Position Overview:
Provide intake assessments, group therapy, and coordinate referrals to appropriate services as recommended. Provide individual and group therapy in a community-based outpatient clinic. Using CareLogic software, the Clinician will document all clinical services provided in accordance with DMHAS and SCSI expectations. The Clinician is expected to manage the caseload assigned, including all documentation, in a timely manner, and to attend team meetings and clinical supervisions.
Essential Function:
* Provides clinical services to persons enrolled in the outpatient program.
* Documents outpatient services rendered through the completion of progress notes that include description of the nature of the service, the persons response to the therapeutic intervention and its relation to the goals of treatments.
* Provides individual, group and family therapy to an assigned case load.
* Completes treatment plan reviews that include updates on client progress at least every ninety (90) days and discharge summaries whenever a case is closed.
* Documents all client contacts in the client's record.
* Completes all documentation i.e. referrals, treatment plans and reassessments and progress notes in accordance with the agency's Quality Assurance standards and in a timely manner.
* Provides appropriate linkage with community resources, e.g., meal centers social rehabilitation, case management and residential services.
* Demonstrates comprehensive understanding of all agency programs and program criteria.
* Documentation is legible and relates directly to recovery plan goals.
* Master electronic health record functionality.
* Manages case load, regular reviews of CareLogic System, performing utilization review independently.
* Participates in individual and group clinical supervision, multi-disciplinary team meetings, agency committees and program safety drills.
* Participates in community awareness presentations as a representative of SCSI.
* Adheres to all agency policies and procedures, including but not limited to confidentiality and the code of ethics.
* Attends and participates in training, both to maintain State of CT licensure, based on the needs of the agency and populations served.
* Always maintains confidentiality.
* Meets productivity requirements, as outlined by the agency.
* Will attend adequate clinical supervision by LCSW to achieve successful completion of licensure testing in two (2) years from start date- resulting in LCSW/LPC licensure.
* Performs all other duties as assigned.

The SocialWorker / Clinician provides direct clinical care to patients in the therapeutic process from admission to discharge, and provides individual, family, and group therapy, preparing discharge plans as indicated. The SocialWorker / Clinician will interface with internal and external stakeholders to ensure patient-centered, evidence-based, high-quality care for patients.
Duties/Responsibilities:
Carries a clinical caseload, conducts family therapeutic meetings utilizing biological, psychological, and systemic perspectives in understanding patients and their families to develop treatment plans and interventions from those perspectives appropriate to the ages of patients served.
Works cohesively with other team members and is a contributor to the team; actively communicating with the Director of SocialWork and other SocialWorkers, as needed. Ensures care provided is appropriate to the ages of the patients served.
Holds self-accountable to helping the patient and family achieve desired results, understanding and identifying professional strengths, limitations, and challenges.
Actively participates in clinical rounds & socialwork staff meetings/trainings.
Facilitates discharge planning and implementation.
Conducts group therapy sessions with techniques appropriate to the ages of the patients served.
Provides intern and/or staff supervision, as requested by the Director of SocialWork or Director of Clinical Operations.
Required Skills/Abilities:
Master's degree in SocialWork or Mental Health Counseling required.
LMSW/LCSW/LPC or LPC-A required.
Experience working with addiction and/or a psychiatric patient population required.
Must have excellent organizational, interpersonal, and computer skills, and be able to flourish in a fast-paced environment.
Education and Experience:
Master's degree in SocialWork, Mental Health Counseling or related field.
Physical Requirements:
Ability to move light objects weighing less than 20 pounds for short distances.
Ability to sit at a desk and walk around for different periods of time.
