The 7N inpatient behavioral health unit has 30 beds with an average length of stay under 8 days. The Senior Psych Social Worker will assist patients and families to define social, emotional and environmental needs and to enable them to recognize/utilize available resources to resolve needs by providing a full spectrum of social work service. In addition, they will be responsible for the coordination of care, family therapy sessions, group therapy sessions, individual therapy sessions, safety planning, discharge planning, admission biopsychosocials, timely transmission of transition records, compliance with federal and state mandates for aftercare planning, treatment team, and support with involuntary commitments including court hearing.
Work Schedule:
Mondays to Fridays: 8:00am - 4:30pm
No weekends or holidays
Key Responsibilities:
Works with the patient, family, and other members of the health care team to safely and expeditiously return the patient to the appropriate post-acute setting.
Gathers and assesses information regarding the patient's physical needs, mental status, family support system, financial resources and available community and governmental resources.
Provides factual information based on current knowledge which may assist the patient/family in coping with the disease. Provides specific information on how to communicate with physicians and other Christiana Care staff so as to better utilize the system's resources.

Summary Statement
The incumbent in this position will be working with clients suffering from severe and persistent mental illness and substance abuse issues both on a 24-hour hotline, and in the community. Responsibilities include: individual counseling, crisis counseling and assessments, and monitoring client psychiatric symptoms. The incumbent will consult with an interdisciplinary team, the consumer, consumer partners, and external agencies as necessary to promote person-centered recovery goals. Applicants hired as a Crisis Intervention Specialist II will perform duties as a Credentialed Mental Health Screener. The incumbent is considered essential personnel and must report to work in a state of emergency, while working a rotating schedule.

POSITION OVERVIEW

Responsibilities will include conducting patient assessments, diagnosing conditions, ordering and interpreting tests, assisting with procedures, and managing treatment plans.

• Conduct comprehensive patient evaluations, including medical history, physical examinations, and diagnostic assessments.
• Diagnose and manage acute and chronic gastrointestinal conditions such as IBD, GERD, celiac disease, and functional bowel disorders.
• Order and interpret laboratory tests, imaging studies, and diagnostic procedures (e.g., stool tests, liver function tests, endoscopy results).
• Develop and implement treatment plans, including medication management, lifestyle modifications, and dietary recommendations.
