Sign-on Bonus Available! The Social Work position for Mobile Health Services provides professional services to clients and their families utilizing the Mobile Health Clinic. This position will help to meet identified psychosocial, emotional, financial and environmental needs. The social worker provides psychosocial assessments, supportive counseling, emergent crisis intervention appropriate to setting, financial resource information, environment enhancements, advance directive planning and referrals to community agencies for clients and their caregivers/families in the mobile health clinic (MHC). Using an interdisciplinary team approach, the social worker ensures clear communication and helps to facilitate holistic care. The social worker identifies and implements interventions at the individual and systemic levels and provides expertise to high risk clients across the continuum. The social worker works collaboratively with the multi-disciplinary team to support the Mobile Health team performing at the highest level of their license in addition to maximizing the social worker's specialized training to address complex cases. Maintain a caseload of 3-6 housing clients and 10-15 outreach clients.
Provide regular supervision to youth housed in the emergency hotel and assist them in obtaining more stable housing.

ESSENTIAL FUNCTIONS: To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Provide assertive outreach and act as a liaison between students, parents, staff and community agencies.
Assess students to determine their need for school social work or other services and address the social and emotional needs of students based on assessment.
Provide crisis intervention to school age children and their families as appropriate and as requested by administrative personnel assigned to school.
Establish and maintain home/school ties for students and work with families concerning home situations that relate to school performance.
Participate in Special Education Services evaluations of students, including adaptive behavior assessments and social development studies.
Provide assistance to teachers or teams in planning to meet the needs of students.
Participate in efforts to remediate student truancy, delinquency and behavior problems.
Maintain records memorializing professional social work services rendered.
Coordinate student transitions within District programs, schools, services and communities.
Maintain communication with appropriate community agencies and aid in finding solutions to community-wide problems that affect students and families.
Evaluate and refer students and their families to appropriate resources within the school and community.
Adhere to laws and procedures involving child abuse/neglect and cooperate with community agencies to identify the need for protective services for children.
Identify and offer in-service training for school personnel and parents engage in group social work meetings.
Participate in professional growth activities including workshops, institutes, seminars and
research projects.
Participate in local, state, regional and national professional organizations related to social work profession.
Recommend lesson and curriculum modifications to meet needs of individual students and groups of students.
Perform any and all additional duties as assigned by the Board of Education, Superintendent of Schools, Building Principals and/or the Assistant Superintendent for Special Services.
QUALIFICATIONS:
EDUCATION
Minimum Master's Degree from an accredited college or university in social work.
EXPERIENCE
Two years or more of school-based experience preferred except in the case of a new Social Worker.
LICENSES/CREDENTIALS: Valid Illinois School Social Work Certificate

TERMS OF EMPLOYMENT:

Job Description
The Mental Health/Social Work Intern's role is to provide quality care and support to clients and families receiving Skill Sprout services as well as assist in projects and tasks aimed at growing Skill Sprout's innovative therapy programs. It is a part-time position and is volunteer-based. This position reports directly to an assigned Master's level or clinically licensed supervising clinician.
Specific responsibilities may include:
• Provide therapy services with Skill Sprout clients within the mental health department, which may include social skills treatment and/or individual, family and group counseling, and/or within the ABA (Applied Behavior Analysis) Program
• Conduct diagnostic clinical intake interviews with clients at the time of the initial service delivery, as needed
• Prepare treatment materials as directed by Consultants
• Assist with training new therapy staff
• Accurately and reliably collect data in therapy sessions as outlined by program procedure
• Provide clinical case coordination services and case management duties, as assigned
• Conduct diagnostic services, such as free screenings and/or mental health assessments, as assigned
• Attend all team meetings and trainings, and complete all necessary paperwork as directed by supervisor or administrative staff
• Commit to and follow a weekly schedule, documenting all services and activities
• Develop staff training resources, as assigned
• Develop and lead parent support and training groups
• Develop and lead sibling support groups
• Develop and lead Peer Model training programs
• Assist in oversight and management of the Peer Model Volunteer program in an assigned region
• Develop and oversee Peer Model recruitment and retention efforts
• Create training and/or educational material for the purpose of Blogs and Social Media networking for the department
• Create marketing materials and engage in marketing events for the department and company

Qualifications
The Mental Health Intern will have the following experience and attributes:
• Candidates will be actively pursuing a Master's degree in Social Work, Psychology, Sociology, Counseling or related field.
• Preferred prior experience working with children with disabilities; experience working with typically developing children will be considered.
• Strong writing skills
• Exceptional interpersonal communication skills
• Familiarity with technology programs including comfort with Microsoft Office
• Ability to quickly learn new technology programs
• Minimum of 3.0 GPA
• Ability to work nights and weekends as needed
