The opportunity for professional growth working at The Haven at Ramapo Ridge, New Jersey's first and only inpatient trauma- informed care program and which has received the Clinical Excellence in Behavioral Health award from the New Jersey Hospital Association (NJHA). Clinical supervision provided on site to those seeking hours toward LCSW licensure. Competencies: Conducts individual, group and family psychotherapeutic counseling and may function as primary therapist in identified cases. Completes psychosocial assessments including obtaining information for diagnosis and treatment as required by regulatory and accrediting agency requirements. Serves as liaison to patients and families and available community partners and resources. Maintains accurate, timely documentation as a basis for review by regulatory bodies, accrediting agencies and the interdisciplinary treatment team. Provides support, education and assistance to families and their support system regarding community resources, course of treatment and hospitalization, and discharge planning. Coordinates and facilitates post-acute care plan for assigned patients. They provide assistance to both patients and families to meet related social and emotional needs for the psychiatric condition, treatment, and recovery. Additionally, they assist with obtaining needed resources and coordinating discharge plans for patients in collaboration with physicians, and other members of the multi-disciplinary team. Expedites transition from acute care to next level of care throughout the healthcare continuum and develops interventions if required. Maintains working knowledge of community resources and assists in obtaining needed resources. Maintains working knowledge of STATE Mental Health Law, as well as national, state, county, and community standard and follows legal guidelines in mandatory reporting. Serves as subject matter expert and resource expert for mental health law related to the involuntary treatment of patients in the acute care hospital. Regularly assesses the mental status of patients in their care to determine acuity of illness and danger to self or others and takes immediate action if needed. Social work services include, but are not limited to, social work assessment, social work counseling, social work consultation, social work planning, social work community organization, social work policy, social work administration, social work research, and social work client-centered advocacy; Clinical social work services, under the supervision of a LCSW. Organizational Relationship: Reports to the Administrator Occupational Exposure Category: B: Limited exposure to blood borne pathogens Responsibilities: Receives the intake referral information and prepares paperwork/tools necessary. Obtains all pertinent medical history from patient, family or significant others. Performs home safety check and environmental assessment of the patient's home environment. Performs the socio-psychological evaluation of the support systems available to the patient and documents necessary emergency contacts etc. Assists patients and their families with personal and environmental difficulties which predispose then toward illness or interfere with obtaining maximum benefits from medical care (counseling members of the family to assisting patients with admission to a nursing home).
