Summary/Objective:
The Hospice SocialWorker is responsible for the implementation of standards of care for medical socialwork services and for adherence to all conditions In the Service or Employment Agreement which include providing care based on the SocialWork Practice Act.

Work Environment/Physical RequirementsThe employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements. The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs.
Required Education and Experience• Graduate of a Bachelor's or Master's program in socialwork accredited by the Council on SocialWork Education. Meets personnel qualifications stated in §418.114(b)(3).• Minimum of one (1) years' experience in health care (hospice experience preferred)• Experience working with death and dying, individuals/families/caregivers.

This position formulates and implements individual and group interventions in the areas of reintegration planning and substance abuse treatment. The specialist will conduct evaluations using evidenced-based evaluation rating tools, will implement case and treatment plans and will delivery evidence-based substance abuse programming. The specialist will develop and implement therapeutic and behavioral interventions to improve a persons' in our care (PIOC) success in completing program requirements.
Salary Information
This position is in pay schedule and range 12-64 with a starting pay of $27.84/hour - $34.34/hour.
This position may receive a $1.00 to $2.00 per hour add-on pay based on the type of SAC credential possessed at the time of hire.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
To receive the bonus, a candidate must sign an agreement indicating they will remain in that position for a year. If the candidate leaves their position prior to completing the one year, they may be required to reimburse the initial payment.

Responsible for monitoring accounts receivable escrow accounts, generates disbursements from escrow accounts, and processes necessary paperwork for release of funds. Works in conjunction with Section 8 housing staff to coordinate housing assistance payments.
Responsible for informing prospective participants about the Section 8 Homeownership program, determines income eligibility, refers to mortgage lenders, and serves as the liaison between the Housing Authority and the internal staff (FSS & HCV) and various outside agencies and lending institutions.
May be responsible for meeting with participants for Section 8 briefings, individual counseling and corrective action planning, responding to and addressing resident/participant concerns and/or complaints.
Responsible for coordinating homebuyer education courses, financial counseling, and wellness sessions as applicable and with outside agencies.
Recommends changes and implements programmatic procedures and policies to ensure compliance with applicable federal, state and HUD regulations.
Assist and/or provides information for the preparation of all required financial reports for the responsible housing programs including escrow account disbursements and requisition of funds for training programs.
Responsible for communication and correspondence with residents, applicants, participants, and landlords based on HRA and program regulations and guidelines.
Responsible for submitting reports as required for internal and external reporting.
Assists with operationalizing grants as assigned.
Responsible for completeness of work related to housing program associated with intake eligibility, verification of information, entering data into the respective systems to ensure compliance with the Federal regulations.
Responsible for complete and accurate calculation of income, assets and property ownership verification and utilization of computers where applicable.
Responsible for the completion of assigned Housing Quality Standard inspections for the Housing Choice Voucher and Homeownership program and respective process follow-up.
Responsible for troubleshooting problems with program files.

Salary Information
This position is in pay schedule and range 12-64 with a starting pay of $27.84/hour - $34.34/hour.
This position may receive a $1.00 to $2.00 per hour add-on pay based on the type of SAC credential possessed at the time of hire.
Candidates with no prior permanent employment with the State of Wisconsin in the past 5 years may be eligible to accept a sign-on bonus of $3,000. The sign-on bonus will be paid in two installments: $1500 on the first paycheck and $1500 with the paycheck for the pay period in which the employee attains permanent status (usually 1 year). (UW System employment isn't disqualifying.)
