What You Will Do
Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties
Collaborates with another team member to perform psychosocial evaluations and assess mental health service needs of clients and their families through observation and interviewing
Creates and execute short-term service plans for clients in collaboration with an interdisciplinary team
Responds promptly to dispatch calls, traveling from the office to the clients' homes within two hours
Ensures seamless program operations as a vital liaison to community agencies and stepping in for the Program Coordinator as needed
Ensures uninterrupted comprehensive care from counseling and care management to long-term supplemental treatment referrals and follow-up
Prepares case histories and prepares and maintains case records, in accordance with the Program record-keeping mechanism
Encourages resistant clients to accept mental health services through interventions with clients and/or family members and friends concerned with the client's welfare

Qualifications
Current registration to practice as a Licensed Master Social Worker in New York State Required as determined by operational/regional needs
Valid driver's license may be required

What You Will Do
Applies mutually shared and lived experiences navigating the systems of care as a caregiver. Serves as a role model, advocate, and mentor for children and families

RESPONSIBILITIES
The Social Worker will conduct clinical crisis assessments as part of a Mobile Crisis team serving all of Suffolk County for adults, adolescents and children with the full range of DSM diagnoses, including both mental health and substance use disorders.
Screen and collect referral and collateral information from third party callers to inform assessment and linkage to immediate and long term supports.
The Social Worker will provide telephonic triage, assessment and de-escalation to determine the best disposition for individuals in crisis.
Utilize resources such as PSYCKES to review relevant clinical information.
Work closely with law enforcement to accept referrals and provide a co-response to individuals in crisis, when clinically necessary.
Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
The Social Worker will provide in-person/in-community de-escalation, clinical assessment and linkage to individuals in crisis.
Coordinate with outside providers and collateral contacts.
Work as part of a multidisciplinary team, conferencing cases, communicating outcomes and providing follow up.
Attend routine supervision/team meeting.

QUALIFICATIONS
Masters Degree in a relevant clinical field is required.
LMSW, LCSW, LMHC or limited permit is preferred.

About you
● You're a fully-licensed Clinical Social Worker at a Master's level or above with LCSW, LICSW, or LCSCW licensure (accepted on a state by state basis), a valid NPI number, and malpractice insurance.
