POSITION SUMMARY:
The Community Mental Health Worker (CMHW) provides mental health service to residents of Prairie Mountain Health, and may be based in both community and facility settings. As a member of the assigned interdisciplinary team, the incumbent provides a broad range of mental health services to clients and their families, and acts as the primary counsellor and/or case manager for all newly referred clients as well as those with long-term service needs. In collaboration and partnership with other health care providers, clients' families and community stakeholders, the CMHW focuses on supporting individuals in their personal recovery where they live, learn, work and socialize.
The CMHW requires a diverse and comprehensive knowledge base about trauma informed psychiatric care as well as other adopted mental health therapies and the ability possess and integrate into practice crisis management and suicide intervention skills. Through the application of therapeutic skills that promote and build on client's strengths, the CMHW assists the client/family to maximize independence, self-responsibility and promotes personal wellness, community integration and social justice. Depending on client needs and particularly when working with individuals with enduring mental illness, the CMHW applies case management functions that focus on functional assessment and skill development, relapse prevention, and rehabilitation.
In addition to being a valued member of the interdisciplinary team, the CMHW conducts a sound professional, autonomous practice, demonstrating effective problem-solving and organizational abilities. The CMHW has a working knowledge of the Mental Health Act and other legislation that protect their client(s) and the public.
RESPONSIBILITIES:

* Engages with clients and families to build trusting therapeutic relationships as well as developing case formulation, intervention plans and services.
* Collects and reviews initial clinical and psychosocial information, gathers and incorporates collateral information into initial screening and assessment.

* Provides assessment information, diagnostic impressions and treatment recommendations to other health service providers following PHIA and FIPPA requirements.
* Assists the client to attain and maintain optimal mental and physical health.
* Engages family as important contributors to care planning as defined and chosen by the client. Provides emotional support and education to family members within the following of privacy legislation.
* Ensures that the individual or community receives appropriate and effective service, including crisis planning and intervention for safety and protection.
* Collaborates with family members, other agencies, organizations and individuals from hospital and community to facilitate a well-coordinated, recovery-oriented service system for clients with chosen follow up systems.
* Provides follow-up services and monitoring of mental health status as required.
* Provides individual, group and public education to promote awareness of mental health problems, mental illness, mental wellness, and resiliency.

POSITION REQUIREMENTS/ QUALIFICATIONS
Must have a minimum of a Master's Degree in Human Services or similar disciplines such as
Sociology, Psychology, Criminology, SocialWork, Behavioral Health, etc.
Must have one of the following valid and active License in Mental Health Counseling, Licensed Clinical SocialWorker, Licensed Marriage and Family Therapist in the state of Florida
Computer proficient in EMR and MS Office programs
Bilingual (Spanish/English) a plus but not required
Previous experience in mental health is a plus!
Must be at least 21 or older, have a valid driver's license, and have reliable personal transportation.
POSITION RESPONSIBILITIES:
Conduct thorough assessments of patients and develop service plans, treatment plans, and discharge plans.
Provide individual, family, and group therapy sessions with adults, children, and families in an outpatient, acute care inpatient, or community setting.
Accurate and timely documentation of clinical and administrative services following program guidelines and external standards (Joint Commission, Medicaid, etc.).
Meets productivity requirements for the designated program through efficient workflow management.
Maintains cooperative relationships with other internal departments and external agencies and keeps supervisor informed at all times of all relevant client, program, and community issues
Understands the basic tenants of Co-Occurring disorders and the relationship between mental health and substance abuse.
The therapist will also demonstrate knowledge of the age-specific needs of the clients treated in the program.
The therapist will be responsible for maintaining patient care following program guidelines and external standards (Joint Commission, Medicaid, etc.) to various populations inclusive of individuals, families, and group therapy to children, adolescents, and adults.

QUALIFICATIONS:
Education & Experience:

Under the direct supervision of the Manager, Intake and Customer Relations, the Intake Counselor is the first point of contact for families, advocates and other early learning community members requesting information on ELCHC programs and referrals, with a specialized focus on childcare resources and referrals. This position will assist families with Voluntary Prekindergarten (VPK) applications and VPK child transfers. This position will also assist with School Readiness Provider transfers, as well as, providing CCR&R listing requests from families. The Intake Counselor position will be responsible for offering funding notification from the School Readiness waitlist and determining preliminary eligibility for the School Readiness Program. The primary mode for carrying out this work will be through the Customer Relations Contact Center (CRC).
Essential Duties and Responsibilities:
Conduct client interviews by phone to assess customer's needs and to identify available community resources
Screen potential clients for Voluntary Pre-Kindergarten and School Readiness eligibility for federal, state and local funding by comparing customer information to program requirements
Approve VPK applications and VPK re-enrollment
Approve School Readiness provider transfer requests
Resolve customer inquiries via phone and email in a courteous, timely, professional manner
Capable of resolving issues and concerns by providing solutions to customers
Identify and escalate priority issues to the appropriate leader
Input and review client information in the Statewide Data System
Document all call information according to standard operating procedures
Maintain exceptional customer service skills at all times, including high call volume periods
