What You Will Do
Delivers outreach services to mentally ill individuals in the community, providing in-home mental health intervention to those experiencing or at risk of psychosocial difficulties

Key Responsibilities:

assess patients and complete comprehensive psychosocial evaluations,
work with patients to develop patient-centered treatment plans,
conduct individual counseling sessions,
facilitate weekly group therapy sessions,
make appropriate referrals for services as necessary,
timely and properly complete documentation in accordance with OASAS regulations.
Requirements
Associate's Degree (Bachelor's Degree preferred),
CASAC or CASAC-T (must be in full effect, "pending" not accepted),
two years' experience,
excellent written and interpersonal communication skills,
ability to work efficiently as part of an integrated team in a fast-paced environment,
sensitivity to the complex needs of patients in treatment for substance use disorder,
bi-lingual (English-Spanish) preferred,
Monday-Friday 7:00 am-3:00 pm.

Salary Range
$54,787.00 - $68,099.00 per year
