Facility: Dayton Children's - Main Campus Department: SocialWork Schedule: Full time Hours: 40 Job Details: The Medical SocialWorker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff, to effect overall care coordination for the transition of care and complex discharge planning process. Through psychosocial assessment and intervention strategies, is responsible for assisting patients and their families in understanding and learning to cope with illness and disability. Assists families in resolving complex personal and environmental difficulties which interfere with effective social functioning and obtaining maximum benefit from medical care.
Participates as a professional team member in patient care planning along with the hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Responsible for the delivery of socialwork services which are available to the patient, the patient's family, and other persons significant to the patient, are well organized and directed, and integrated with other units and departments of the hospital and community. This position will collaborate interdepartmentally with all divisions of the hospital, inpatient, ambulatory, and Emergency.
SocialWorkers assume additional leadership duties to help with onboarding, training, coaching and job shadowing for newer staff, participate in the department's peer mentoring program for new staff around developing socialwork skills and completed additional special projects as requested. LISW SocialWorkers are eligible to volunteer work time to provide group supervision for independent licensure preparation for dependent licensed socialworkers and students, as well as attend other leadership opportunities as able.

Duties:
Conducts group therapy with Crisis Residential clients - helping them manage emotional regulation, relationships, and illness management and recovery.
Collaborate & communicate with all staff to assure appropriateness and safety of programming for each client.
Contribute to treatment process as needed. Assists with intakes, treatment plans, discharge planning, discharges and other unit responsibilities and client's activities of daily living.
Manage a case load of clients as needed (typically up to 5 clients, but possibly more depending on client census in the CSU); communicate with manager about the needs of clients; maintain a solid working knowledge of all clients in CSU to allow reporting of important information to subsequent shift members, external agencies, etc.;
Provide general case management services and referrals for clients while on the CSU; contact sources of collateral information such as family members, therapists, case managers, family doctors, etc.
Delegate non-clinical tasks to technicians as necessary (e.g. copying resource flyers; obtaining supplies, making coffee, cleaning rooms, etc.);
Perform linkage of clients to other mental health agencies via the hospital linkage program;
Serve as multi-disciplinary team member on CSU treatment team.
Provide discharge planning, crisis interventions, daily client care, and other duties as needed.

Overview
Under general direction, is responsible for professional correctional counseling work of average difficulty and related work as required. This is the working level class in the Correctional Counselor sub-series. An employee in this class supervises a caseload of incarcerated offenders inside a correctional facility. This class is flexibly staffed with and differs from the Correctional Counselor 1 in that an incumbent of the latter performs entry-level work. This class differs from the Correctional Counselor 3 in that an incumbent of the latter is responsible for leading and supervising others in counseling work.
Responsibilities
1. Calculates sentencing information to determine accuracy of an offender s sentence computation.
2. Verifies that offender sentencing complies with judgment order.
3. Completes classification/reclassification and/or risk-needs assessments for offenders.
4. Prepares reports on offender history, progress, adjustment, rule violations, visitation, recommendations for special visits, and other related statistics.
5. Evaluates all offender records, case notes, and information to ensure compliance with all applicable standards, laws, policies, rules, and regulations while under the guidance of a higher ranking Correctional Counselor.
6. Conducts and/or monitors Prison Rape Elimination Act screenings to determine potential victim or aggressor status.
7. Monitors environment to identify security related concerns in order to assure continued safety of facility.
8. Identifies appropriate intervention techniques to deal with offender behavioral issues.
9. Assists offenders in addressing concerns regarding job pay, trust fund, sentence information, and educational needs and programs and with communicating with outside agencies to expedite release plans or respond to emergencies.
10. Schedules outside services to assist with offender transition from prison to the community.
11. Develops and collaborates reentry plans with re-entry Counselor for offenders within established time frames and in accordance with departmental procedures.

POSITION DESCRIPTION:
Outpatient Children's Therapists provide individual, family and group psychotherapy for children and their families, provides crisis intervention/emergency services, performs clinical diagnostic and assessment functions, and develops and implements initial and revised Individual Service Plan.
They complete all appropriate case documentation and case planning information, completes all necessary reports per agency policy and protocol, completes appropriate documentation of collateral contacts, and participates in clinical supervision. Other duties include linking client with CPST resources; monitor client care and progress; collaborate with other providers when appropriate.
Additionally, Outpatient Children's Therapists attend required meetings, and provides other administrative functions as needed, while conforming to all relevant agency standards/regulations.

Requirements
Bachelor's degree in Counseling, SocialWork, Psychology or closely related field required, Master's degree preferred. Ohio licensure (such as LSW, LISW, LPC, LPCC) is required. Position requires a working knowledge of the DSM-5-TR, family dynamics, and of various psychotherapy modalities and techniques. Position requires an ability to work effectively as part of an interdisciplinary agency team, to communicate and relate effectively with individuals of all ages and families, understanding the Recovery model and working in this model with families, and to be sensitive and attentive to the cultural needs of each client and family. Position also requires good written communication skills, and strong organization skills.
