisciplinary teams in developing an individualized discharge plan based on age, social and cultural considerations, clinical diagnoses, level of functioning and other relevant needs
* Participates in hospital initiatives and projects to reduce length of stay and related barriers which may include collaborating with providers for clinical adjustments in care
* Assists with ensuring the hospital is in regulatory compliance for care management related conditions of participation
* Assists with efforts to improve patient flow in a high-capacity hospital setting
* Works in a fast-paced hospital environment
* Effectively re-prioritizes competing time sensitive job demands
* Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery
* Incorporates data and information into practice to continually improve care and practice to enhance patient outcomes
* Supports site and system quality initiatives
* Provide age appropriate short- and long-term therapeutic interventions
