Responsibilities: The Senior Therapist is responsible for assisting the Director of Rehab in the efficient Clinical management of rehabilitation services in their assigned account(s). Also, this position assists in integration of rehabilitation services in all settings. The Senior Therapist assesses the need for, develops, and delivers therapy programs to facilitate rehabilitation. The Senior Therapist identifies the need for and makes referral(s) to other disciplines to address the comprehensive needs of the patients. They implement individualized therapy programs designed to restore, reinforce, and enhance programs. In addition, the Senior Therapist instructs, educates, and trains patients and caregivers in those skills and functions essential for promoting independence and productivity, in order to diminish or correct pathology. They direct activities to promote and maintain health. The Senior Therapist supports the delivery of the highest standard and quality of rehabilitation services.
1. Assists therapy staff in effective and efficient delivery of rehab services; adherence to an established plan of care, department and patient(s)' schedule, and timely documentation of services.
2. Assists in clinically managing Powerback Rehabilitation personnel and consultants within facility or home care contracts.
3. Continues direct patient care.
4. Supports the delivery of the highest standard of rehabilitation services through appropriate utilization of resources, the promotion of clinical programs, and staff development in collaboration with licensed therapists, the Clinical Operations Area Director, and Regional Clinical Director.
5. Assists DOR with customer service.
6. Ensures that the clinical management of the rehabilitation department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification is overseen by a licensed therapist.
7. Assists DOR in ensuring practice act compliance.
8. Attends facility meetings and is responsible for information sharing at facility meetings at the DOR's direction.
9. Recognized as "go to person" from a clinical standpoint in the gym.
10. Assists in clinical development and growth of therapy staff in their own or other disciplines.
11. Assists in identification of areas of opportunity for clinical growth for the discipline or department in collaboration with the Director of Rehab and Regional Clinical Director.
12. Assists the Director of Rehab developing clinical excellence to support customer service.
13. Assists Director of Rehab with promoting good team work, company culture, and diversity within the rehab gym.
14. Assists in developing, coordination and utilization of student and mentor programs.
15. Assists the Director of Rehab with efficient scheduling of the rehab gym.
16. As necessary or applicable, supervision of Physical Therapist Assistants or Occupational Therapy Assistants, temporary licenses, students and support personnel in accordance with state licensure and professional standards.
17. Attends and contributes to patient care, staffing conferences and other related meetings.
18. Makes referrals to other disciplines, agencies in order to address the comprehensive needs of the patient.
19. Consults and collaborates with other members of the health care team to select the most appropriate therapeutic regimen consistent with the needs and capabilities of each patient.
20. Adapts program and/or plan of care according to the needs of the individual patient.
21. Conducts training programs and participates in training medical, nursing and other personnel in treatment techniques and objectives, consistent with the patient's course of treatment or those in common to the site of service.
22. Performs other related duties as required. Qualifications: 1. They must have a Bachelor's degree in an occupational therapy curriculum accredited by ACOTE standards.
2. They must have initial registration from the National Board for Certification in Occupational Therapy.
3. The Occupational Therapist must hold a valid and unencumbered occupational therapy license as required in the state of practice.
4. A thorough knowledge of Medicare and third party billing is required.

This position is responsible for the direct provision of emergency, counseling, and intensive case management/coordination services as part of the Region 4 Crisis Response & Stabilization Team (CReST). Work is performed under the limited supervision of the CReST Supervisor and/or CReST Program Manager.
Crisis Response & Stabilization Team (CReST) is a resource for the residents of Region 4 who are in a mental health crisis and need help. With quick response and with assistance connecting to ongoing services, CReST hopes to reduce the cycles of crises and prevent the need for acute psychiatric hospitalizations.
CReST provides a therapeutic response to crisis and is staffed with LMHP-E/LMHP Clinicians providing short term crisis counseling. CReST serves across the lifespan and regardless of insurance. New clinicians will have the opportunity to specialize in adult, children and/or cross-population teams.
Hybrid telework and flexible schedules, with a primiary site location at one of the Region 4 CSB's.
Overnight positions available: 12 hour shifts, 8a-8p, rotating to include varied weekends/days.
Essential Functions
Directly provides emergency supports and short-term crisis counseling services to adults, children, adolescents, and families presenting with significant emotional and behavioral needs, and in psychiatric distress/crisis, as assigned by the CReST Supervisor and according to the program model.
Determines immediate, short, and long-term service needs of the individual and/or family; conducts thorough evaluation and prepares comprehensive assessment.
Develops and implements crisis response plans for individuals and families served through the CReST program.
Refers and ensures linkage of individual and family to other needed services and community resources.
Coordinates individual care with family and other applicable service providers.
Conducts home visits, and school and other community site visits as required and/or permitted.
Monitors, documents, and reports on individual and family treatment progress.
Prepares, documents in, and fully maintains the electronic health records (EHR) of assigned CReST cases.
Collects and enters required individual consumer data and submits reports on individual consumer progress.
Positively communicates and maintains good working relationships with community partners.

Education and Experience

Responsibilities:
Conducts ongoing assessments to meet the needs of patients served.
Communicates between supervisors, direct care staff members, family members, guardians, and collateral individuals regarding an individual's progress.
Provides a monthly schedule to supervisors of appointments, home visits, and observations to monitor work advancement.
Coordinates home, work and/or community-based visits to meet the requirements of observations, chart reviews, staff interviews, and data analysis.
Ensures the Behavior Intervention plan is implemented effectively and properly.
Participates in Interdisciplinary Team Meetings and other pertinent meetings as required.
Completes monthly and/or quarterly progress notes.
Organizes training to certify staff and other team members are competent in the implementation of the Behavior Intervention Plan and/or documentation sheets.
Provides follow-up interviews, home visits, and observations for continued support of an individual's needs.
Maintains strong clinical records to guarantee information is readily available to interested parties.
Acts as liaison with the public and mental health community to support development of ongoing goals.
Assists in creating procedures for training materials to improve programs for individuals served.
Complete continued education on behaviorally based interventions & techniques to stay competitive in the field.
Qualifications:
Must have experience with behavioral intervention and/or strong educational emphasis on learning and developmental psychology.
Two or more years of experience working with individuals with developmental disabilities or behaviorally challenged population.
Must have reliable transportation.
Ability to perform physical tasks, including standing for extended periods, bending, move from seated to standing regularly.
State Specific Requirements:
Illinois: Board Certified Behavior Analysts (BCBA), Licensed Clinical Psychologist, Board Certified Associate Behavior Analyst (BCaBA), or a minimum of a bachelor's degree with 1500 hours of supervised experience performed by a Licensed Psychologist or BCBA.
Indiana: A master's degree in a human related field (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)
Kentucky: A master's degree in a human related field and 2 years of experience (this includes but is not limited to BCBA, Licensed Psychologist, Licensed Counselor)
Georgia: Master's degree in psychology, special education, counseling, social work, or related field OR BCBA, Licensed Professional Counselor, Licensed Clinical Social Worker, or Licensed Psychologist/Psychiatrist. All applicants MUST have at least 2 years of experience providing positive behavioral supports (writing BSP, functional assessment; plan development, training, and implementation) to people with intellectual and/or developmental disabilities. Must have documented coursework in Applied Behavior Analysis reflected on resume or transcript.

As a Primary Care Licensed Clinical Social Worker, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider. Core Responsibilities
Interview individual members to assess social and emotional capabilities.
Provides case management to Veterans and families as needed throughout the continuum of care.
Conducts group therapy (which may include PTSD groups; weight management; anger management & coping skills, grieving; and personal interactions).
Helps members through group experience to develop attitudes and social skills for improved family relations and community responsibility.
Addresses issues such as advance directives and organ donation in accordance with acceptable standards.
Inputs all patients' information into VISTA/CPRS.
Provides patient and family health education with a focus on self-management, prevention, wellness, related health issues and healthy lifestyle choices.
Maintains confidentiality of veterans' data and information.
Complies with all VA and company training requirements.
Fulfill compliance requirements of the Office of Inspector General (OIG), Joint Commission (JC), Environment of Care (EOC) oversight, lab compliance and other related items.

Requirements:
Qualifications
Master's degree, or higher, in Social Work from a program registered by the Department of Education is required, with specific training dealing with adult patients.
Must comply with and maintain all requirements for a valid, unrestricted LCSW/MSW (or equivalent) license in the state of desired employment, or in any U.S. state or territory, depending on VA contractual requirements.
Minimum of 1-3 years' post-master direct counseling experience with adult patients. Specific requirements could vary based on individual VA contract.
Proven mastery of advanced sociological, psychological, and mental health care related interventions and issues.
Must be credentialed and remain in good standing through the Veterans Health Administration (VA).
Current certification in Basic Life Support (must be renewed periodically as specified by the certifying agency - AHA valid for two years, e.g.) and in ACLS as specified by individual VAMC contract
Proficiency in written and spoken English.
Strong computer skills, including EMR experience
