Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.
Completes patient psychosocial and an individualized treatment plan within the required time frame.
Identifies any clinical/case management needs and works to address those needs.
Performs individual, group, and family counseling as required.
Performs direct one-on-one patient contact through individual or group counseling.
Reports patient abuse, neglect and exploitation as required.
Reports patient grievances as required.
Educates patient in all aspects of treatment, corresponding health issues and steps to recovery.
Obtains Urine Drug Screens and initial patient photo identification.
Assists in monitoring all patient activities on center premises.
Actively participates in community relations activities as directed and authorized.
Ensures the reading and understanding of the Policy and Procedures Manual.
Ensures compliance with 42 CFR Part 2 (Federal Confidentiality Regulations) and 45 CFR, Parts 160 & 164 (HIPAA)
Is responsible for obtaining or maintaining proper licensure and/or certification according to specific state requirements.
Actively participates in preparation for surveys and inspections conducted by CARF, the State, DEA, Board of Pharmacy and any other agency site visits as dictated by the state laws or regulations.
Participates in all staff meetings
Ensures compliance with local, State, Federal and Colonial Management Group, LP rules, regulations and policies.
Always acts in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.
Performs other tasks as assigned by clinic, region or corporate leadership.
Minimum Qualifications:

ROLES AND RESPONSIBILITIES
The Family Support Specialist is responsible for making decisions and taking actions appropriate to ensure that every child has a permanent home that provides physical and emotional safety and that each region has the resources needed to provide support for families. Training & Capacity Building
Onboard, train, and mentor new Family Support Managers, ensuring alignment with program standards and core values.

Monitor FSM performance and provide constructive feedback to strengthen skills and confidence.

TIMING, LOCATION AND COMPENSATION
Better Together seeks to fill this position as soon as possible. The job is full-time and may require some work in the evenings and on weekends. In addition, Family Response Managers participate in our on-call schedule rotation. Better Together is headquartered in Naples, Florida. Ideally, candidates for this role will be based in Southwest Florida or surrounding areas. We are currently not considering applicants from the Panhandle, Miami, or North Florida regions.
A majority of your work time in this role will be spent in the community, helping families. Individuals in this role must be comfortable functioning and making decisions in crisis situations. This can involve personally helping individuals relocate to safe spaces and transporting to a safe space. Additionally, individuals in this role will be expected to attend weekly in-person or virtual team meetings and discuss the status of their cases with the team. There also will be quarterly in-person team meetings during which goals will be set and you will be expected to make suggestions on how to seize new opportunities.

Key responsibilities include:
* Provide rehabilitative and support casework geared to restoring residents to their optimum level of social health adjustment
* Help residents utilize the resources of their families and the community
* Assist residents and their families with personal and environment difficulties, which predispose them toward illness or interfere with obtaining maximum benefits
* Consult with the physician and other members of the care team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient's health problems
Qualifications
We seek the following principal qualifications:
* Graduate of a school of social work approved by the Council of Social Work Education with a Master's Degree

Job Description:
This is independent professional counseling work providing vocational rehabilitation services to eligible disabled individuals to attain employment; and providing technical assistance to vocational rehabilitation counselors.
This position is responsible for providing independent professional vocational rehabilitation counseling to individuals with a diversity of disabilities in order that they may attain employment; providing and coordinating complex services; and providing technical assistance on vocational rehabilitation issues to internal and external customers. The incumbent of this position will access client information, maintain case records, produce correspondence, and perform other automated functions within the scope of the position's responsibilities.
Assesses medical, psychological, and vocational information to determine eligibility for services. Provides technical assistance to internal and external customers. Develops and implements with full participation of the customer with a disability, an Individual Plan for Employment to determine employment outcomes.
Provides professional vocational rehabilitation counseling to individuals with a diversity of disabilities who require complex vocational rehabilitation strategies in order that they may attain appropriate employment outcomes. Provides information to assist individuals with disabilities in making informed choices regarding services, service providers and employment outcomes.
Authorizes, coordinates and/or negotiates services leading to successful employment outcomes. Negotiates with individuals with disabilities regarding plan development.
Devises job placement opportunities through job placement activities, including job analysis, job development, and labor market information to assist individuals with disabilities to obtain appropriate employment outcomes. Develops employer contacts and serves as liaison with employers. Utilizes the resources of community rehabilitation providers in securing appropriate job placement for eligible individuals with disabilities. Coordinates or collaborates with others in other placement activities. Provides technical assistance to internal and external customers.
Conducts in-depth interviews of individuals with disabilities regarding complex rehabilitation strategies to determine eligibility for vocational rehabilitation services and the feasibility for achieving a vocational goal.

Background Screening Requirement:

Summary
The Emergency Department Licensed Clinical Social Worker is experienced in rapid assessment, developing crisis management plans and aids physically ill patients and their families. Assists in resolving the social and psychological problems related to the illness. The Social worker sensitizes other health care providers about the social and psychological aspects of illness. The Licensed Clinical Social Worker provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high-risk populations with particular focus on frequent utilizers of the healthcare system. They will link patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning. They will provide support to patients and families regarding adjustments to disease process and illnesses. The Licensed Clinical Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning, and conduct screenings for CBT resource linkage.
