You'll play a dual role: supporting the emotional and psychosocial needs of patients and families, while leading the volunteer program to ensure timely, personalized service. As a Hospice SocialWorker (MSW) - Volunteer Coordinator, You Will:
• Perform psychosocial assessments, case management, education, and follow-up for hospice patients and families

• Address long-term care planning, advanced directives, and cultural/spiritual dynamics

About You
Qualifications - What You'll Bring:
• Master's degree in SocialWork from a CSWE-accredited institution
• Licensure as required by the state in which the hospice is located
• At least one year of experience in a healthcare and/or hospice setting (three to five years preferred)
• Deep understanding of the psychosocial dynamics of illness, loss, and death
• Compassionate and tactful when supporting patients and families facing terminal illness
• Ability to provide education, resources, and emotional support during times of high stress
• Comfortable supervising others and coordinating multi-person teams (volunteer or clinical)
• Organized and able to manage documentation, compliance, and volunteer engagement processes
Preferred Experience (Not required):
• Hospice-specific experience as a Medical SocialWorker
• Experience leading or developing volunteer programs
• Familiarity with EMR documentation systems and CMS regulations

* Serve as the designated crisis responder for LAN regions including Kankakee, Vermillion, Will/Grundy, Kane/Kendall, and South & West Cook.
* Complete the Crisis Intervention Intake Packet during each crisis encounter.
* Document all crisis activity in CareLogic within two hours of response.
* Facilitate family reunification, family-generated placement, or short-term agency placement as appropriate.
* Obtain approval from the After-Hours Crisis Supervisor before any placement occurs.

Regulatory and Compliance

The goal is crisis stabilization, safety planning, diversion from juvenile justice involvement, and support in reunification or short-term placement when needed.
This role requires strong judgment, calm decision-making, and the ability to respond effectively in fast-moving, high-pressure situations while maintaining professionalism, empathy, and adherence to agency protocols.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Respond to referring sources (police, hospitals, schools, etc.) by phone within 15 minutes and arrive on-site within 60 minutes (urban) or 90 minutes (rural).
Serve as the designated crisis responder for LAN regions including Kankakee, Vermillion, Will/Grundy, Kane/Kendall, and South & West Cook.
Complete the Crisis Intervention Intake Packet during each crisis encounter.
Document all crisis activity in CareLogic within two hours of response.
Facilitate family reunification, family-generated placement, or short-term agency placement as appropriate.
Obtain approval from the After-Hours Crisis Supervisor before any placement occurs.
Provide linkages, referrals, and resource coordination based on youth and family needs.
Remain accessible by cell phone throughout scheduled shifts.
Submit mileage, time sheets, and required documentation per agency protocols.
Participate in individual and group supervision, team meetings, and program meetings.
Follow Illinois mandated reporting requirements.
Complete all Federal, DHS, Agency Core Curriculum, and Mandated Reporter trainings.
Provide services to program clients throughout the full on-call shift.
Perform additional duties as assigned.

Complete pre-admission screening assessments for individuals needing higher levels of care
Coordinate with referral sources such as hospitals, law enforcement, schools, and partner agencies
Collaborate with internal teams-crisis clinicians, case managers, psychiatric providers, and leadership-to ensure timely, coordinated support
Document assessments, interventions, and follow-up in the electronic health record

Education & Requirements
Five (5) years of supervised experience in behavioral health
Bachelor's degree in a human services field
Graduate degree in counseling, psychology, socialwork, or related field preferred
CADC, LSW, or LPC strongly preferred
Must obtain CADC or licensure (LSW, LPC) within 2 years of hire
Minimum 21 years of age
Valid driver's license, reliable transportation, and proof of auto insurance
Prior experience in crisis services strongly preferred

JOB SUMMARY: The School Psychologist Intern has a foundation in both psychology and education and assists in the development of a school environment that assesses and supports students academically, socially, behaviorally, and emotionally.
ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily.
Implement a problem-solving framework as a basis for all professional activities.
Participate in and facilitate, as needed, RTI/Problem Solving meetings and special education collaboration meetings.
Collect data from multiple sources as a foundation for decision-making.
Use valid and reliable assessment techniques.
Observe academics and behaviors of students in classrooms for evaluation and problem-solving purposes.
Assist teachers, as time allows, in assessment administration.
Assist teachers in data collection, management, and analysis.
Make recommendations based on full individual evaluation data for special education or 504 eligibility, services and/or program placement, academic or behavioral accommodations, and community services.
Comply with district eligibility criteria when determining eligibility for special education services.
Meet all district, State, and federal timelines and requirements for special education evaluation and service delivery procedures.
Prepare clearly-written reports that address referral questions and provide guidance for intervention strategies in a timely manner.
Work within the school and community to facilitate implementation of recommendations.
Consult with teachers and other personnel regarding services and needs.
Contribute to the planning of district-wide programs and services.
Attend building, district, and other required meetings.
Attend professional meetings, trainings, and other activities for professional improvement.
Provide individual counseling, direct instruction, check in/check out, and other activities to enhance student supports in the school as time allows.
Other duties as assigned.
QUALIFICATIONS:
EDUCATION
Master's degree in school psychology, Illinois licensure as School Psychologist Intern
EXPERIENCE
2 years school-based experience preferred

TERMS OF EMPLOYMENT: Salary is $25,000

Overview of the Position:
Behavioral Health Specialists provide compassionate, paraprofessional support to individuals experiencing mental health and/or substance use challenges in a recovery‑oriented, person‑centered environment. Working within our Crisis Stabilization Unit, Residential Rehabilitation Unit, or Group Homes, you'll deliver direct care, skills coaching, safety monitoring, and daily living support that help individuals stabilize, build independence, and thrive in the community. This role is ideal for candidates who are reliable, teachable, and committed to trauma‑informed care, even in fast‑paced or crisis settings.
Core Responsibilities:
Support Daily Living & Recovery
Coach residents in ADLs (meal prep, hygiene, housekeeping, laundry) and independent living skills (medication routines, budgeting, time management).
Promote healthy routines: sleep hygiene, nutrition, movement, and community participation.
Client Engagement & Skills Coaching
Use motivational, strength-based approaches to build rapport and engagement.
Facilitate or co‑facilitate psychoeducational groups (coping skills, wellness, relapse prevention, community integration).
Crisis & Safety Support
Maintain therapeutic milieu and conduct scheduled safety checks.
Use de‑escalation techniques and follow crisis protocols; promptly seek clinical support when risk increases.
Support transitions between CSU, RRU, Group Homes, and community settings.
Medication & Health Support (non-clinical)
Support medication routines per program protocol (e.g., reminders, observation/ documentation when applicable).
