The Youth Advocate will assist Native youth in California Tribal TANF Partnership participant families and local partnership Tribes. This position will be responsible for educating youth in substance abuse disorders through workshops and providing preventative information to youth. This position will also send referrals to the appropriate facilities based on the needs of the Native youth. ESSENTIAL DUTIES & RESPONSIBILITIES The essential functions include, but are not limited to the following: Provides information on available programs in all counties in CTTP service areas for treatment and rehabilitation, and assists youth in applying for rehabilitation services. Conducts outreach programs at CTTP Sites and/or local Partner Tribes to help youth identify the signs of addiction and other destructive behavior, as well as steps to avoid such behavior. Develops and facilitates drug and alcohol life skills training, pregnancy prevention training, and other training that will assist youth in making healthy life choices. Maintains youth Participant records in regards to any referrals made on behalf of the youth, makes timely case notes in the TANF Assistant Software, and keeps the Caseworker updated on the progress of the youth participant. Maintains case management of caseload, making referrals and collateral contacts with multiple community agencies. The incumbent provides counseling services to individuals, families, children, youth and adults. ESSENTIAL DUTIES and RESPONSIBILITIES: Participates in the intake process and the evaluation of problems presented by clients. Develops appropriate treatment plans. Conduct pre and post assessments related to emotional health needs; Provide training to clinic staff in the area of mental health; Address Behavior Health referrals from Clinic Providers; Meet with individuals and or family members to assist them with functioning more effectively; Meet with Health Clinic patients to observe behavior, assess needs and creates appropriate treatment recommendations; Provide therapeutic services to patients and their families; Interview individual or family members to obtain information concerning behavioral health history, treatment and other pertinent information; Educate patients and their families about mental illness, treatment plans and resources; Facilitate regular multidisciplinary review meetings for the purpose of planning care and assessing the effectiveness of therapeutic interventions, group therapy process and program components; Implement and evaluate individual or group counseling treatment needs; Discuss progress toward goals such as medication regiment, family relationships, educational programs and other related problems; Complete and maintain individual and or group counseling notes and documentation; Provides crisis services as needed. May perform other duties commensurate with the functions and level of the position. MINIMUM QUALIFICATIONS: Education:Masters degree from an accredited college or university in clinical social work or closely related study. Postgraduate experience in acute outpatient mental health services, with specific experience in individual and group process counseling. Experience: One (1) year experience in a clinical setting. Licensing: Current LCSW or LMFT licenses from the State of California; Current Basic Life Support (BLS) Certification.
