GENERAL DUTIES / RESPONSIBILITIES:
1. Conducts telephonic outreach to assigned members to assess health, environment, nutrition, and psycho-social areas of concerns using a variety of assessments.
a. In response to assessments, coaches and problem solves with member to identify and address specific goal(s) to support health and behavior change.
b. Provides appropriate interventions to optimize health and well-being. Interventions may include education, the coordination of community-based support services, and other resources.
c. Charts member's treatments and progress in accordance with state regulations and department procedures.
d. Makes referrals to case manager, as appropriate, and/or refers member's family to community support services and resources.
2. Provides home assessment to high-risk members and develop an individual care plan
3. Collaborates with physicians in screening and evaluating members for psychotropic medications.
4. To better serve members and implement the model of care, understands the clinical program design, program monitoring and reporting.
5. Practices as an interdependent member of the health team and provides important components of primary health care through direct socialwork services, consultation, collaboration, referral, teaching, and advocacy.
6. Assess' and treats outpatients in individual and family modalities exercising mature professional judgment and using a wide range of socialwork skills to include individual and family counseling to assist patients and their families in dealing with chronic and acute diseases/injuries.
7. Conducts psychosocial assessments to determine patient needs and resources (both family support and community support). Provides counseling to patient and family in matters directly related to patients' limitation, adjustment to medical condition, and ongoing treatment. Develops and implements discharge plans, follow-up care, and transfers to other health care facilities (e.g., nursing homes, rehabilitation hospitals, etc.)
8. Provides consultation services to medical, nursing, and ancillary hospital staff regarding psychosocial issues, discharge plans, and follow-up care for patients and families.
9. Provides crisis intervention services.
10. Responds independently, and with various media, to appropriate community requests. Take the initiative in seeking out opportunities to present programs to meet the needs of patients/members and their families.
11. Consults with Hospital administration, and Plan supplying information and feedback regarding procedures and services provided by the Psychology Division.
12. Develops and maintains working relationships with community resources. Coordinate with physicians, and representatives of their service disciplines for the benefit of the member and their families. Take initiative in identifying and assessing the needs of the community and organize responses to address those needs.
13. Interfaces with the RN Case Manager(s) and the Interdisciplinary Team (IDT) in the development and implementation of the Case Management Program (CMP).
14. Integrates socialwork case management and nurse case management as a team.

Group dynamics can be difficult at best when critical decisions need to be made. A Group Meeting Facilitator interacts with groups that need coaching to work productively as a team to reach a common goal. The group meeting facilitator guides discussions of the topics laid out in group meetings and/or project/taskforce initiatives agenda, with the purpose of moving the team towards a common goal.
A skilled Group Meeting Facilitator fosters collaboration and teamwork and manages conflict among team members. The ProSidian Consultant | Group Meeting Facilitator will coordinate, lead, and support all phases of team meetings to ensure progression and completion of work as well as supports the Global Project Lead (GPL) And Project Team, and is accountable for the quality, timeliness, and effectiveness of group meetings and/or project/taskforce initiatives and ends with delivery of the final outputs that achieve stated mission objectives.
The job of a “Group Meeting Facilitator” is more of the "conductor" than a participant throughout the Program Period. The ProSidian Group Meeting Facilitator shall serve as a consultant to clients across private companies, fortune 1,000 enterprises, and government agencies of all sizes. You direct the flow and energy throughout the program period. You observe participants' body language, listening carefully to what they say, drawing out those members who are not speaking, and gently moving group meetings and/or project/taskforce initiatives along when a member monopolizes a conversation.

Follow-Up Deliverables

Post-Event Hot Wash / After-Action Analysis / After Action Reports
- after the session is over, a complete report describing everything that went on in group meetings and/or project/taskforce initiatives shall be prepared. An organized debrief post-meeting and provide post-meeting feedback session held to provide suggestions and improvements for future meetings. All action items agreed to by the participants are documented as part of this report.
Public Involvement And Communications
- foster collaboration and communication through open, ongoing and effective maintenance of knowledge repositories to include correspondence, presentations, meeting summaries, work products, and responses to information requests by ensuring distribution of requested information.
Document Retention And Knowledge Repositories
- live capture of meeting discussion/decisions in Microsoft word or excel is also expected. Maintain a library per established guidelines as a repository of related information co-located with the clients, program leadership, sub-group chairs, meeting participants, and other stakeholders as the official record required (i.e. Per Federal Advisory Committee Act - FACA) for an information repository.

Qualifications
A Bachelor's Degree or a minimum of 3 years' experience in a similar position.
Ability to foster collaboration and communication, lead coordination of meeting schedules, assist with the creation of agendas, facilitate group meetings and/or project/taskforce initiatives and distribute of meeting minutes are expected.
Ability to live capture of meeting discussion/decisions in Microsoft word or excel and able to summarize chunks of information for clarity and understanding.
Combination of education and experience that provides the individual with the required knowledge, skills, and abilities minimum of three (3) years industry experience, with a thorough knowledge of the project management discipline and demonstrated ability to apply within a complex organization.

We are currently seeking a compassionate and experienced fully licensed Therapist with a background in adolescent/teens mental health treatment to join our clinical team.
Position Overview: The Primary Therapist will be responsible for providing individual, family and group therapy to residents, developing and implementing individualized treatment plans, and collaborating with other members of the clinical team to ensure holistic and effective care for our residents.
Key Responsibilities:
Individual and Group Therapy:
Conduct individual therapy sessions with residents to address their mental health needs and support their therapeutic goals.
Facilitate group therapy sessions to foster peer support and address common issues among residents.
Treatment Planning:
Develop and implement individualized treatment plans based on comprehensive assessments and ongoing evaluations.
Set measurable goals and objectives for each resident, and regularly review and update treatment plans as needed.

Group dynamics can be difficult at best when critical decisions need to be made. A Group Meeting Facilitator interacts with groups that need coaching to work productively as a team to reach a common goal. The group meeting facilitator guides discussions of the topics laid out in group meetings and/or project/taskforce initiatives agenda, with the purpose of moving the team towards a common goal.
A skilled Group Meeting Facilitator fosters collaboration and teamwork and manages conflict among team members. The ProSidian Consultant | Group Meeting Facilitator will coordinate, lead, and support all phases of team meetings to ensure progression and completion of work as well as supports the Global Project Lead (GPL) And Project Team, and is accountable for the quality, timeliness, and effectiveness of group meetings and/or project/taskforce initiatives and ends with delivery of the final outputs that achieve stated mission objectives.
The job of a “Group Meeting Facilitator” is more of the "conductor" than a participant throughout the Program Period. The ProSidian Group Meeting Facilitator shall serve as a consultant to clients across private companies, fortune 1,000 enterprises, and government agencies of all sizes. You direct the flow and energy throughout the program period. You observe participants' body language, listening carefully to what they say, drawing out those members who are not speaking, and gently moving group meetings and/or project/taskforce initiatives along when a member monopolizes a conversation.

Develop and support the approval of individualized Plans of Action and Milestones (POA&M) incorporating requirements of program leadership, sub-group chairs, and meeting attendees. The plan shall be designed to capitalize on the strengths while ensuring coordination of both professionally based principles and support tools, technology, and processes.
Topic/Agenda Acceptance - work with groups to clarify meeting content, objectives and to establish ground rules that govern behavior. Review information from previous meetings and develop agenda/desired outcomes. Follow up on action items. Introduce a variety of strategies to keep the process structured without interfering with group productivity. Following acceptance, pre-work is done includes interviewing program leadership, sub-group chairs, and meeting attendees to gain consensus on the topic/agenda for each program session
Group Meeting Agendas - work with clients, meeting participants, and stakeholders to develop agendas (adequately estimated for time frames) that will achieve stated goals and objectives. When desired, and as needed, introduce dialog, decision-making, and other communication models and tools as part of the agenda or as just-in-time training to enhance the process and achieve desired results.
Client Conference/Program Participation - select appropriate methods and tools that will enable participants to achieve results. Listen and proactively respond to client and stakeholder intentions and desires to enhance the process and achieve desired results. Help clearly define stated goals and objectives for success, both in group meetings and/or project/taskforce initiatives and beyond.
Ongoing Course Adjustments - consultative facilitation while working in close partnership with clients, meeting participants, and stakeholders throughout the program period; making the best possible use time and resources; correctly observing interests and developments, and working to enhance the process and achieve desired results. When problems or issues come up, work to resolve conflict and return to goals.
Dynamic Process Interactions - create environments that allow civil dialogue and input from all participants and above all -- remaining neutral throughout the process. Recognize that group meetings and/or project/taskforce initiatives are dynamic processes and that new insights and ideas can emerge that could significantly shift original intentions. Work to respond to those emerging trends, needs, and themes while keeping everyone focused on the clearly defined meeting objectives while achieving desired results.
Leadership Coaching/Support Initiatives - work to ensure successful implementation of primary objectives through the facilitation of cross-functional meetings and internal department level meetings. Provide real-time coaching to clients, program leadership, sub-group chairs, meeting participants, and other stakeholders on ways to enhance group meetings and/or project/taskforce initiatives focused on the clearly defined meeting objectives that achieve desired results.
Follow-Up Deliverables - work to improve the contributions from meeting participants; coordinate meeting schedules; provide assistance with creation of agendas; facilitate meetings; distribute meeting minutes, and maintain task and issues logs.
Post-Event Hot Wash / After-Action Analysis / After Action Reports - after the session is over, a complete report describing everything that went on in group meetings and/or project/taskforce initiatives shall be prepared. An organized debrief post-meeting and provide post-meeting feedback session held to provide suggestions and improvements for future meetings. All action items agreed to by the participants are documented as part of this report.
Public Involvement And Communications - foster collaboration and communication through open, ongoing and effective maintenance of knowledge repositories to include correspondence, presentations, meeting summaries, work products, and responses to information requests by ensuring distribution of requested information.
Document Retention And Knowledge Repositories - live capture of meeting discussion/decisions in Microsoft word or excel is also expected. Maintain a library per established guidelines as a repository of related information co-located with the clients, program leadership, sub-group chairs, meeting participants, and other stakeholders as the official record required (i.e. Per Federal Advisory Committee Act - FACA) for an information repository.
Program/Recommendations Implementation - help participants reach decisions and accomplish goals. Help facilitate group meetings and/or project/taskforce initiatives, gain consensus, and manage the execution of action items, recommendations, and resolutions through counsel and support to clients, program leadership, sub-group chairs, meeting participants, and other stakeholders.
Qualifications
A Bachelor's Degree or a minimum of 3 years' experience in a similar position.
Ability to foster collaboration and communication, lead coordination of meeting schedules, assist with the creation of agendas, facilitate group meetings and/or project/taskforce initiatives and distribute of meeting minutes are expected.

IVA'AL Solutions, LLC provides support to the Air Force (AF) Family Advocacy Program (FAP) and is responsible for staffing qualified Licensed Clinical SocialWorkers, Licensed Registered Nurses, Certified Victim Advocates, and Program Assistants at U.S. AF Military Treatment Facilities (MTFs) across the country. The AF FAP is a Department of Defense, (DoD) funded program whose purpose is to both prevent and respond to intimate partner abuse, child abuse, and problematic sexual behavior with children and youth.
The Family Advocacy Treatment Manager (FATM)
is a member of the multidisciplinary Family Advocacy Program (FAP) team and provides the following services:
Assess and treat individuals, families, and groups whose maltreatment allegation(s) meet criteria for maltreatment.
Provide voluntary, comprehensive prevention services to prevent the occurrence of family maltreatment.
The FATM is the primary provider of treatment services for assigned maltreatment and prevention cases, coordinating services for families and monitoring client participation and progress utilizing the modalities of social casework, psychotherapy, and psycho-educational interventions.
The FATM completes psychosocial assessments in accordance with Air Force Family Advocacy Policies and Standards.
The FATM assesses for safety and continuously assist families with safety planning.
The FATM evaluates the effectiveness of the services provided.
Requirements
Education and Experience/Qualifications:
Master's degree from a Council on SocialWork Education (CSWE) accredited school.
Licensed Clinical SocialWorker at the independent level with current, unrestricted State license.
Must possess two years' full-time post-master's degree experience (within the last three years) providing clinical counseling services to adults and children experiencing family violence.
Must have experience as a group therapy facilitator or co-leader.
Must be able to obtain privileges at the Military Treatment Facility (MTF).
Must obtain and maintain Basic Life Support Certification (Course C).
Must have transportation and a valid drivers' license.
Technical Skills:
Must be computer-proficient to work autonomously using Microsoft Office and possess data entry skills needed to create and maintain clinical records.

REPORTING TO: Mental Health Program Development Manager

JOB SUMMARY: The Licensed Clinical SocialWorker (LCSW) at the Health Center is a key member of our integrated care team, responsible for delivering high -quality, patient -centered mental health services. This role involves providing evidence -based psychotherapy, crisis intervention, and collaborating with primary care providers, case coordinators, and community health workers.
Duties and Responsibilities:
Clinical Duties:
Deliver evidence -based psychotherapy to individuals, couples, and families.
Collaborate with primary care providers to conduct warm handoffs for timely mental health interventions.
Perform crisis interventions and create safety plans as needed.
Maintain accurate, timely clinical documentation in the Electronic Medical Record (EMR) system, adhering to FQHC and state regulations.
Uphold patient confidentiality and comply with HIPAA regulations and quality management policies.
Maintains all BLS certifications and state licensure up to date
Administrative Duties:
Assist with the completion of intake forms and other clinical documentation.
Participate in multidisciplinary team meetings to develop and update integrated care plans.
Participate in quality improvement activities to align mental health services with the center's mission and values.
Ensure all clinical services meet the Health Resources and Services Administration (HRSA) Health Center Program standards.
Stay informed about and comply with all state of Nevada FQHC requirements, including billing, reporting, and documentation standards.

The LCSW ensures compliance with state and FQHC regulations and adapts to evolving program requirements, supporting our mission to offer holistic care to underserved populations.
The LCSW ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
The LCSW is also expected to actively participate in and support quality improvement activities and the clinical practice team efforts emphasizing the importance of the individual patient and putting the patient's needs first.
Requirements
Qualifications:
Master's degree in socialwork from an accredited institution - required
Active LCSW license in Nevada - required
At least 2 years of clinical experience with a focus on psychotherapy - preferred
Experience working in an integrated care setting, preferably within a primary care environment - preferred
Familiarity with FQHC regulations and requirements - preferred
Fluency in Spanish and English - preferred
