May obtain patient's vital signs, height and weight as assigned and document in patient record.
Facilitate patient educational-rehabilitative groups which cover a variety of topics including social skills, coping skills, anger management and independent living skills.
Engage patients in activities and interactions designed to encourage achievement of treatment goals.
Complete and maintain required documentation. Assist with follow-up and paperwork as required on incidents and events that may take place in the facility.
May provide transportation for patient or coordinate transportation with appropriate staff member.

Position is Full-Time
8 hour shifts: Days, evenings, and nights. Everyother weekend required.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
· High school diploma or equivalent required.
· Six months or more experience working with the specific population of the facility preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
· CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
· First aid may be required based on state or facility requirements.

Provide domestic violence and sexual assault advocacy at the individual and system level in English and Spanish.
Conduct safety planning, assessment of immediate needs and assist Domestic Violence and/or Sexual Assault survivors with determining next steps to ensure safety following the incident.
Assist survivors of crime when requested by law enforcement at crime scenes, hospitals or other safe locations. Accompany survivors to court, legal, medical when appropriate.
Provide survivors with support, referrals, linkages, and information related to community resources, crime victim's rights, crime victim compensation information and applications, and orders for protection as appropriate.
Recruit participants for and facilitate support groups for survivors of sexual assault and/or domestic violence. Coordinate logistics for support groups including but not limited to location, food, participant transportation, and childcare room volunteers (approved through CLUES Volunteer Services) as assigned.
Provide crisis intervention 24/7 for survivors of sexual assault and answer hotline when on on-call rotation.
Drive and transport clients in personal motor vehicle when appropriate while maintaining all necessary safety precautions (wearing seatbelts, observing traffic laws, using properly installed car seats for small children, etc.).
Maintain accurate and up-to-date records and necessary paperwork, including case note/case history records, documentation of services and referral provided, and other reporting as assigned.
Collect data and provide reporting on program as assigned. Prepare statistical and written reports as required by department and funding sources in a timely manner (e.g., monthly, quarterly & yearly).
Provide outreach and education to social services providers and general community about Domestic Violence and Sexual Assault.
Collaborate with supervisor and team in policy and systems change efforts to raise community awareness regarding sexual violence in the community. Assist in developing and organizing annual sexual and domestic violence prevention and awareness campaign.
Participate in community meetings, forums, trainings and collaborations as required.
Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards.
Complete required trainings, including sexual assault 40 hours training.
Carry out other duties as assigned.

This position has the responsibility for assuring compliance with the Resident's Bill of Rights, the Patient's Self-Determination Act as well as meeting the requirements of the State and Federal regulations and facility policies and procedures on a continuing basis.
This position cooperates with other facility disciplines in the carrying out of responsibilities and assuring that the needs of the residents are being met in a pleasant working atmosphere.
NATURE AND SCOPE
This position reports to the Healthcare Administrator.
In performance of duties this position is entrusted with, but not limited to, the following responsibilities:

Assess the psycho-social strengths and needs of residents on the unit to which the socialworker is assigned.
Develop and maintain a social history, which becomes a part of the resident's permanent medical record.

Assist in orientation of families to the facility.

Complete required documentation, including MDS and quarterly summaries.

About this position
The Licensed Graduate SocialWorker (LGSW) provides services to customers and their families to help them adjust to physical or mental illness, injury or hospitalization. Assists the interdisciplinary treatment team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for the biopsychosocial assessments and discharge planning for customers with complex psychosocial and medical and/or mental health problems in collaboration with the responsible family and case management team. Facilitates psychosocial coordination including assessment, planning, intervention and evaluation of customer/family needs throughout the hospital stay. Identifies needed community services and coordinates appropriate services for customers in need.
In addition to the above position purposes, SocialWorkers are available to assess the behavioral and psychological needs of customers. They take into consideration a customer's presenting concerns in cases related to psychiatric disorders, chemical health issues and emotional/behavioral health. They are responsible for the discharge planning and case management for the customers they are assigned.

Education
* Masters Degree from an accredited university or college in socialworkworking towards license as an Independent Clinical SocialWorker and fulfills the requirements as set forth by the Minnesota Board of SocialWork.
Experience
* Socialworkers who are required by clinical assignment to psychiatrically diagnosis or engage in the practice of psychotherapy shall have a minimum of 4000 hours of post-graduate clinical social mental health professional as defined by Minnesota statute

This is a 0.6FTE (6 shifts per two week pay period) Acute Care Float SocialWork position at Fairview- The University of Minnesota Medical Center! This position floats to cover various units throughout the hospital including the ICU, Cardiology, Neurology, Ortho, Med/Surg, Observation and Emergency Departments. The hours are 8a-4:30pm. There is an every 6th weekend rotation and 1 holiday per calendar year requirement.
The acute socialworkerworks on-site with the interdisciplinary teams including providers, physical therapists, nurses, dieticians, chaplains and pharmacists to assist patients and families in navigating through their hospitalization. The acute socialworker completes psychosocial assessments, care planning/ progression and creates comprehensive discharge plans from the hospital. The socialworker is patient facing, responds to crisis situations, manages patient and family expectations, creates solutions to identified discharge barriers and aims to reduce length of stay and prevent avoidable hospital readmissions.
Responsibilities
* Attends daily in person interdisciplinary care team rounds
* Conducts thorough psychosocial assessments with patients and/ or their surrogate decision makers
* Helps the care team navigate and mediate complex psychosocial dynamics
* Is well versed in the surrogate decision maker policy and assists care team members with identifying surrogate decision makers according to the policy
* Communicates frequently with the interdisciplinary team to ensure continuity and effective coordination of care
* Provides emotional support and counseling surrounding adjustment to sudden illness, grief/bereavement and end of life planning
* Addresses social determinants of health and assists with connecting patients to needed resources in the community such as chemical dependency treatment, domestic violence resources, housing stabilization services, mental health services, food shelves, transportation, financial assistance etc.
* Arranges and facilitates in person care conferences as needed to support patients and their families surrounding navigating complex medical decision-making processes/ goals of care
* Documents all interactions thoroughly and timely following established department standard work
* Serves as the expert in system adult and child protection reporting policies
* Collaborates with patients, families and interdisciplinary teams in developing an individualized discharge plan based on age, social and cultural considerations, clinical diagnoses, level of functioning and other relevant needs
* Participates in hospital initiatives and projects to reduce length of stay and related barriers which may include collaborating with providers for clinical adjustments in care
* Assists with ensuring the hospital is in regulatory compliance for care management related conditions of participation
* Assists with efforts to improve patient flow in a high-capacity hospital setting
* Works in a fast-paced hospital environment
* Effectively re-prioritizes competing time sensitive job demands
* Demonstrates the ability to advance evidence-based practice by utilizing standards, guidelines and pathways for care delivery
* Incorporates data and information into practice to continually improve care and practice to enhance patient outcomes
* Supports site and system quality initiatives
* Provide age appropriate short- and long-term therapeutic interventions
