Key Responsibilities:
Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
Develop and implement evidence-based treatment plans tailored to the needs of each patient.
Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).

Requirements:
Active and unrestricted license to practice as a Licensed Clinical SocialWorker in State applying for.

Ability to provide in-person services at assigned facilities, up to 45 minutes.

Juntos provides important services to unaccompanied minor children who immigrated into the United States. Services include; temporary shelter and support with family unification and home study and post release services. Family Reunification is the Juntos programs primary goal. The Overnight Residential Counselor will be responsible for assisting youth with their daily life skills, education, and chores in the program and providing a direct level of support and supervision. This is an overnight position, shift is 12am - 8am, may work other shifts as needed. Bilingual/Multilingual preferred but not required.
Duties and Responsibilities including but not limited to;
Ensures that program structure is consistent with stated goals and objectives and that a Nurtured Heart, trauma-informed program is followed.
Reports to and serves on the treatment team with Therapists and other designated staff.
Provides life skills education.
Coordinates recreation and physical education activities scheduled during their shift.
Ensures that daily living needs of youth are met through delegation of responsibilities to residential counseling staff.
Coordinates appointments with medical consultants, ensures all over-the-counter and prescription supplies and medication is on hand and correctly administered.
Coordinates other appointments (i.e. court appearances) with Therapists and Periodic Transport staff.

Qualifications
High School Diploma or Equivalent with a minimum of 1 year experience in the child welfare field, working with children and/or adolescents in a social service setting
1 - 3 years experience working with the program population or relevant direct service delivery in a residential setting,

How you will make an impact
Completes psychosocial assessments, MDS, and other required documentation in accordance with department policy, State, and Federal regulations.

Ennoble Care is seeking a Full-time SocialWorker Care Manager (Hybrid) to service our patients within and around the Clinton, NJ area. This role is well-suited for early-career socialworkers looking to build experience in integrated behavioral health within a collaborative, team-based environment.
This position will be primarily remote with occasional travel to patients homes/facilities.
Responsibilities:
Support the mental and physical health care of patients on an assigned patient caseload.
Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
Provide patient education about common mental health and substance abuse disorders and the available treatment options.
Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
Support psychotropic medication management by consulting with the behavioral health consultant and then communicating with the primary care provider assigned.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider. Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, or in person.
Track patient follow up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients. Registry functions can be accomplished through an EHR build, on a spreadsheet used in conjunction with an EHR, or can be built into a stand-alone clinical management tracking system that may or may not be linked to an EHR.
Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Be comfortable utilizing Excel and Word and completing assessments and productivity trackers daily.
Time-management skills are a must and the BHCM needs to be able to manage their day effectively through calendars and the EMR system.
This role is PRIMARILY telephonic, however in person visits are done and the BHCM has to be comfortable completing them both ways.
Qualifications:
The SocialWorker Care Manager will have a minimum of MSW
Preferred Licensure or provisionally licensed
Behavioral Health experience and knowledge
Must be licensed driver with an insured automobile in good working order
Ability to communicate effectively, both verbally and in writing.

ABOUT THE OPPORTUNITY:
We are seeking a dedicated and compassionate School SocialWorker to provide counseling and social-emotional support aligned with student IEPs, as well as in-class support to both students and teachers across multiple schools. The ideal candidate will be experienced in working with students with special needs and familiar with the DDD population and regulatory requirements.
RESPONSIBILITIES:
Provide individual and group counseling to students based on their IEP needs.
Support teachers and students directly in classrooms across elementary and high school settings.
Participate in IEP meetings and provide professional input on counseling-related goals.
Train and guide classroom staff on techniques to support student behavior and social-emotional growth.
Assist in student intakes to support smooth school transitions.
Collaborate with therapists and behavior teams in implementing behavior plans.
Provide crisis response counseling as needed.
Maintain accurate documentation of student progress and counseling sessions.
Serve as a liaison with families and community agencies (e.g., DCF, DDD, PerformCare).
Conduct transition workshops for parents (e.g., SSI application, guardianship, DDD resources).
Develop and facilitate social skills groups.
Coordinate and host family engagement events and PTTA meetings.
Maintain regular communication with school leadership and attend all required meetings, trainings, and conferences.
Support school-wide initiatives and fulfill additional responsibilities as assigned by school administrators.
Strong knowledge of counseling techniques for individual, group, and family sessions.
Understanding of IDEA, ADA, and other relevant education laws.
Familiarity with child development, family systems, and culturally responsive practices.
Experience with NJ DDD, PerformCare, and DCF strongly preferred.
Effective verbal and written communication skills.
Ability to maintain confidentiality and work in a collaborative educational setting.
Excellent organizational and case management skills.
QUALIFICATIONS:
Master's Degree in SocialWork, Public Administration, Health Administration, or a related field.
At least 3 years of experience in a school, rehabilitation, or health-related setting (1 year in a supervisory capacity preferred).
Current NJ DOE School SocialWorker Certification required.
Must pass fingerprint and background check as per NJ State requirements.
Reliable transportation to travel between school locations.

thereafter) to provide a forum for ongoing discussions between resident/family
and treatment team with a primary focus of discharge planning;
· Collects pertinent social data upon admission of each resident and places
it in the medical record, including information about the personal and family
problems related to the resident's illness and care, support network, actions
taken to meet the resident's individual needs and eventual discharge to an
appropriate level of care based on functional capacity. Pertinent social data
shall be made available to the attending physician and other appropriate staff
members;
· Acts as a liaison between residents, families, outside agencies, and the
facility Administrator to ensure that the resident's rights are maintained;
· Functions in a manner that adheres to all policies/procedures of the
facility, as a representative of the Windsor Health Care, LLC's Center
administration;
· Documents and prepares any reports requested by the administration; also
councils and provides assistance when change in financial status occurs for
residents in the Windsor Health Care, LLC's Center;
· Coordinates discharge planning and assists with the discharge planning and coordination of community
resources.
· Concerns his/herself with the safety of all facility residents in order
to minimize the potential for fire and accidents. Also, ensures that the facility adheres to

ABOUT THE OPPORTUNITY:
We are seeking a dedicated and compassionate School SocialWorker to provide counseling and social-emotional support aligned with student IEPs, as well as in-class support to both students and teachers across multiple schools. The ideal candidate will be experienced in working with students with special needs and familiar with the DDD population and regulatory requirements.
RESPONSIBILITIES:
Provide individual and group counseling to students based on their IEP needs.
Support teachers and students directly in classrooms across elementary and high school settings.
Participate in IEP meetings and provide professional input on counseling-related goals.
Train and guide classroom staff on techniques to support student behavior and social-emotional growth.
Assist in student intakes to support smooth school transitions.
Collaborate with therapists and behavior teams in implementing behavior plans.
Provide crisis response counseling as needed.
Maintain accurate documentation of student progress and counseling sessions.
Serve as a liaison with families and community agencies (e.g., DCF, DDD, PerformCare).
Conduct transition workshops for parents (e.g., SSI application, guardianship, DDD resources).
Develop and facilitate social skills groups.
Coordinate and host family engagement events and PTTA meetings.
Maintain regular communication with school leadership and attend all required meetings, trainings, and conferences.
Support school-wide initiatives and fulfill additional responsibilities as assigned by school administrators.
Strong knowledge of counseling techniques for individual, group, and family sessions.
Understanding of IDEA, ADA, and other relevant education laws.
Familiarity with child development, family systems, and culturally responsive practices.
Experience with NJ DDD, PerformCare, and DCF strongly preferred.
Effective verbal and written communication skills.
Ability to maintain confidentiality and work in a collaborative educational setting.
Excellent organizational and case management skills.
QUALIFICATIONS:
Master's Degree in SocialWork, Public Administration, Health Administration, or a related field.
At least 3 years of experience in a school, rehabilitation, or health-related setting (1 year in a supervisory capacity preferred).
Current NJ DOE School SocialWorker Certification required.
Must pass fingerprint and background check as per NJ State requirements.
Reliable transportation to travel between school locations.
