Operating in over 70 treatment centers in multiple states, our team members are engaged in medication-assisted treatment, counseling, support, and care management of individuals living with OUD. We treat the whole person and, in so doing, address the underlying causes of OUD in an effort to provide a continuum of care that not only addresses treatment needs but supports the patient on their journey to recovery.
Job Summary:
New Season operates a number of nationally recognized and accredited methadone treatment centers dedicated to helping individuals with prescription drug abuse and/or opiate addiction regain control of their lives.

Essential Functions:
Works with patients to complete all intakes, admissions, discharges, and transfer paperwork.
Documents patient progress through counseling and interaction through groups.
Completes patient psychosocial and an individualized treatment plan within the required time frame.
Identifies any clinical/case management needs and works to address those needs.
Performs individual, group, and family counseling as required.
Performs direct one-on-one patient contact through individual or group counseling.
Reports patient abuse, neglect and exploitation as required.
Reports patient grievances as required.
Educates patient in all aspects of treatment, corresponding health issues and steps to recovery.
Obtains Urine Drug Screens and initial patient photo identification.
Assists in monitoring all patient activities on center premises.
Actively participates in community relations activities as directed and authorized.
Ensures the reading and understanding of the Policy and Procedures Manual.
Ensures compliance with 42 CFR Part 2 (Federal Confidentiality Regulations) and 45 CFR, Parts 160 & 164 (HIPAA)
Is responsible for obtaining or maintaining proper licensure and/or certification according to specific state requirements.
Actively participates in preparation for surveys and inspections conducted by CARF, the State, DEA, Board of Pharmacy and any other agency site visits as dictated by the state laws or regulations.
Participates in all staff meetings
Ensures compliance with local, State, Federal and Colonial Management Group, LP rules, regulations and policies.
Always acts in the best interests of the program and company; honors, supports, and protects the proprietary data and rights of the company.
Performs other tasks as assigned by clinic, region or corporate leadership.
Minimum Qualifications:
Education/Licensure/Certification:
Experience in substance abuse field is not required, but preferred

Summary of Responsibilities
A Behavioral Health Practitioner (BHP) works under the direction and supervision of a Clinical Manager, providing community-based behavioral health services to individuals with psychiatric illness, substance use issues and/or co-occurring disorders. The BHP must be open and willing to work with culturally and gender diverse groups and individuals to facilitate goals of self-efficacy. This position requires a high degree of self-initiative and motivation. The schedule is flexible, as determined by the person served' s needs and may include early morning, evening or weekend appointments. The position may involve services provided in the designated BHS clinic, driving to persons served homes or various locations within the community where services are provided (if services are offered on-site according to the program and funding source.) It may include telehealth services utilizing both audio and visual communication with individuals. BHPs provide individual, family and group counseling, crisis intervention, conflict resolution, initial assessments to determine level of care, treatment planning, case presentations, presenting psychoeducational information to individuals regarding diagnosis, symptoms, medications, relapse prevention and safety planning. The BHP provides services to both children and adults. The position also includes participating in multidisciplinary case conferences and supervision, treatment plan reviews, developing aftercare treatment and all required documentation utilizing Collaborative Documentation.
Essential Duties and Responsibilities
•Primary focus is on providing comprehensive clinical services and therapy which includes biopsychosocial assessments, screening and placement for level of care, diagnosis, individual psychotherapy, group therapy, family therapy, brief interventions utilizing evidence-based care and tools and educational counseling services to a caseload of person's served consisting of adults and children.

Purpose & Impact:
The purpose of the Child Welfare Case Manager, in partnership with various individuals and groups within the child welfare system and community, is to ensure and promote the social, physical, psychological and emotional well-being and safety of the children and families they serve.
The Case Manager maintains case ownership responsibility as the integrator of all services and supports identified for each child, including therapy, mental health services, health and dentistry, developmental services, educational support, permanency and safety.
The Case Manager manages cases toward goals recorded in case plans while adhering to Florida Statutes, Administrative Code, written policies and orders of the Dependency Court. The Case Manager reports to the Case Management Supervisor.
Essential Functions:
Accept all cases assigned by unit supervisor and meet with children in their primary residence within 2 working days of ITR staffing and with identified parents in sufficient time to complete a Family Assessment within 15 working days of ITR staffing.
Negotiate and develop a case plan based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.
Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.
Accurately document all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.
Submit service requests to the lead agency Utilization Management Department, and provide clients with timely referrals to services. Develop and maintain knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintain regular contact with service providers and document service progress in FSFN.
Complete and submit court documentation within in required time frames. Prepare for, attend, and participate in all court activities as necessary.
Arrange for, attend, and participate in individual case staffings as necessary. Complete all required staffing and application packets.
Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary.
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.
Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.
Conduct home studies as required for prospective placements. Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective care-givers as needed. Plan and facilitate parental and sibling visits as needed and appropriate.
Transport children as needed.
Ensure that all Independent Living functions are completed as required
Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.
Function as agency on-call Case Manager as scheduled.
Organize, prioritize and complete all work assignments by the established deadlines.

Social services members at the PD-15 Judicial Circuit assist attorneys and clients in many ways, which varies based on the client's needs, availability of community services, the number of face-to-face meetings with each client, and the offense.
PD-15's unpaid/volunteer social work interns are typically local university MSW students, who work with the office for the academic year in order to establish and maintain a relationship with clients. MSW may receive credit for their participation. To expose first-year MSW student interns to the field of social work, interns learn case management, interviewing, and assessment skills through shadowing one of PD-15's mental health social workers. Interns also work with juvenile probation cases to motivate offenders to complete the requirements of probation and ideally achieve early termination of their probation early.
We accept two, first-year MSW students throughout the year.

Job Functions: The Child Advocate supports the Community First Hug Program and will assist in taking responsibility for the assessment, identification and referral of developmental needs of children birth middle school participating in the program. The position also focuses on the parents of these young children and providing education and support to ensure developmental and educational success. In addition, the Child Advocate will assist in the administering of the Parent Child Plus Program (PC+)
Essential Responsibilities:
Implement the childrens developmental assessment process using ASQ-3 and other validated developmental screening tools to develop a plan for each child.
Facilitate completion of ASQ-3 at every necessary interval including follow up, sharing results with caregivers and entering data.
Make appropriate referrals for children with questionable or delayed scores and assist with follow through to ensure service needs are met.
Collect and report developmental screening data.
Meet regularly with the parents of children 0-middle school age children, providing education and resources, and introducing opportunities for parental involvement.
Coordinate needed medical services and further developmental evaluations as needed.
Coordinate all services with families primary Family Advocate.
Participate in regular meetings to develop plans for families and monitor progress.
Keep confidential records according to the Ethical Boundaries for Those That Serve. Maintain professional boundaries.
Demonstrate flexibility in work schedule to ensure that client/program needs are being met.
Lead Parenting Classes such as SIDs, Circle of Security and Emotion Coaching.
Support the Education team, Director of Education and PCH+ program as needed.
Requirements:
Education and Experience:
A minimum of a B.A. or B.S. degree in an education, health or social services related field. Minimum of three years of experience in the fields of social services or education. Direct experience working with children and parents required. Home visitation experience. Bilingual required (English & Spanish).
Skill Requirements:
Must be able to work effectively with children and parents, representing and communicating their status and progress to small groups and professionals within a network comprised of the Hillsborough County School District, Metropolitan Ministries service providers, and applicable and related medical social service providers in the surrounding community. Must be able to work within a multi-disciplinary team framework; strong interpersonal and supervisory skills; strong oral and written communications skills; dependability; demonstrates maturity and sensitivity to cultural and individual differences in clients served; ability to handle job related matters in a professional, diplomatic, and confidential manner; willing to work with minimal supervision, yet be able to accept direction; well organized and detail oriented. Knowledge of community resources available to homeless population. Must demonstrate maturity and sensitivity to issues of faith, culture, and other sources of diversity. The Social Worker (SW) is a member of the care treatment team including the PCP, other Medical Specialists, LCSW/BHS, and Case Managers. The incumbent in this role is responsible for providing psychosocial assessment, social case work and linkage to community resources for patients who have chronic, life threatening or altering diseases and disorders. He/She advocates for services and resources for the underprivileged and victims of abuse, neglect, or other difficult personal situations to help them maintain an optimum level of health. Social Workers will adhere to strict departmental goals/objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures as defined by industry standards and the enterprise.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Needs identification and assessment:
Conducts timely and appropriate assessment and needs identification, prioritizing patients on the PCP's High Priority Patients (HPP) and Top 40 patient lists. Assesses the patients for psychosocial, financial, family issues, palliative care/end of life issues, home safety, etc. that contributed to hospitalization.
Dialogues with PCPs in order to support and advise concerning social needs and resources available.
Medicaid and other benefit eligibility assessment:
Conducts appropriate assessment of needs and financial benefit eligibility.
Assesses patients for Medicaid criteria and assists with application process as needed.
Assists patients to obtain community resources/services as appropriate, e.g. meals, medications, housing, daycare, DME, HHA. etc.
Resource coordination and intervention:
Serves as care coordinator, linking patients with internal and external resources, prioritizing patients on the PCP's HPP and Top 40 patient lists.
Facilitates connections to community resources as identified by the patient's SDoH Wellness Screening.
Works with patient, family, and case manager to facilitate applications for higher level of care.
Works to provide self-management support and ongoing phone contact with patients.
Maintains an accurate repository of social wellness tools for the care team's awareness and utilization.
Communication.
Maintains communication with other healthcare team members by attending appropriate meetings (i.e. weekly Super Huddles and Transitional Care Team meeting.).
Provides consultation in an integrated health care environment regarding social determinants of health and community resources.
Timely and accurate documentation:
Maintains timely, accurate, thorough and appropriate documentation/reports per company policies and procedures. Initial psychosocial assessments will be completed withing 48 hours. All follow up visits phone calls and collaborative contacts will be documented within 24 hours. Assures documentation meets billing guidelines.
Additional duties and responsibilities may include:
Works closely with the transitional care team to secure the appropriate level of care post hospital/SNF discharge. Further interventions may be conducted in the center, by phone call or patient's home.
Performs other duties as assigned and modified at manager's discretion.
