Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility for to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy; Education provided to physicians, patients, families, and caregivers; and Leads a population of patients by service line and/or leads the team by being a resource to Tenet performance standards.
Responsibilities
This individual's responsibility will include the following activities:
a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff, and ancillary departments, I) assuring patient education is completed to support post-acute needs, j) timely complete and concise documentation in Case Management system, k ) maintenance of accurate patient demographic and insurance information, l) precepts new staff members and acts as a resource to all staff, m) facilitates TEMPO as needed, n) participates in department quality improvement initiatives, and o) other duties as assigned.

Social Worker, Early Intervention - LICSW

CHD's Early Intervention Program has an opening for a Licensed Social Worker. This is a FFS or Full-Time opportunity located in Springfield. The role of our EI Social Worker will be responsible for conducting assessments with other members of the Early Intervention team in the greater Springfield area in the client's homes. You will also be coordinating community resources, attending appointments, providing education on developmental issues with the families of our clients.
Responsibilities:
• Provide ongoing assessment of children enrolled in the program.
• Participate in arena transdisciplinary eligibility evaluations of infants and toddlers.
• Development of Individualized Family Service Plans (IFSPS), treatment plans,
approaches, and strategies.
• Instruct and coach parents in implementation of developmentally appropriate parenting
strategies, emphasizing social/emotional development and the importance of family
involvement, follow through and outcome/goals.
• Provide and request consultation to team members which may include caregivers, day
care providers, and/or educators.
• Provide direct treatment to children and families with significant social and/or emotional
needs.
Qualifications:
• Licensed Clinical Social Worker (LCSW) Licensed Independent Clinical Social Worker
(LICSW) by the Massachusetts Board of Registration of Social Workers.
• Candidates must have experience working with children and families.
• Meets (or is working toward) Department of Public Health requirements for Early
Intervention Specialist Certification.
• CPR (Infant & Toddler) and First Aid certification.
• High degree of personal organization.
• Verbal, written, and basic computer competence.
• Interest/commitment to providing family focused services to families of diverse ethnic,

Job Summary: The community embedded social worker provides clinical services/counseling, screening, and case management services to patients/clients/participants in one or more specific embedded community setting (distinct jobs in which services are provided at primary care or specialty medical practice offices; or working with persons who are unhoused; or working to provide team-based care to a specific population or setting) as an independently licensed practitioner. This work is typically as a part of an interdisciplinary team that could include medical providers (doctors, nurses, etc.), case managers, peer support services, housing advocates, and/or others. The work is often fast paced, and involves on-the-fly pivots according to the needs of the team, and the clients served.
QUALIFICATIONS:
Bachelor's degree in social work or other behavioral health discipline required. Master's degree in clinical social work preferred
Substance use counseling experience and expertise required. CDAC, LACD, or comparable certification in the state of Vermont is required.
If unlicensed, must be willing to work toward licensure.
Knowledge of and experience with population being served
Possession of a valid driver's license and access to a private vehicle for day-to-day job performance.
Working knowledge of computer skills

Position Summary:
The Social Worker is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, balanced with the patient's resources and right to self-determination.
