Purpose Statement:

ESSENTIAL FUNCTIONS:
Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.
Through patient observation and patient/family interview obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.

Promote recovery, symptom reduction and the achievement of the highest level of social functioning.

Manage a caseload and assume responsibility for patient chart documentation.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
* Master's degree in SocialWork from an accredited college or university or a new graduate of accredited master's degree socialwork program with successful completion of socialwork field education, internship or similar practicum through master's degree program required.
* Two or more years' experience working with patient population preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
State licensure, as required by the state in which the facility operates; which could include: Licensed Master SocialWorker (LMSW), Licensed Clinical SocialWorker (LCSW), Licensed Practical Counselor (LPC)
CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
First aid may be required based on state or facility.

The Role
The position is responsible for the general and day-to-day operations of a Head Start/Early Head Start center or cluster of outlying centers. You will be responsible for providing coverage to multiple classrooms, assuring regulatory and legal compliance with federal, state, and local regulations, communicating with parents to learn about and use community resources, working with parents on goal setting; encouraging parent involvement and participation in their child's education. You will also assist families as they identify and meet their own goals through case management model developing and carrying out program's Family Partnership Agreements, assisting with the completion of the child health requirements and other required screenings and assessments.

Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:
The Family Intervention Specialist:
Carries a caseload of 4 to 6 families
Holds individual and family sessions with each family 3 times a week, scheduled at the convenience of the families
Conducts on-going assessment of youth to determine their needs from a strength-focused, solution-based perspective
Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills
Drives up to 60 to 80 miles to meet with families in their homes and communities (additional travel may be required)

Provides on-call crisis support to the youth and family (schedules vary by location)

Job Summary
The Social Services Assistant provides supportive services to socialworkers, case managers, and clients. This role assists with administrative tasks, outreach efforts, and client referrals to ensure efficient and effective service delivery.
Primary Responsibilities
Assist in client intake and service referrals. Provide basic counseling and resource navigation. Maintain accurate records and case documentation. Support community outreach and public education initiatives. Coordinate scheduling and follow-ups for client services.
